Assessing outcomes of enhanced chronic disease care through patient education and a value-based formulary study (ACCESS)-study protocol for a 2×2 factorial randomized trial.

BACKGROUND: Chronic diseases result in significant morbidity and costs. Although medications and lifestyle changes are effective for improving outcomes in chronic diseases, many patients do not receive these treatments, in part because of financial barriers, patient and provider-level knowledge gaps, and low patient motivation. The Assessing outcomes of enhanced chronic disease care through patient education and a value-based formulary study (ACCESS) will determine the impact of two interventions: (1) a value-based formulary which eliminates copayment for high-value preventive medications; and

The Next Frontier of Kidney Care Quality: Addressing Known Gaps to Transform Care Delivery.

Over the past decade, quality measurement for patients with kidney disease has been centered on pay-for-performance metrics for kidney failure. From the seminal Centers for Medicare Services ESRD Quality Incentive Program in 2012 to Centers for Medicare Services dialysis start ratings in 2015 to more recent Centers for Medicare and Medicaid Innovation value-based care kidney models, like ESRD Treatment Choices and Kidney Care Choices, there has largely been a focus on care provided in dialysis units. The value of existing kidney-based quality measures has been evaluated, illustrating challenge

Evaluation of family health history collection methods impact on data and risk assessment outcomes.

Information technology applications for patient-collection of family health history (FHH) increase identification of elevated-risk individuals compared to usual care. It is unknown if the method of collection impacts data collected or if simply going directly to the patient is what makes the difference. The objective of this study was to examine differences in data detail and risk identification rates between FHH collection directly from individuals using paper-based forms and an interactive web-based platform. This is a non-randomized epidemiologic study in Singaporean population from 2016 to

Miscarriage and wellbeing: performative rituals for visualizing loss.

BACKGROUND: Whilst arts-based approaches are used across healthcare, an arts and health approach to miscarriage is unique in the context of mainstream emotional care provision. This pilot study was set up to explore whether performative rituals could be used as a strategy to support women and health professionals in the aftermath of miscarriage to fill an existing gap in provision. METHODS: A diverse sample of 12 women who had experienced one or recurrent miscarriages were selected from 100 women recruited via our project partners, Miscarriage Association and HOPE Support Services. A particip

Enhancing the SUSTAINable uptake of evidence to minimise harms from alcohol and other drugs in New South Wales, Australia.

Although alcohol and other drug use is increasingly the focus of policy and research efforts, there are challenges identifying and applying evidence-based strategies to minimise harms for alcohol and other drugs in health care and community settings. These challenges include limited available research, variability across settings, and lack of 'fit' between research evidence and their intended settings. In this commentary, we describe a novel approach to develop and evaluate tailored, sustainable strategies to enhance the uptake of evidence-based activities into health services and community se

Self care integrative treatment demonstrated in rural community setting improves health related quality of life of lymphatic filariasis patients in endemic villages.

This study assessed impact of community based self care integrative treatment provided through mass camps in villages of three districts of Kerala, India endemic for lymphatic filariasis (LF). Two most endemic Primary Health centres (PHCs) were selected from each of the three districts, where maximum concentration of LF patients is recorded. Fourteen one day LF camps, each attended by 30-40 patients were conducted. Trained Accredited Social Health Activists encouraged LF patients to attend camps. Skin wash and drying, care of bacterial entry points using dermatology drugs, and simple yoga and

Demand assessment and price-elasticity estimation of quality-improved primary health care in Palestine: a contribution from the contingent valuation method.

This paper proposes a new methodology to assess demand and price-elasticity for health care, based on patients' stated willingness to pay (WTP) values for certain aspects of health care quality improvements. A conceptual analysis of how respondents consider contingent valuation (CV) questions allowed us to specify a probability density function of stated WTP values, and consequently, to model a demand function for quality-improved health care, using a parametric survival approach. The model was empirically estimated using a CV study intended to assess patients' values for improving the quality

White Paper AGA: The Impact of Mental and Psychosocial Factors on the Care of Patients With Inflammatory Bowel Disease.

Patients with chronic medically complex disorders like inflammatory bowel diseases (BD) often have mental health and psychosocial comorbid conditions. There is growing recognition that factors other than disease pathophysiology impact patients' health and wellbeing. Provision of care that encompasses medical care plus psychosocial, environmental and behavioral interventions to improve health has been termed "whole person care" and may result in achieving highest health value. There now are multiple methods to survey patients and stratify their psychosocial, mental health and environmental risk

Why Value Framework Assessments Arrive at Different Conclusions: A Multiple Myeloma Case Study.

