Is Hypoxia Related to External Cervical Resorption? A Case Report.

Despite the fact that external cervical resorption (ECR) is a well-known and rather frequently met condition, the driving force of this phenomenon still remains unclear. Recently, hypoxia has been linked to ECR. Thus, the aim of this work was to investigate the existence of hypoxia in ECR and hypothesize on its role at the time of extraction. This work is a case study of a tooth with ECR. ECR diagnosis was based on clinical and radiographic examination with cone-beam computed tomographic imaging. The extracted tooth was further analyzed by using nanofocus computed tomographic imaging and immun

The prevalence and severity of non-carious cervical lesions in a group of patients attending a university hospital in Trinidad.

Non-carious cervical lesions (NCCLs) are often encountered in clinical practice and their aetiology attributed to toothbrush abrasion, erosion and tooth flexure. This paper aims to determine the prevalence and severity of NCCLs in a sample of patients attending a university clinic in Trinidad and to investigate the relationship with medical and dental histories, oral hygiene practices, dietary habits and occlusion. Data were collected via a questionnaire and clinical examination. Odds ratios were used to determine the association of the presence of lesions and the factors examined. One hundred

CBCT Use in Endodontic Diagnosis.

Endodontic diagnosis and treatment planning has taken a giant leap forward due to introduction of CBCT in dentistry. While conventional 2-D radiographs remain the most cost-effective and routine method to evaluate a patient’s dentition, their diagnostic potential is limited. The 3-D manipulation of images that CBCT offers provides better insight into diagnostic dilemmas and complicate treatment decisions. Despite the advantages of CBCT imaging, it should be used complimentary to 2-D radiography, not as a replacement. The principle of ALARA (in which patients should be exposed to radiation “as

Distal cervical caries in the mandibular second molar: an indication for the prophylactic removal of the third molar?

AIMS: Distal cervical caries (DCC) in mandibular second molar teeth are responsible for the removal of up to 5% of all mandibular third molars. Our aim was to identify the clinical features of these patients. METHODS: We evaluated the records of 100 patients who had 122 mandibular third molars removed because of distal cervical caries in the second molar. RESULTS: Eighty-two percent of third molars had a mesial angulation of between 40 degrees and 80 degrees. The peak age for removal of third molars was 5 years later than in other studies and patients had better dental health than average. T

Bone morphology after delayed tooth replantation - case series.

The purpose of this report was to describe the morphological changes in the alveolar bone after delayed replantation of avulsed teeth using three dimensional cone-beam computed tomography in 11 during the time period 2003-2012. The radiographic observations revealed the following: Delayed replantation results in ankylosis-related replacement root resorption; the resorption is delayed or arrested around the cervical area superior to the alveolar crest. The buccal bone is reduced in thickness but not the palatal bone. The buccal bone resorption of the alveolar crest progresses approximately to t

Histometric study of resorption on replanted teeth with enamel matrix-derived protein.

BACKGROUND: Enamel matrix-derived proteins (Emdogain) stimulate the proliferation of periodontal ligament cells, contributing to their regeneration. AIM: This study was to perform histometric assessment of root surface resorption in replanted teeth with the use of Emdogain®. MATERIALS AND METHODS: Male Wistar rats (Rattus norvegicus), weighing 180 to 200 gm, were anesthetized, subjected to extraction of the upper right incisor tooth, which was then replanted into the alveoli with the following treatment: (a) control group--tooth with root canal filled with calcium hydroxide paste and (b) tre

Stress distribution in restored mandibular molars with external cervical resorption: a finite element analysis.

This study evaluates the stress distribution in mandibular molar teeth with external cervical resorption restored using various restorative and biomimetic materials through finite element analysis. A mandibular first molar was scanned to create a 3D model. 'Class 2Bp' external cervical resorption defects were simulated on the buccal aspect and restored with different materials. Composite resin or highly viscous glass ionomer cement restored the coronal portion, while mineral-trioxide-aggregate, Biodentine, BioAggregate, or glass-ionomer cement filled the radicular portion. Models representing

Non-carious cervical lesions associated with multiple gingival recessions in the maxillary arch. A restorative-periodontal effort for esthetic success. A 12-month case report.

Restoration of non-carious cervical lesions (NCCLs) represents a major challenge for resin materials due to the different adhesive properties of the tooth structure, the biomechanical aspects of the cervical area, and the difficulties in the access and isolation of the operative field. Furthermore, NCCLs should be approached with a complete understanding of the role played by the marginal periodontal tissue. Whenever a cervical lesion is associated with a gingival recession, the interplay between restorative dentistry and periodontology is decisive for full esthetic and long-term success. A ca

Clinical and anatomical factors limiting treatment outcomes of gingival recession: a new method to predetermine the line of root coverage.

Complete root coverage is not always achievable, even in gingival recession with no loss of interproximal attachment and bone. The cemento-enamel junction is the most widely used referring parameter to evaluate root coverage results. The aim of the present study was to describe the most frequent diagnostic mistakes that may lead to incomplete root coverage in Miller Class I and II gingival recessions and to suggest a method to predetermine the level/line of root coverage in non-molar teeth. The line of root coverage (i.e., the level/line to which the soft tissue margin will be positioned after

An Analysis of the Stress Induced in the Periodontal Ligament during Extrusion and Rotation Movements: A Finite Element Method Linear Study Part I.

BACKGROUND: Orthodontic tooth movement occurs due to various biomechanical changes in the periodontium. Forces within the optimal range yield maximum tooth movement with minimum deleterious effects. Among various types of tooth movements, extrusion and rotational movements are seen to be associated with the least amount of root resorption and have not been studied in detail. Therefore in this study, the stress patterns in the periodontal ligament (PDL) were evaluated with extrusion and rotational movements using the finite element method FEM. MATERIALS AND METHODS: A three-dimensional (3D) FE

The prevalence of cervical tooth wear in patients with bruxism and other causes of wear.

PURPOSE: The aim of this study was to investigate the prevalence of cervical wear lesions in three groups of patients: bruxists, combined tooth wear, and controls. The hypothesis was that those subjects presenting with bruxism were more likely to develop cervical wear lesions. MATERIALS AND METHODS: Of 119 subjects, 31 were bruxists with a mean age 48.7 years [standard deviation (SD): 11.6]; 22 had combined wear, aged 43.5 years (14.2); and 66 controls aged 44.9 years (17.0). The clinical appearance of the tooth wear was used to recruit subjects to the bruxist and combined tooth wear groups.

External cervical resorption associated with the use of bisphosphonates: a case series.

Although the pathogenesis and etiology of external cervical resorption (ECR) are not well understood, several predisposing factors have been reported to potentially contribute toward the initiation of ECR. However, a potential link between systemic medication and ECR has not been reported. Bisphosphonates (BSPs) are 1 group of these medications (the amino-containing BSP), which are associated with an acute-phase response and the release of proinflammatory cytokines, which could be associated with the initiation of ECR. Therefore, a possible causal link between BSPs and ECR could be drawn.

Invasive Cervical Resorption: Clinical Management in the Anterior Zone.

According to the high number of articles published on invasive cervical resorption (ICR), this pathology, as commonly believed, is a more frequent form of cervical resorption. ICR is often misdiagnosed as internal resorption or caries, which leads to inappropriate treatment and even unnecessary tooth loss. Despite a correct diagnosis, the treatment of this type of hyperplastic invasive external resorption poses a challenge for the clinician. The Heithersay classification and the use of cone-beam computed tomographic imaging have increased our knowledge of the pathology and helped improve its p
