Reimplantation of an avulsed mature permanent tooth after 6 days: a 1-year follow-up.

Tooth avulsion in children and adolescents demands emergency treatment to increase the chances of successful reimplantation. The treatment prognosis depends on intrinsic and extrinsic factors, such as the extra-alveolar time until reimplantation, storage medium, handling, and periodontal ligament condition of the avulsed tooth as well as the patient's general health. This case report describes the treatment of an 11-year-old boy who suffered an avulsion of the maxillary right central incisor followed by delayed reimplantation after the tooth was stored in a dry medium for 6 days. Although the

Clinical management of a complicated crown-root fracture: a case report.

This report describes the clinical procedures involved in the treatment of a complicated crown-root fracture in the maxillary left central incisor with a wide open apex of a 10-year-old male patient, due to fall from his own height. Post-trauma treatment comprised cervical pulpotomy and adhesive tooth fragment reattachment. After 1 year, clinical and radiograph examinations showed pulp necrosis and an associated periapical lesion. Endodontic therapy with calcium hydroxide-base intracanal dressing, root canal filling and orthodontic extrusion were performed. Extrusion was completed within appro

Clastic cells are absent around the root surface in pulp-exposed periapical periodontitis lesions in mice.

INTRODUCTION: Clastic cells, originating from the monocyte-macrophage lineage, resorb mineralized tissues. In periapical periodontitis, alveolar bone around the tooth apex becomes resorbed; however, the roots of the teeth are often left intact by yet unknown mechanisms. Here, we examined the status of clastic cells in a periapical periodontitis model in mice. METHODS: Periapical periodontitis was induced by performing pulp exposure on the maxillary first molar. The contralateral maxillary first molar was used as a control. The maxillae were harvested, fixed, and subjected to μCT scanning and …

Novel management of internal root resorption with vertical root fracture using mineral trioxide aggregate - A case report.

INTRODUCTION: This case report presents the diagnosis and management of extensive internal root resorption (IRR) in a 17-year-old male patient, with a 9-year-old history of trauma. METHOD: The affected tooth 21 was associated with vertical root fracture (VRF) and incomplete apex closure with a substantial loss of tooth structure, including dentin and cementum. Encouraged by a healthy periodontal condition, the choice was made to use mineral trioxide aggregate (MTA) to reconstruct and reinforce the resorptive defect and the fractured segment. A composite veneer was placed to enhance the aesthe

Case of hidden tooth: mesiodens fortuitously discovered on a cone-beam CT examination.

Mesiodens is the most common type of supernumerary tooth, located between the maxillary central incisors. A young man was referred by his orthodontist for management of a supernumerary tooth located in quadrant I, superposed to the bottom of the right maxillary sinus, distally orientated with the crown in contact with the apex of the palatal root of the maxillary first molar. The tooth was found on a panoramic radiography before starting his orthodontic treatment. To remove it and in order to study, its relationship to the anatomical structures a cone-beam CT examination was performed. This re

Micro-CT evaluation of canal shaping extent with apex locator-integrated endodontic motors: An in vitro study.

INTRODUCTION: This in vitro study evaluated the extent accuracy of canal shaping to apical constriction (AC) with electronic apex locator (EAL)-integrated endodontic motors using micro-computed tomography (MicroCT). MATERIALS AND METHODS: Sixty single-canalled teeth were decoronated, scanned using microCT, and distributed into three groups: Rooter Universal, Tri Auto ZX II, and control group (n = 20 each). The working length (WL) was determined simultaneously during canal shaping using the auto-reverse function set at the 0.5 mark in the RU and TA groups. In the control group, the WL was meas

Working length variation during multiple-visit endodontic treatment: An observational study.

This study investigated variation in working length during multiple-visit endodontic treatment. Patients (N = 106) with pulpitis or pulp necrosis, with or without apical periodontitis, were included. During the first appointment, glide path was performed with ProGlider and shaping with ProTaper Next. Working length was detected four times. Working length was then re-recorded prior to filling during the second appointment and a 0.5 mm threshold was selected as the minimum clinically meaningful variation. A logistic regression model was used to evaluate the impact of tooth anatomy, preoperative

Histopathological Investigation of Dental Pulp Reactions Related to Periodontitis.

