Lupus Retinopathy Mimicking Ocular Tuberculosis: A Diagnostic Dilemma.

Systemic lupus erythematosus (SLE) or Lupus is often called the "great mimicker" due to its ability to mimic other systemic disorders. SLE is a multisystem autoimmune disorder with varied manifestations, including ocular involvement. Retinal vascular occlusions are an uncommon but potentially vision-threatening presentation of lupus retinopathy. When coexisting systemic features mimic infectious etiologies such as tuberculosis (TB), diagnosis becomes challenging, delaying appropriate immunosuppressive therapy. We report a diagnostically complex case of a 22-year-old female who presented with s

Presumed Ocular Tuberculosis: Insights from an Egyptian Cohort.

PURPOSE: To report the clinical presentations and outcome of ocular tuberculosis (OTB) in an Egyptian cohort. METHODS: The medical records of 42 consecutive patients with presumed ocular tuberculosis referred to a tertiary referral center in Alexandria, Egypt, were reviewed retrospectively. RESULTS: Sixty-seven eyes of 42 patients were included in the study. The most common clinical presentation was anterior and intermediate uveitis in 25 eyes (37.3%), followed by anterior uveitis in 14 eyes (20.9%), intermediate uveitis in 11 eyes (16.4%), panuveitis in 10 eyes (14.7%), scleritis and sclero

Unilateral Iris Mass Lesions in Presumed Ocular Tuberculosis.

Iris mass lesions are uncommon and often raise suspicion of intraocular malignancy. However, infectious and inflammatory conditions, particularly ocular tuberculosis (TB), can closely mimic neoplastic lesions and pose significant diagnostic challenges, especially in the absence of systemic disease. A 36-year-old male presented with unilateral redness, pain, and reduced vision in the right eye for 15 days. Ocular examination revealed granulomatous anterior uveitis with two lobulated iris mass lesions and 360° posterior synechiae. Ultrasound biomicroscopy and anterior segment optical coherence t

Ocular tuberculosis in acquired immune deficiency syndrome (AIDS).

AIM: To determine the prevalence and clinical features of ocular tuberculosis in patients with the acquired immune deficiency syndrome (AIDS). DESIGN: Prospective cross-sectional study. METHODS: Detailed history and ocular examination of 46 patients (92 eyes) in the outpatient department of an AIDS clinic. RESULTS: Seventeen of 46 (36.9%) patients had clinical and radiological evidence of pulmonary tuberculosis. Of these, four (23.5%) showed findings consistent with ocular tuberculosis. Lesions included tubercles (1 eye of 3 patients) and chorioretinitis (1 eye of 1 patient). Commonly, thes

Mycobacterium Tuberculosis Modulates Fibroblast Growth Factor and Vascular Endothelial Growth Factor in Ocular Tuberculosis.

Purpose: To evaluate the role of angiogenic growth factors in the pathogenesis of intraocular tuberculosis. Methods: Retinal Pigment Epithelium (RPE) cells were infected with varying dilution of Mycobacterium tuberculosis (MTB), ranging from several thousand to a few MTB bacilli to replicate paucibacillary conditions. Angiogenesis growth factors were evaluated using multiplex fluorescent bead based flow cytometry in the culture supernatant of RPE cells infected with MTB, vitreous fluids and tear samples of uveitis patients visiting retina clinic. Results: Vascular endothelial growth factor (VEGF

Presumptive ocular tuberculosis: A case series of rare cases.

Ocular tuberculosis (TB) may present with various clinical manifestations and may involve any part of the eye. In 80% of cases, pulmonary involvement is not found. Here, we are presenting three cases of presumptive ocular TB with three rare manifestations. The first case is of bilateral retinal detachment involving one eye after another, the second case is of tubercular phlyctenular keratoconjunctivitis with keratolysis, and the third case is of bilateral neuroretinitis with extensive cerebral involvement. Despite the availability of various tests, clinical findings, tuberculin skin test, ches

Prognosis of Ocular Tuberculosis Following Long-Term Antitubercular Therapy.

Purpose: This study presents clinical features and prognosis after long-term (12-18 months) antitubercular therapy (ATT) in patients with ocular tuberculosis (OTB) in East China, an endemic area of tuberculosis. Methods: This retrospective study reviewed data from OTB patients treated at the Eye and ENT Hospital of Fudan University from 2008 to 2018. All the patients completed a minimum follow-up of 6 months after the cessation of ATT. Results: Sixty-six patients with OTB were studied. The ocular manifestations included retinal vasculitis (51.6%), choroiditis (24.2%), panuveitis (23.2%), inter

Pathogenesis of ocular tuberculosis: New observations and future directions.

Ocular tuberculosis (OTB) encompasses all forms of intra- and extra-ocular inflammation associated with Mycobacterium tuberculosis (Mtb) infection. However, the organism is rarely found in ocular fluid samples of diseased eyes, rendering the pathomechanisms of the disease unclear. This confounds clinical decision-making in diagnosis and treatment of OTB. Here, we critically review existing human and animal data related to ocular inflammation and TB pathogenesis to unravel likely pathomechanisms of OTB. Broadly there appear to be two fundamental mechanisms that may underlie the development of T

Immune stromal keratitis: a rare ocular presentation of tuberculosis.

An 11-year-old female patient presented with diminution of vision in both the eyes for the last 4 days. She had redness, watering and photophobia for the past 11 days. Slit lamp examination revealed multiple disc-shaped corneal stromal infiltrates with an overlying epithelial defect and hypopyon in both the eyes. A provisional diagnosis of infective keratitis was made. The patient was started on empirical antimicrobial therapy. However, no improvement was noted over the next 72 hours. Microbiological examination of the corneal scraping from both the eyes was negative. Considering the above, pr

Steroid treatment in ocular tuberculosis: A double-edged sword?

INTRODUCTION: We report the case of a Caribbean patient with an atypical presentation of bilateral tuberculous chorioretinopathy. PATIENT AND METHODS: A 57-year-old woman, with hypertension and non-insulin dependent diabetes, was referred to our center for a sudden loss of vision in the right eye to hand motions. Ophthalmic examination revealed only right papilledema. Brain magnetic resonance imaging was normal. Laboratory examination revealed no signs of inflammation. A right non-arteritic acute anterior ischemic optic neuropathy was first suspected. One week later, the visual acuity (VA) in

[Ocular tuberculosis in systemic lupus erythematosus and immunosuppressive therapy].

BACKGROUND: A marked reduction in eye disease attributed to tuberculosis has occurred over the past several decades. In recent years, however, tuberculosis has reemerged as a serious public health problem. We report a case of a severe ocular tuberculosis in a patient with systemic lupus erythematosus and immunosuppressive therapy. PATIENT: The 36-years-old woman underwent an immunosuppressive therapy because of a systemic lupus erythematosus detected two years earlier. After holidays on the Philippines Mycobacterium tuberculosis was found in a bronchial lavage. Two months later fundoscopy sho