As the United States transitions from a volume-based health care system to one that rewards value, new frameworks are emerging to help patients, providers, and payers assess the value of medical services and biopharmaceutical products. These value assessment frameworks are intended to support various types of health care decision making. They have the potential to substantially affect patients, whether as tools for shared decision making with their doctors, as an input to care pathways used by providers, or through payer use of the frameworks to make coverage or reimbursement decisions. Promin

Health Care Rationing in a Just Society: The Clinical Effectiveness Model.

Representing 18% of gross domestic product, and projected to increase to 20% by 2022, health care costs in the United States are an unsustainable expense. The clinical effectiveness model of cost containment is an ethical and self-sustaining paradigm that can assist bending the health care-cost curve. As envisioned by Buyx et al, clinically effective care is aimed at making the practice of medicine more explicitly evidence based with the goals of improving clinical success, efficiency, and value. I provide a vision for applying the clinical effectiveness model to the American health care syste

The development of a value-based healthcare burns core set for children after burn injury.

BACKGROUND: This study, involving stakeholders from three Dutch burn centres was conducted to optimize person-centered burn care and improving patient-relevant outcomes, as part of a value-based healthcare (VBHC) approach. Transparent measurement and reporting of patients' relevant outcomes is essential to determine VBHC algorithms, and requires a core set of the most patient-relevant outcomes. The aim of this study was to develop a VBHC-burns core set for children. METHODS: We conducted a two-round modified Delphi study to reach consensus among caregivers of pediatric burn patients (<18 year

Development of International Quality Measures Targeting Low-Value Care in Hand Surgery.

PURPOSE: Low-value care, defined as care in which there is no evidence of benefit, increases the risk of harm, or adds unnecessary costs, persists in hand and upper extremity care globally. To date, there are no quality measures to measure low-value care for a global setting. We aimed to develop international quality measures that are important, feasible, usable, and scientifically acceptable for reducing low-value care in hand surgery. METHODS: We performed a literature review to identify areas of potential low-value care for hand surgery. A consortium of 11 United States-based surgeons with

Discharge disposition for home health care patients with Alzheimer's disease and related dementia: The role of living arrangements and rural living.

PURPOSE: To examine the relationship between living arrangements and discharge disposition, and how this relationship differs by the rural or urban characteristics of the patient's residence among home health care patients with Alzheimer's disease and related dementia (ADRD). METHODS: This retrospective study used the 2019 Outcome and Assessment Information Set and the Master Beneficiary Summary File. Our study was based on 531,269 Medicare fee-for-service patients with ADRD. We used linear probability regression models to examine the relationship between discharge disposition (to the communi

Report of the ACR's Economics Committee on Value-Based Payment Models.

A major outcome of the current health care reform process is the move away from unrestricted fee-for-service payment models toward those that are based on the delivery of better patient value and outcomes. The authors' purpose, therefore, is to critically evaluate and define those components of the overall imaging enterprise that deliver meaningful value to both patients and referrers and to determine how these components might be measured and quantified. These metrics might then be used to lobby providers and payers for sustainable payment solutions for radiologists and radiology services. Th

Central Venous Access: The Missed Patient Safety Goal.

Hospital-acquired conditions are conditions that never should happen to a patient while in the care of physicians, nurses, and the health care facility. Central line-associated bloodstream infections plague the nation's health care facilities. With increasing rates of infections being reported during hospitalization, hospital-acquired conditions, namely, infections, and more specifically central line-associated bloodstream infections, are now at the top of patient safety concerns and impact organization's reimbursement. Increased surveillance of infections by regulatory agencies and the implem

Re-anchoring the Value of Innovative Therapies in NICE Decision Making When Comparators are Cost Ineffective: A Case Study of Late-Onset Pompe Disease.

National Institute for Health and Care Excellence (NICE) technology appraisal processes assume that the standard of care (SoC) is itself cost effective. However, many treatments in use in the UK National Health Service (NHS), particularly in rare diseases, were historically commissioned without formal value assessment and are priced without reference to cost-effectiveness thresholds. Cost-ineffective comparators distort how value is ascribed to new technologies, undermining the coherence of NICE's decision-making framework, and imposing substantial opportunity costs on the NHS. Using late-onse
