Spontaneous positive end-expiratory pressure (sPEEP) is a simple, low-cost method of providing low to moderate levels of PEEP as an alternative to continuous positive airway pressure (CPAP), whether used alone or with continuous positive pressure ventilation (CPPV) in intermittent mandatory ventilation (IMV). The therapeutic effects of sPEEP therapy are demonstrated in 7 adult respiratory distress syndrome patients. All patients, initially hypoxemic on a high F1O2, had an acceptable PaO2 (with F1O2 less than or equal to 0.4) within 12 hours of initiation of sPEEP therapy. sPEEP, unlike CPAP an

BACKGROUND AND METHODS: Home-mechanical-ventilation in Duchenne muscular dystrophy improves survival and quality of life. In this observational study in Duchenne muscular dystrophy patients, lung function and respiratory muscle strength at the time of ventilation-initiation and their change under home-mechanical-ventilation were investigated. Main outcomes of interest were age and pulmonary function at the time of ventilation-initiation.
RESULTS: Age at start of nocturnal home-mechanical-ventilation was 19.8 ± 4.8 years (n = 80) and 23.8 ± 5.4 years when home-mechanical-ventilation was extend

During assisted ventilation in patients with hypoxemic respiratory failure and acute respiratory distress syndrome (ARDS), achieving a balance between ventilator support and patient effort is essential. Contemporary approaches favor light sedation and limited use of neuromuscular blocking agents: most recent evidence would suggest that spontaneous breathing should be encouraged for as long as possible, provided that excessive inspiratory effort does not injure the lungs or the diaphragm. While spontaneous breathing is beneficial in case of mild-moderate hypoxemia, it may become injurious in mo

Patients suffering from Ménière's disease are particularly sensitive to negative pressure in the middle ear. For example, attacks of vertigo can be triggered by a descent in an aircraft when ventilation of the middle ear can become critical. Positive-pressure pulse treatment has been shown to have a beneficial effect on the symptomatology and is now a true alternative in the treatment of Ménière's disease. In this study, we compared two devices that produced positive-pressure pulses delivered to the inner ear via the external ear canal and after the insertion of a middle-ear ventilation tube.

Policies to prevent respiratory virus transmission in health care settings have traditionally divided organisms into Droplet versus Airborne categories. Droplet organisms (for example, influenza) are said to be transmitted via large respiratory secretions that rapidly fall to the ground within 1 to 2 meters and are adequately blocked by surgical masks. Airborne pathogens (for example, measles), by contrast, are transmitted by aerosols that are small enough and light enough to carry beyond 2 meters and to penetrate the gaps between masks and faces; health care workers are advised to wear N95 re

Negative-pressure pulmonary oedema caused by upper airway obstruction after tracheal extubation is well recognised, but extensive pulmonary haemorrhage is rare. We report a case of post-extubation, laryngospasm-induced pulmonary oedema with associated pulmonary haemorrhage. The patient required mechanical ventilation with high positive end-expiratory pressure.

Chronic Obstructive Pulmonary Disease (COPD) is a prevalent condition in surgical patients and is recognized as a significant risk factor for negative postoperative outcomes. Such adverse outcomes include increased admissions to intensive care units, extended hospitalizations, prolonged ventilation, and elevated rates of perioperative morbidity and mortality. Intraoperative mechanical ventilation is a crucial strategy for managing COPD and improving postoperative outcomes in this high-risk population. However, typical ventilation measures employed during general anesthesia may exacerbate COPD-

We report the case of a 6-month-old child who developed acute pulmonary edema because of laryngeal spasm during orthopedic manipulations for congenital hip dysplasia. Laryngospasm was probably secondary to an unsuspected light level of anesthesia, maintained via face mask. No other predisposing factors, such as enlarged adenoid tonsils, laryngitis, epiglottitis, mechanical stimulation of the larynx or aspiration of foreign material were identified. Serious oxygen desaturation and bradycardia ensued, during inefficient attempts at positive pressure ventilation. After emergency intubation withou

