The possible immunosuppressive action of antithyroid drugs in vivo has been assessed using Buffalo (Buf) strain rats with thyroiditis produced by neonatal thymectomy; this model shares many features with spontaneous thyroiditis in man. Both propylthiouracil and methimazole significantly reduced the severity of thyroiditis (P less than 0.01 compared to controls), irrespective of effects on thyroid status. Thyroidal Ox 8 (suppressor/cytotoxic) and W3/25 (helper) T-cells were reduced in number equally in treated animals, and thyroid follicular cell Ia expression was absent in a high proportion of…

OBJECTIVE: The study aims to investigate the impact of thymectomy (TX) on immune function and cytokines levels in patients with myasthenia gravis (MG). METHODS: A total of 44 patients with MG were enrolled in the study, divided into two groups based on the treatment they received: the MG-TX group (14 MG patients who underwent thymectomy) and the control group (30 MG patients who received drug treatment alone). Patients were assessed at baseline (0 months), 1 month, 3 months, 6 months, and 12 months post-treatment. The levels of T lymphocytes subtypes (CD3+, CD4+, CD8 + T cells, and the CD4+/CD…

This study aimed to evaluate surgical and oncological outcomes of robot-assisted (RATS) thymectomy in the treatment of large (diameter > = 5 cm) resectable thymomas. Clinical data of 301 thymectomies for thymoma, performed in 7 high-volume centers from 2010 to 2023, were retrospectively reviewed. 132 RATS thymectomies were performed: 59 (44.7%) for small and 73 (55.3%) for large thymomas. Conversion (p = 0.48), significant complications (p = 0.26), and hospital stay (p = 0.79) were similar in both groups. Overall, 191 large thymomas were operated on: 73(38.2%) by RATS, 118 (61.8%) by open app…

Autoimmune enteropathy (AIE) is a rare cause of chronic diarrhea associated with autoantibodies and susceptibility to other autoimmune diseases, such as rheumatoid arthritis, diabetes, autoimmune hemolytic anemia, and atopic dermatitis. While it is more common in children, the prevalence of AIE in adults is increasing. Due to the nonspecific nature of its presenting symptoms and the lack of consistent findings, AIE can be challenging to diagnose. Here, we present a 66-year-old male patient who presented to the emergency department with diarrhea and recurrent bilateral pleural effusions two mon

BACKGROUND: The continuous evolvement of minimally invasive thymectomy over the last decades has potential advantages over trans-sternal thymectomy with similar oncologic outcomes of thymoma and complete remission for myasthenia gravis patients. A variety of different minimally invasive approaches have been described previously. The aim of this article is to present our subxiphoid and subcostal approaches in thymectomy for patients with myasthenia gravis and thymomas and to investigate the early surgical outcomes of these patients.
METHODS: A retrospective analysis was performed of 95 patient…

The thymus is critical for central tolerance and diverse T-lymphocyte repertoire development, to provide lifelong defense against pathogens while maintaining self-tolerance. Peak thymic output occurs in utero, during infancy, and in early childhood, diminishing throughout life. Infants with congenital heart disease requiring sternotomy often undergo thymectomy to clear the surgical field. The long-term effects of early thymectomy are just being appreciated. Many patients remain asymptomatic despite immunologic findings mirroring those of immunosenescence. Few develop increased infection or lym…

OBJECTIVE: To evaluate transcervical and transsternal thymectomy benefits in large myasthenia gravis (MG) cohort.
METHOD: We retrospectively evaluated MG patients (n = 184) who had undergone thymectomy between 2004 and 2015 at National Institute of Mental Health and Neurosciences, Bangalore (India). Myasthenia gravis foundation of America guidelines were followed to assess clinical outcome. Anti-acetylcholine receptors (AChR) antibodies, repetitive nerve stimulation (RNS) and Neostigmine tests were performed at pre and post-thymectomy stage.
RESULTS: Most of the patients were fell under MG g…

BACKGROUND: To present the technique of minimally invasive extended thymectomy performed through the subxiphoid-bilateral subcostal video-assisted thoracoscopic surgery (VATS) approach, with double elevation of the sternum for nonthymomatous myasthenia gravis (MG).
METHODS: The whole dissection was performed through the 4-7 cm transverse subxiphoid incision with single 10 mm extra-long bariatric laparoscopy ports inserted subcostally to the right and left chest cavities for videothoracoscope and subsequently for chest tubes. The sternum was elevated with two hooks connected to the sternal fra…

BACKGROUND: In parathyroid hyperplasia (HPT), parathyroid glands within the cervical thymus are a cause for recurrence. As a result of differences in pathophysiology, variable practice patterns exist regarding performing bilateral cervical thymectomy (BCT) in primary hyperplasia versus hyperplasia from renal failure or familial disease. The objective of this study was to capture patients where thymic tissue was found with subtotal parathyroidectomy (PTX) and intended BCT, identify number of thymic supernumerary glands (SNGs), and determine overall cure rate.
METHODS: Retrospective review of p…

A 46-year-old woman was referred to our hospital for a tumor in the anterior mediastinum. She had no symptoms of myasthenia gravis(MG). Acetylcholine receptor antibody(AchR-Ab) was within the normal range. The tumor was completely resected by thymectomy. Pathological examination of the tumor identified it as a type AB thymoma (Masaoka stage I). Five days after the surgery, she experienced post-thymectomy MG (raised AchR-Ab and positive tensilon test). Her symptoms improved with anti-cholinesterase and tacrolimus therapy.

BACKGROUND: In thymoma patients without myasthenia gravis, it is debatable whether thymectomy should be performed in addition to thymomectomy, the procedure in which the thymoma alone is resected. In this study, we proposed to compare the surgical results in early-stage nonmyasthenic thymoma patients who underwent thymomectomy with and without extended thymectomy.
METHODS: A total of 95 patients without clinical evidence of preoperative myasthenia gravis, who underwent surgery for early-stage thymoma (stages I and II), were selected for the study. Thymomectomy with extended thymectomy was per…

Recently, the prognosis of myasthenia gravis (MG) has improved considerably because the commercial measurement of acetylcholine receptor (AChR) antibody has been introduced and early treatment with steroid at large quantities has been utilized. On the other hand, the efficacy and the safety of thymectomy for MG patients without thymoma are still controversial. A meta-analysis of thymectomy for MG patients without thymoma revealed an uncertain effect. Since 2005, a controlled randomized study for thymectomy in patients with MG has been in progress at 66 centers in 16 countries. This study compa…

PURPOSE: Transsternal thymectomy is well established in the treatment of myasthenia gravis (MG). The objectives of this study were to evaluate the influence and prognostic factors of thymectomy as treatment for MG.
PATIENTS AND METHODS: Surgical results of 54 patients with MG who underwent transsternal thymectomy were retrospectively reviewed. We investigated clinical outcomes of extended transsternal thymectomy in MG, and we analyzed the data to clarify the effect of prognostic factors on clinical outcome.
RESULTS: A total of 54 patients, including 28 males and 26 females, were analyzed. At…
