[Hemiplegic forms of human African trypanosomiasis].

Hemiplegic forms of human African trypanosomiasis are unusual. From 1963 to 1987, 14 cases have been reported in the literature. One may be mistakenly led to look for a space-occupying lesion when clinical features include hemiplegia, vascular shift from median line during arteriography, focal EEG anomalies and intracranial hypertension. The discussion on diagnosis also covers subacute or chronic meningo encephalitis (tuberculosis syphilis or fungal infection). CT scanner findings suggest the association of a massive demyelination of centrum semiovale, with cerebral oedema.

Macrophage activation in murine African trypanosomiasis.

African trypanosomiasis is accompanied by a profound general immunosuppression in which both suppressive T cells and macrophages (M phi) have been implicated. The present studies define changes in the M phi surface, endocytic and secretory properties, during the infection of mice by Trypanosoma brucei. Peritoneal M phi obtained after the control of the first wave of parasitemia displayed characteristics similar to those activated by intracellular pathogens, such as Mycobacterium bovis bacillus Calmette-Guérin, e.g., the enhanced expression of Ia antigen, decreased M phi-specific antigens, rece…

[Cutaneous lesions in human African trypanosomiasis].

We report the case histories of 12 patients infected by Trypanosoma gambiense seen between 1967 and 1979. The course of the disease follows … ad cutaneous lesions induced by melarsoprol. Thus dermatologic features are common in human African trypanosomiasis; they were found in 75% of the patients in our department.

[Hepatic involvement in Trypanosoma rhodesiense trypanosomiasis].

The authors report a clinical and biological study of 40 cases of T. rhodesiense trypanosomiasis. In 40% of cases there was a hepatomegaly, in 3 cases an ascitis, and biological abnormalities in 25% of cases. After having compared these results with other authors works (including T. rhodesiense and T. gambiense), they reached the conclusion that hepatic involvement in T. rhodesiense trypanosomiasis in frequent, leading sometimes to transient acute hepatic failure.

Experimental immunity against trypanosomiasis in sheep.

Three groups of male sheep were used. Group one was not inoculated; group two was inoculated with T. rhodesiense homogonate and group three with T. rhodesiense "fraction 3". Subsequently they were challenged with 1 x 10(6) T. vivax. Group inoculated with T. rhodesiense fraction 3 had high IgG levels and parasitemia was absent.

[Interpretation of the CATT (Card Agglutination Trypanosomiasis Test) in the screening for human trypanosomiasis due to Trypanosoma brucei gambiense].

Mass screening for Gambiense sleeping sickness is usually done with the Card Agglutination Trypanosomiasis Test (CATT) in series (total blood CATT followed by a serum CATT if the first test is positive) and the search for trypanosomes in cervical adenopathies. At present, the double positives (blood CATT and serum CATT) as well as the subjects in whom the trypanosome was found (in the blood or gland juice) are treated. The existence of patients whose gland punction was proved positive whereas the total blood CATT remained negative, has led the authors to make a survey with the CATT in parallel…

Transforming the chemotherapy of human African trypanosomiasis.

SUMMARYPrior to 2019, when the orally available drug fexinidazole began its clinical use, the treatment of human African trypanosomiasis (HAT) was complex and unsatisfactory for many reasons. Two sub-species of the Trypanosoma brucei parasite are responsible for HAT, namely the rhodesiense form found in East and Southern Africa and the gambiense form found in Central and West Africa. Diseases caused by both forms manifest in two stages: stage 1 before and stage 2 after central nervous system involvement. Prior to 2019, different drugs were required for each of the two parasite sub-species at e…

Generation of neuroinflammation in human African trypanosomiasis.

Human African trypanosomiasis (HAT) is caused by infection due to protozoan parasites of the Trypanosoma genus and is a major fatal disease throughout sub-Saharan Africa. After an early hemolymphatic stage in which the peripheral tissues are infected, the parasites enter the CNS causing a constellation of neurologic features. Although the CNS stage of HAT has been recognized for over a century, the mechanisms generating the neuroinflammatory response are complex and not well understood. Therefore a better understanding of the mechanisms utilized by the parasites to gain access to the CNS compa…

Fexinidazole for the treatment of human African trypanosomiasis.

On November 15, 2018, Fexinidazole Winthrop received a positive opinion from the European Medicines Agency (EMA) (under Article 58) for treatment of first-stage (hemolymphatic) and second-stage (meningoencephalitic) human African trypanosomiasis caused by Trypanosoma gambiense (gHAT) in adults and children 6 years and older and weighing 20 or more kg. This is the first oral regimen for gHAT that is effective in treating both disease stages. Although fexinidazole has potential to simplify current therapies, it does not entirely eliminate the need for disease staging by lumbar puncture because p…
