[Experience in the treatment of normogonadotropic infertility in men].

This study presents an experience of application of follicle-stimulating drug in the treatment of men with normogonadotropic infertility. Patients of the study group have received standard treatment, including multivitamins, and additionally received follitropin-alpha (Gonal-f). They also underwent ultraviolet blood irradiation. Patients of comparison group were treated according to a standard scheme. All men underwent standard clinical and laboratory examination. The therapy consisting of follitropin alpha and ultraviolet blood irradiation in patients with infertility normalizes the function

[Pruritus].

Pruritus is an annoying skin disturbance which is often amenable neither to diagnostic nor to therapeutic interventions. Occasionally, generalized pruritus can be ameliorated by treating the underlying disease (e.g. iron deficiency, hyperthyroidism). The good response to ultraviolet radiation opens new possibilities which should be tried in patients with chronic renal insufficiency or cholestasis. The existence of a UV-sensitive pruritogen could explain a multitude of findings and phenomena.

Bath-PUVA still represents a valuable treatment option for the subsets of psoriatic patients who are not eligible to or rejecting systemic treatments and are not responsive to NB-UVB phototherapy.

BACKGROUND: Photochemotherapy with bathwater delivery of psoralens plus UVA exposures (bath-PUVA) is mainly used for those psoriatic patients who are not responsive to narrowband (NB)-UVB phototherapy and oral-PUVA therapy and belong to two categories (1) patients with psoriasis without systemic comorbidities who do not need long-term continuous treatment and (2) patients who have contraindications to immunosuppressive drugs and oral-PUVA or refuse systemic drugs, including oral ingestion of psoralens, for personal reasons. However, it is not known how many patients belong to the second group

[Treatment of psoriasis by UVB and PUVA phototherapy].

PUVA has definitely proved effective in outbursts of extensive psoriasis vulgaris. Efficacy is enhanced by combination with retinoids. The pharmacokinetics of psoralen, which lead to adjustment of dosage to each case, is a concrete improvement which facilitates treatment. The side-effects of PUVA, and notably the risk of carcinogenesis, are well known and can be alleviated. UVB phototherapy, successfully used for a long time against psoriasis, has benefited from technical advances in the field of artificial sources of UV. In its conventional modality (wide UVB band), the adjunction of greasy e

Hypopigmented Mycosis Fungoides: Clinical, Histological, and Immunohistochemical Remission Induced by Narrow-band Ultraviolet B.

Mycosis fungoides (MF) is the most common type of primary cutaneous lymphomas. Several clinical variants of MF have been described. Purely, hypopigmented variant of MF (HMF) is rare. Phototherapy, especially photochemotherapy (Psoralen and ultraviolet), is the most widely used method and is recommended as the first-line treatment for HMF. However, there are no standard guidelines for phototherapy as the disease is uncommon. We, hereby, report a 30-year-old woman with HMF in whom clinical, histopathological, and immunohistochemical remission was achieved following narrow-band ultraviolet B ther

Narrowband UVB as an effective substitute for psoralen plus UVA: lessons from a psoralen shortage.

From March to August 2010, there was a shortage of encapsulated liquid 8-methoxypsoralen (8-MOP), the psoralen used for bath psoralen plus UVA (PUVA) in Toronto, Canada. Patients were forced to discontinue bath PUVA treatment and were transitioned to other therapeutic modalities, including narrowband UVB (nbUVB). A retrospective chart review was conducted of all patients who discontinued bath PUVA due to the unavailability of 8-MOP, with a focus on those who were switched to nbUVB. Sixty-three patients discontinued PUVA, 39 of whom were switched to nbUVB. Fifteen of 17 patients with mycosis fu

Ultraviolet radiation in skin ageing and carcinogenesis: the role of retinoids for treatment and prevention.

