Urinary retention after orthotopic neobladder substitution in females.

PURPOSE: Urinary retention is a common complication after orthotopic neobladder urinary diversion. We reviewed a case series of women who underwent neobladder creation and discuss anatomical findings, and urinary retention etiology and prevention. MATERIALS AND METHODS: We retrospectively reviewed the records of all orthotopic neobladder urinary diversions in female patients performed at our institution from 1999 through 2010. We abstracted baseline clinical and demographic characteristics, operative information, and postoperative clinical, urodynamic, imaging and secondary procedure followup…

Neuroblastoma causing acute urinary retention: a rare presentation.

BACKGROUND: Neuroblastoma is the most common extracranial solid tumor in children under the age of 5 years and, after acute lymphoblastic leukemia, is the second most prevalent malignancy in childhood. Acute urinary retention is rare in children, especially due to a neuroblastoma. OBJECTIVES: To present the potential complications of a pelvic neuroblastoma. CASE REPORT: A 9-month-old infant presented with urinary retention that had been attributed in the past to the infant's chronic constipation. She was catheterized and a large residual of urine was obtained; she was discharged home. She re…

Urinary elimination: Part 2--retention, incontinence and catheterization.

In part two of this article, the nursing role in relation to the assessment and management of urinary retention, urinary incontinence and catheterization are considered using recent literature and best practice statements. Retention of urine and incontinence have a major detrimental impact on a person's life and nurses have an important role to play in supporting patients. Catheterization is often necessary for acute retention of urine but is the last resort for incontinence. However, when catheter use is appropriate, asepsis technique during insertion and continuing management can help to min…

Urinary retention following endoscopic totally extraperitoneal inguinal hernioplasty.

BACKGROUND: The impact of preperitoneal mesh after endoscopic totally extraperitoneal inguinal hernioplasty (TEP) on voiding function has not been previously examined. The objectives of the present study were to evaluate the incidence of and risk factors for urinary retention following TEP. METHODS: Three hundred consecutive patients who underwent TEP between June 1999 and September 2001 were recruited. Patient records were reviewed retrospectively to identify those who developed postoperative urinary retention. For each case patient, five age-matched control patients were randomly selected. …

Acute urinary retention secondary to Herpes simplex meningitis.

We report a case of acute urinary retention in a 24-year-old man with Herpes simplex meningitis without genital lesions. Since the differential diagnosis in young patients who present with acute urinary retention also includes multiple sclerosis, lumbosacral disk herniation, rheumatological disorders and drug intoxication, a thorough history and careful neurological examination are of paramount importance in distinguishing these syndromes. As part of a directed neurological evaluation prompt performance of lumbar puncture is indicated; a lymphocytic pleocytosis is suggestive of herpetic mening…

Postoperative urinary retention: Part II--A retrospective study.

Postoperative urinary retention, a potential postoperative complication, can lead to discomfort and contribute to atony of bladder wall, need for catheterization and, sometimes, prolonged hospital stays. A retrospective study was conducted to analyze variables related to postoperative urinary retention. Data was collected by a chart review of 100 postoperative cases in a large Veterans Administration Medical Center. All patients were male, had a mean age of 61.1 years, and had received spinal or general anesthesia. Patients with known urologic problems, paraplegic patients, quadruplegic patien…

Urinary sodium and chloride during renal salt retention.

One hundred ten episodes of renal salt retention (urinary sodium and/or chloride less than 10 mEq/L) were studied retrospectively to determine the significance of discordance of urinary sodium from chloride. In 16 episodes the urinary sodium exceeded chloride by at least 15 mEq/L. This disparity was associated with the necessity for urinary excretion of substantial quantities of poorly reabsorbed anions (penicillin, ketones, or diatrizoate), a rapidly falling serum bicarbonate level (due to resolving metabolic or developing respiratory alkalosis), or substantial renal insufficiency (serum crea

[A case of leiomyoma of the urinary bladder in a woman with the chief complaint of total urinary retention].

Benign nonepithelial tumor of the urinary bladder is very rare. We experienced a case of a 33-year-old woman who complained of total urinary retention. Vaginal examination revealed a hen's egg sized retrovesical tumor. IVP revealed a filling defect on the cystogram. Cystoscopy revealed protrusion of left side of the bladder neck and intact mucosa. Transvaginal needle biopsy of the tumor was done, and pathohistological diagnosis of the tumor was done, and pathohistological diagnosis of the specimen was leiomyoma. The tumor was intramural type and was resected. It was 5.5 X 5 X 5 cm in size, wei…

[URINARY RETENTION FOLLOWING TRANSURETHRAL RESECTION OF THE BLADDER TUMOR].

