Absent right superior vena cava.

A patient is presented in whom pulmonary artery catheter insertion fortutiously demonstrated persistent left superior vena cava on two separate occasions. Further studies with venogram and a first pass nuclear scan demonstrated total absence of the right superior vena cava. Although this anomaly is commonly associated with intracardiac defects and rhythm disturbances only the latter was seen in our patient. Invasive hemodynamic monitoring is frequently performed in the majority of modern intensive care units. The bedside technique used for insertion of a Swan-Ganz catheter using only pressure

Membranous obstruction of the inferior vena cava.

Membranous obstruction of the inferior vena cava (MOVC) is a rare clinical entity. The incidence is higher in Japan and Africa than that in USA and Europe. MOVC is considered to be different from the Budd-Chiari syndrome. There are differences in clinical manifestation and pathologic changes in the liver. Findings of the liver revealed cirrhosis in all patients but in one. Ascites and esophageal varices could be observed in 79% and in 58% of the patients, respectively. According to Sugiura's classification, they consisted of Type Ia in 63%, Type Ib in 11%, Type II in 11%, and Type III in 16%.

Transvenous interruption of the inferior vena cava.

Transvenous interruption of the inferior vena cava was successful in 45 of 47 consecutive patients treated during a 41/2-year period. Greenfield filters were used in 40, and Mobin-Uddin umbrellas in 5 others. Technical details of the insertion procedure are of particular importance for successful implantation of the Greenfield device. Follow-up data were available for 34 of 36 surviving patients and autopsy findings for 4 of the 11 who had died. Patency of the vena cava was assessed by radionuclide scan, venography, or autopsy. Of 33 patients with Greenfield filters who were evaluated, only on

Traumatic thrombosis of the inferior vena cava.

A case of a 21-year-old male with blunt abdominal trauma and subsequent acute thrombosis of the inferior vena cava, presenting as abdominal pain, is reported. The apparent mechanism of proximal inferior vena cava thrombosis with distal extension into the iliac veins differs from that suggested in earlier reports. Resolution of symptoms was achieved by heparinization and bedrest alone, in spite of thrombus in the suprarenal vena cava.

Congenital interruption of the inferior vena cava.

In the usual form of interruption of the inferior vena cava (IVC), the post-renal IVC continues as the azygos and hemiazygos vein. We report a patient with complete interruption of the IVC in whom no direct continuity existed between the IVC and the azygos system. Connection between these two systems was via the vertebral plexus and ascending lumbar veins. Associated venous malformations included bilateral azygos veins and anomalous connection of pulmonary and hepatic veins.

The silhouette sign and the inferior vena cava.

Pulmonary lesions causing obliteration of the normal inferior vena cava (IVC) shadow on the lateral radiograph are described. Retrocardiac mediastinal mass lesions and subpulmonic pleural effusions may also obliterate the IVC contour. Loss of the IVC shadow is an additional radiographic sign of right lower lobe collapse and frequently of disease involving the medial basal segment of the right lower lobe. This finding should be an indication that further investigation is needed to explain the loss of normal aeration.

Injuries to the inferior vena cava and their management.

Injuries to the inferior vena cava are being seen with increasing frequency in the civilian population. A review of the experience at UCLA/Harbor General Hospital Medical Center over a ten year period (1966 to 1976) discloses thirty-four patients with major injuries to the inferior vena cava, with an overall mortality of 53%. The factors that appear critical to patient survival are: (1) level of injury (suprarenal versus infrarenal sites); (2) presence or absence of profound shock on admission; and (3) the speed with which diagnosis is made and treatment carried out. Technical considerations r

A Rare Case of Shone Complex With a Persistent Left Superior Vena Cava and Absent Right Superior Vena Cava.

Shone complex is a rare and underrecognized congenital heart disease involving four obstructive lesions of the left heart. This report presents a rare case of incomplete Shone complex, along with other associated anomalies, requires an extensive clinical workup for accurate diagnosis and appropriate surgical planning.
