Sequential stress fractures of the ulna and radius in a patient with neurofibromatosis type 1: A case report.

Stress fractures of the forearm are uncommon, and to our knowledge, sequential involvement of both the ulna and radius has not been well characterized. Neurofibromatosis type 1 is associated with increased bone fragility and delayed fracture healing due to impaired osteoblastic differentiation, reduced mineralization capacity, and increased osteoclastic activity. We report the case of a woman in her early 60s with neurofibromatosis type 1 and lower-extremity weakness who relied on upper-extremity weight-bearing ambulation. She initially developed a nondisplaced transverse stress fracture of th

LCP distal ulna hook plate as alternative fixation for fifth metatarsal base fracture.

Intramedullary screw fixation is the most common treatment for fifth metatarsal base fractures. Screw application does not achieve accurate reduction in fracture with small fragments, osteoporotic bone, or Lawrence zone 1 fractures, however. On the basis of similar anatomical architectures between the distal ulna and the fifth metatarsal base, the purpose of this study was to assess the results of a locking compression plate (LCP) distal ulna hook plate in stabilizing displaced zone 1 or 2 fifth metatarsal base fractures. Nineteen patients with Lawrence zone 1 (n = 12) or 2 (n = 7) fractures o

[Complex fractures of the proximal end of the radius and ulna in adults: a retrospective study of 38 cases].

PURPOSE OF THE STUDY: Complex lesions of the proximal end of the radius and ulna are uncommon and generally associate a fracture of the proximal ulna and a dislocation of the radial head, which also may be fractured. We assessed the effect of the type of fracture and treatment on functional outcome and complications. MATERIAL AND METHODS: Thirty-eight adults (25 men and 13 women) were treated for complex fractures of the proximal end of the radius and ulna. For 25 of them, mean follow-up was 2.4 years. For the ulna, there was an epiphyseal fracture in 10 cases, a metaphyseal-epiphyseal fractu

Results from resection of the distal end of the ulna after fractures of the lower end of the radius.

Pain, impaired mobility and weakness in the wrist are common complications after fractures of the lower end of the radius. When these symptoms persist, resection of the distal end of the ulna has been the surgical treatment of choice. 24 patients who had undergone this procedure were reviewed. 50% stated they were not improved by the operation. Of 11 patients with degenerative changes in the distal radio-ulnar joint on preoperative X-ray, 8 stated they were helped by the operation, while of 13 patients without any signs of arthrosis in the distal radio-ulnar joint, only 4 experienced relief of

Outcomes of Treatment of Monteggia Fractures with Reduction and Internal Titanium Elastic Nail Fixation of the Ulna.

BACKGROUND: The purpose of this paper is to evaluate the usefulness of radiographic indices of humero-radial joint instability in order to predict treatment outcomes in Monteggia fractures. MATERIAL AND METHODS: A retrospective analysis was conducted in a group of patients who underwent closed reduction and internal titanium elastic nail (TEN) fixation of the ulna and closed reduction of a dislocation of the radial head between 2016 and 2018. The indications for intrame-dullary fixation comprised transverse or short oblique fractures. The direction of the dislocation was classified according

Plating of an isolated fracture of shaft of ulna under local anaesthesia and periosteal nerve block.

Isolated fractures of shaft of ulna are common. Plate fixation with anatomic reduction is thought to produce the best functional results in closed or open fractures. Surgery can be done under general and various types of regional anaesthesia. We report a case of fracture shaft of ulna treated by plating under a combination of WALANT (wide awake, local anaesthesia, no tourniquet) using tumescent anaesthesia and periosteal nerve block as a day care procedure.

Impact of olecranon fracture malunion: Study on the importance of PUDA (Proximal Ulna Dorsal Angulation).

PURPOSE: The Proximal Ulna Dorsal Angulation (PUDA) is part of the proximal ulna's normal anatomy. The importance of restoring ulnar anatomy characteristics precisely after olecranon fracture is not known, however, failure to recreate the PUDA after surgery may result in poorer functional outcomes. The purpose of this study is to evaluate the impact of minimal proximal ulna malunion on elbow ROM and function at least one year after olecranon ORIF. METHOD: A retrospective comparative cohort study took place in three level-1 trauma centers. Forty-nine adult volunteers who underwent ORIF for ole

Periosteal nerve blocks for distal radius and ulna fracture manipulation--the technique and early results.