We analyzed the incidence and risk factors associated with urinary retention following transurethral resection of the bladder tumors (TURBT). This retrospective study included a total of 218 TURBT cases we experienced from September 2021 to April 2023. Twenty-six patients (12%) experienced urinary retention after the procedure. The use of a thick urethral catheter (20 Fr or larger) post-surgery (p=0.013) was identified as a significant risk factor. Of the 26 patients with urinary retention, 18 were able to recover spontaneous urination, while the remaining 8 required either permanent urethral …

Tamsulosin for Prevention of Urinary Retention After Midurethral Sling.

IMPORTANCE: Midurethral sling (MUS) is a common procedure to correct stress urinary incontinence but carries a risk of postoperative urinary retention (POUR). OBJECTIVE: The objective of this study was to evaluate the efficacy of perioperative tamsulosin for reducing the incidence of POUR in patients undergoing MUS placement, with or without concomitant pelvic organ prolapse (POP) surgery. STUDY DESIGN: This was a retrospective cohort study of female patients who underwent MUS placement, with or without concomitant gynecologic surgery. Patients were excluded if they underwent transobturator …

Severe Persistent Urinary Retention Following Treatment With Intravenous Lecanemab.

Lecanemab is an amyloid-targeted antibody indicated for treating patients with amyloid-confirmed early Alzheimer's Disease in mild dementia or mild cognitive impairment stages. We report here a case of a subject with early stage of Alzheimer's Disease dementia, amyloid positive, who developed severe acute urinary retention following his first dose of intravenous lecanemab. His urinary retention resolved after a week but recurred following the second intravenous dose 2 weeks later. Lecanemab was discontinued, but the urinary retention has persisted for 8 months indicating possible permanent adv…

Exime prostatic stent in acute and chronic urinary retention.

PURPOSE: The Exime prostatic stent, a silicone-based device designed for temporary urinary retention management, offers an alternative to indwelling urethral catheters (IUCs), allowing spontaneous voiding. This study aimed to compare the efficacy and safety of the Exime device with IUCs in patients with urinary retention due to benign prostatic obstruction (BPO). METHODS: This retrospective study included 43 patients from 2020 to 2024 with failed trial without catheter (TWOC) following acute urinary retention (AUR) secondary to BPO, prostate volume ≤ 120 mL, and intravesical prostatic protrus…

Urinary Retention in Orthopaedic Surgery: An Evidence-based Algorithm.

Postoperative urinary retention (POUR), the inability to urinate spontaneously after surgery, affects up to 43% of orthopaedic patients and can lead to pain, bladder injury, urinary tract infections, increased healthcare costs, and delayed rehabilitation. POUR arises from decreased neurologic signaling, reduced bladder sensation and contractility, or mechanical obstruction. Risk factors include advanced age, prior urinary retention, comorbidities (eg, benign prostatic hyperplasia, diabetes, renal dysfunction), anesthesia type, perioperative opioid use, and catheterization. Despite its prevalen…

Adhesive arachnoiditis: a rare cause of postoperative urinary retention.

Postoperative urinary retention is a common consequence of pelvic surgeries. It is often related both to the nature of surgery and the medication used in the perioperative period. Adhesive arachnoiditis (AA) is a rare condition, which has various clinical presentations ranging from back pain to disabling neurological deficits. Numerous aetiologies for AA have been described in the literature, one such noted is spinal anaesthesia. We report a patient who presented with isolated bladder dysfunction in the form of urinary retention after spinal anaesthesia. She was evaluated and diagnosed with AA…

Wandering projectile, a rare cause of acute urinary retention.

BACKGROUND: Urethral obstruction due to retained projectile migrating into the genitourinary system has rarely been reported. The literature describes two main methods of retained projectile removal from the genitourinary system: (1) spontaneous expulsion during voiding and (2) manual extraction due to urethral obstruction causing acute urinary retention. CLINICAL PRESENTATION: We present a case in which a 23-year-old man presented with acute urinary retention four days after suffering a gunshot wound to the right distal posterolateral thigh. A retained projectile eroded through the posterior…