The mechanisms of UV-induced ageing and carcinogenesis of the skin have been elucidated in animals and humans, and both UVB and UVA radiation have been shown to have deleterious effects on the skin. Thus the use of solaria which deliver mostly UVA radiation is not safe. There is also an increased risk of ageing when using therapeutic UV sources. UV radiation is beneficial in many cases of skin disorders such as psoriasis, atopic eczema, acne and pruritus. Nevertheless by careful patient selection and follow-up the risks of UV can be minimised when treating patients with artificial UV radiation

A deep dive into UV-based phototherapy: Mechanisms of action and emerging molecular targets in inflammation and cancer.

UV-based phototherapy (including psoralen plus UVA (PUVA), UVB and UVA1) has a long, successful history in the management of numerous cutaneous disorders. Photoresponsive diseases are etiologically diverse, but most involve disturbances in local (and occasionally systemic) inflammatory cells and/or abnormalities in keratinocytes that trigger inflammation. UV-based phototherapy works by regulating the inflammatory component and inducing apoptosis of pathogenic cells. This results in a fascinating and complex network of simultaneous events-immediate transcriptional changes in keratinocytes, immu

Frequency of phototherapy for treating psoriasis: a systematic review.

INTRODUCTION: Narrow band ultraviolet B (NB-UVB) and psoralen-ultraviolet A (PUVA) remain inexpensive and effective anti-psoriatic therapies adopted worldwide with different frequency protocols. We aimed to systematically assess the evidence on the effects of different frequency protocols of phototherapy in treating psoriasis. EVIDENCE ACQUISITION: We used the following terms, namely "photochemotherapy," "phototherapy," "psoriasis," "UVB," "UVA" and "ultraviolet therapy," to search the Cochrane Controlled Register of Trials, MEDLINE and Embase databases on August 1, 2019. We organized results…

[New possibilities of phototherapy (author's transl)].

Absolute and relative indications for phototherapy have to be distinguished. Absolute indications, where no better treatment is available, are psoriasis, mycosis fungoides and - according to newer observations - atopic dermatitis. Relative indications, where phototherapy is an additional therapeutical possibility, are acne, certain types of dermatitis and of urticaria. Acute and chronical side effects are discussed and an outlook on the possibilities of phototherapy is given.

Oral methoxsalen photochemotherapy of extensive pityriasis alba. Preliminary report.

Six patients with extensive pityriasis alba were treated with oral methoxsalen, followed by exposure to midday summer sun or exposure to long-wave ultraviolet radiation in a psoralens plus ultraviolet A (PUVA) cabinet. Complete clearing or marked improvement was obtained in five patients in less than 4 weeks of treatment. The patient with the most extensive skin involvement and the longest duration of the disease achieved a marked degree of improvement after 15 weeks of therapy. A long-term follow-up is still required to define the maintenance treatment and its efficacy.

Facial skin rejuvenation.

PURPOSE OF REVIEW: In recent years, many new products and techniques have arisen that are useful in the rejuvenation of facial skin. Most of the therapies are directed at improving the results of photoaging. These cutaneous changes occur from chronic exposure to ultraviolet B light (290 to 320 nm) associated with sunburn, and ultraviolet AII light (320 to 340 nm), and ultraviolet AI light (340 to 400 nm) associated with photoaging. Clinically chronic photoaging may result in fine wrinkles, texture abnormalities, pigment dyschromias, and actinic keratoses. RECENT FINDINGS: Many methods of pati

Targeted UV therapy in the treatment of psoriasis.

Ultraviolet (UV) light is an effective treatment for extensive psoriasis and some other inflammatory skin conditions. Because the predominant effect of UV is a local one (as opposed to a systemic effect on immunity), localized delivery of ultraviolet B radiation (UVB) may be a useful treatment for localized variants of psoriasis and other conditions. The article reviews the literature regarding use of localized UV therapy. A theoretical benefit of localized UV therapy is reduced toxicity compared with whole-body therapy. Practical benefits in psoriasis treatment include higher efficacy and mor

Squamous cell carcinomas in relation to cyclosporin therapy of non malignant skin disorders.

Two patients with psoriasis and one with pityriasis rubra pilaris developed squamous cell carcinomas in relation to cyclosporin A therapy. Dermatological patients previously treated with ultraviolet radiation and other kinds of immunosuppressive therapy may represent a special risk group with respect to cyclosporin.