BACKGROUND: We present a pilot series of patients with distal forearm fractures manipulated following a proximal periosteal nerve block with local anaesthesia. This is a novel technique which can be utilised in adults and children and is described herein. METHODS: With a median of 40 years (range 10-81 years), 42 patients (16 children) with distal radial and ulnar fractures were included. Of these patients, 40 underwent periosteal blocks in the emergency room or fracture clinic; 2 were already inpatients. Fractures were manipulated routinely and immobilised with plaster. Mobile fluoroscopy wa

Management of comminuted proximal ulna fracture-dislocations using a multiplanar locking intramedullary nail.

Intramedullary nails have been used for the fixation of olecranon fractures in an attempt to reduce the soft tissue irritation and resulting need for hardware removal seen with plating and tension banding. Further benefits include preservation of vascular supply, and increase stability and improved compression over some alternative techniques. Most intramedullary nails have been limited to simple olecranon fractures or osteotomies. One novel multiplanar, locking intramedullary nail, however, is indicated to stabilize all fracture patterns of the proximal ulna, including the coronoid. This part

Pseudo-Volkmann's contracture due to tethering of flexor digitorum profundus to fractures of the ulna in children.

We studied seven children with the unusual complication of pseudo-Volkmann's contracture due to tethering of the flexor digitorum profundus to fractures of the ulna. It was detected 2 days to 16 years after closed reductions of fractures of the shafts of the radius and ulna. The children did not have nerve palsies or undue pain after the reductions. Normal length, excursion, and function of the flexor digitorum profundus was restored by untethering the muscle and its tendons from the ulnar fracture by early manipulation or by late localized myotenolysis. We recommend that the passive range of

Fractures of the radial head and lesions of the lower radius and ulna in the adult: the importance of the prosthesis in resection.

Multi-fragmentary fracture of the head of radius is a common lesion of the elbow; and resection of the head is justified either in isolation or to be completed with prosthetic implant replacement. From a study of 25 hospital records of patients treated at the Orthopedic and Traumatology Center of CRAM of Strasbourg with an average post-operative follow-up of 8 years, it appears that the habitual complications secondary to simple resection of the radial head, in particular cubitus-valgus, ascension of radial shaft and dysfunction of the inferior radio-ulnar articulation, are not observed after

Association of radial head dislocation with fracture and acute plastic deformation of the ulna: unreported subtype of Monteggia fracture-dislocation in children.

Treatment of a Monteggia fracture-dislocation primarily includes anatomical reduction and fixation of the ulna. However, in rare cases, reduction of the radial head remains impossible even after performing this standard procedure. We have encountered patients with Monteggia fracture-dislocation whose radial head could be reduced by 'overcorrecting' the ulna. In this retrospective study, we aimed to propose an unreported subtype of Monteggia fracture-dislocation. Data of three patients (age 6, 14, and 16 years) with Monteggia fracture-dislocations whose radial head could not be reduced by anato

[Fracture of the distal ulna accompanying fracture of the distal radius. Minimally invasive treatment with elastic stable intramedullary nailing (ESIN)].

The distal radius is one of the commonest sites of fracture, and this injury is sometimes associated with fracture of the distal ulna. In recent years, surgical treatment of distal radius fractures has consisted increasingly in internal fixation with locking plates followed by early functional postoperative treatment. The associated injury to the distal ulna has so far not received much attention in the literature. Various techniques have veen described for its treatment: Kirscher wire fixation, tension band wiring, and internal fixation with screws and plates. Following positive results with

Double trouble! Concomitant distal ulna fractures predict worse 1-year outcome in distal radius fractures: a registry-based cohort study of 5,536 patients.

BACKGROUND AND PURPOSE: Data on distal radius fractures (DRFs) with concomitant metaphyseal distal ulna fractures is limited. We aimed to determine whether a combined DRF and distal ulna fracture (DRUF) predicts a worse patient-reported outcome, measured by the Disabilities of the Arm, Shoulder, and Hand (DASH) score, 1 year after injury. METHODS: This prospective registry-based cohort study included 5,536 adult patients with a DRF between 2003 and 2018. The 1-year DASH scores were recorded. All DRUFs were identified. Multivariable binary logistic regression assessed whether the presence of a…

Cuttable plate fixation for small breed dogs with radius and ulna fractures: Retrospective study of 31 dogs.

This retrospective study evaluated complication rates for radius and ulna fractures in small breed dogs in which 1.5 mm to 2.7 mm cuttable bone plates were used for internal fixation. The medical records of all cases from 2004 to 2011 that were presented to our clinic were reviewed. Inclusion criteria were: dogs with body weight < 9 kg, fracture of the radius and ulna with open reduction, and internal fixation utilizing a cuttable bone plate. Thirty-four fractures in 31 dogs met the inclusion criteria. Of 25 dogs that were available for follow-up, all achieved union, minor complications occurr…