OBJECTIVE: Since the 1960s, there has been contradictory evidence regarding the association between periodontal pathology and the status of the pulp. The purpose of this study was to evaluate the histopathological changes of pulp tissue with severe periodontal disease, including vertical bone loss involving the major apical foramen, and compared them with the histological pulpal status of teeth with healthy periodontium. METHODS: This case-controlled study included 35 intact teeth with severe periodontitis of hopeless prognosis (test group) and 35 teeth without periodontitis extracted for ort

Impact of remnant healthy pulp and apical tissue on outcomes after simulated regenerative endodontic procedure in rat molars.

When regenerative endodontic procedures (REPs) are performed on immature teeth diagnosed with pulp necrosis and apical periodontitis, various healing patterns occur. Furthermore, infected immature teeth with endodontic disorders often exhibit some remnant pulp and apical tissue. Therefore, this study investigated the impact of remnant healthy or fully functional pulp and apical tissue on healing patterns after REPs. Simulated REPs were performed on non-infected immature rat molars with different amounts of remnant pulp and apical tissue. Healing patterns in these teeth were assessed after 28 d

Comparative evaluation of the efficacy of three methods of delivering calcium hydroxide into the root canal.

BACKGROUND: Calcium hydroxide, due to its favorable properties, such as an antimicrobial effect as well as its ability to dissolve necrotic tissues, inhibit tooth resorption and stimulate the formation of a hard tissue barrier, is widely used in root canal treatment. OBJECTIVES: The objective of this study was to compare the efficacy of 3 methods of delivery of calcium hydroxide into the canals. MATERIAL AND METHODS: This in vitro experimental study was performed on acrylic resin blocks with simulated curved canals (curvature of 24° and 44°). Calcium hydroxide was delivered into the canals w

Apical resorption in teeth with periapical lesions: correlation between radiographic diagnosis and SEM examination.

This study correlated the radiographic findings and scanning electron microscopy (SEM) images of external apical resorption (EAR) in teeth with periapical lesions. Standard radiographs were taken from 45 teeth with periapical lesions before their extraction. Using a radiographic examination, the root apex of each tooth was classified according to the EAR into the following categories: radiographic external apical resorption (REAR) absent, superficial and deep. The apical root surface was also examined with SEM. Photomicrographs of EARs were classified as periforaminal (PEAR) and foraminal (FEA

Association of mesiodentes and dens invaginatus in a child: a rare entity.

Supernumerary teeth are defined as any teeth in excess of normal number. Mesiodens is a supernumerary tooth, in the central region of premaxilla between two central incisors. Dens invaginatus is a developmental anomaly resulting from invagination in the surface of tooth crown before calcification has occurred. Radiographically, it is observed as infolding of a radioopaque ribbon like structure, with equal density as enamel, extending from cingulum into a root canal and sometimes reaching the root apex. This paper aims to present a rare association of dens invaginatus with two mesiodentes in a

[Severe complication of a bonded mandibular lingual retainer].

Bonding a flexible spiral wire retainer to the lingual surfaces of all six anterior mandibular teeth is a commonly used type of retention. Complications are rare but can be serious enough to produce biologic damage. This article presents a serious complication of a lingual flexible spiral wire retainer. Four years after the orthodontic treatment, a 20-years-old man sought treatment for a broken flexible spiral wire retainer. The clinical examination showed about 35° of buccal root torque of that tooth. A cone-beam computed tomography image showed that the root and the apex of the tooth were al

Treatment of necrotic calcified tooth using intentional replantation procedure.

Introduction. If the teeth are impacted by a chronic irritant, the pulp space possibly will undergo calcific changes that may impede access opening during root canal treatment. In such cases that conventional endodontic treatment is impossible or impractical, intentional replantation may be considered as a last solution to preserve the tooth. Methods. After failing to perform conventional root canal therapy for a necrotic calcified right mandibular second premolar, the tooth was gently extracted. The root apex was resected and the root end cavity was prepared and filled with calcium enriched m

Heritability of teeth angulation and inclination in patients with overt malocclusion.

PURPOSE: To use 3-dimensional cone beam computed tomography (CBCT) images obtained on patients before treatment with dental malocclusion to calculate the heritability estimates of full-tooth angulation and inclination. A heritability estimate is a number between zero and one indicating the percentage of genetic factors involved in determining the phenotypic variation observed. METHOD: CBCT images of 111 patients (51 sibships) were analysed measuring both tooth angulation and inclination using the Anatomage Invivo5 software for central incisor to second molar. The long axis of each tooth was d