The present theory of eustachian tube (ET) function and middle ear (ME) ventilation posits that oxygen absorbed by the ME mucosa causes negative ME pressure which is relieved by periodic opening of the ET during swallowing and yawning. After developing a method to cannulate the ET of mongrel dogs we connected the cannulas hermetically to manometers. This system excluded ET function and tested the oxygen absorption capacity of the ME. When we controlled respiration and maintained blood gas PO2 and PCO2 at normal levels, we were unable to find any manometric evidence of negative pressure of gas

The acute respiratory distress syndrome (ARDS) is a severe form of acute hypoxemic respiratory failure caused by an insult to the alveolar-capillary membrane, resulting in a marked reduction of aerated alveoli, increased vascular permeability and subsequent interstitial and alveolar pulmonary edema, reduced lung compliance, increase of physiological dead space, and hypoxemia. Most ARDS patients improve their systemic oxygenation, as assessed by the ratio between arterial partial pressure of oxygen and inspired oxygen fraction, with conventional intensive care and the application of moderate-to

In the beginning of 2020, the coronavirus (COVID-19) pandemic started to spread globally, causing panic to the lives of people around the world; many countries executed lockdown of cities or even total lockdown of the entire countries. The coronavirus disease (COVID-19) is transmitted via air droplets. In medical environments that use traditional hermetic ventilation systems, medical personnel who come in contact with patients are more susceptible to infection compared to regular staff; therefore, the air flow and air quality of hermetic negative pressure isolation wards are highly critical. BACKGROUND: Diaphragm pacing in patients with intact phrenic nerve and diaphragm can be used as an alternative to mechanical ventilation. Indications cover diseases caused by central hypoventilation like C2-quadriplegia and Ondine's syndrome. Advantages are physiological ventilation with negative pressure and an improvement in articulation.
CASE REPORT: We report our experiences with a patient suffering from chronic hypoventilation caused by a ventilatory pump failure following tetraparesis due to congenital toxoplasmosis.

BACKGROUND: Long-term controlled mechanical ventilation in the intensive care unit induces ventilator-induced diaphragm dysfunction (VIDD). The transition from controlled mechanical ventilation to assisted mechanical ventilation is a challenge that requires clinicians to balance overassistance and underassistance. While the effects of overassistance on the diaphragm are well known, the authors aimed to assess the impact of underassistance on diaphragm function and structure in a piglet model with preexisting VIDD (after long-term controlled mechanical ventilation) or without VIDD (short-term c

BACKGROUND: In patients with respiratory failure, assisted mechanical ventilation is often used after the initial acute phase to support patient's respiratory activity, enhance oxygen exchange, and improve lung ventilation and perfusion. In this phase, monitoring inspiratory effort and respiratory drive might reduce the risk of patient self-inflicted lung injury (P-SILI), thus potentially improving the outcome. P-SILI depends on the negative pressure generated by inspiratory muscles and the positive pressure delivered by the ventilator machine. Measuring the amount of inspiratory pressure gene

Negative pressure pulmonary oedema is one form of non cardiogenic pulmonary oedema that can occur following any general anaesthesia in which the patient was intubated. This complication can be severe, resulting in death/life-threatening if not diagnosed and treated promptly. We report a case of negative pressure pulmonary oedema, in a 26-year-old man, secondary to inspiratory effort with laryngeal spasm. Patient was transferred to intensive care unit for monitoring and non invasive ventilation. Prevention, early diagnosis, and prompt treatment allowed a rapid and uncomplicated resolution.

BACKGROUND: Negative pressure pulmonary edema (NPPE) is a non-cardiogenic hemodynamic pulmonary edema resulting from significant inspiratory effort against obstructed airways. Although rare, it is a recognized cause of postoperative respiratory distress and can mimic differential diagnoses. Patients most predisposed to this condition are muscular or athletic individuals, due to their ability to generate significant negative intrathoracic pressure.
CASE PRESENTATION: An 18-year-old male patient underwent a laparoscopic appendectomy under general anesthesia with tracheal intubation. Two minutes

Negative-pressure pulmonary edema (NPPE) is a clinical entity of anaesthesiologic relevance, perioperatively caused by obstruction of the conductive airways (upper airway obstruction, UAO) due to laryngospasm in approx. 50% of the cases, its early recognition and treatment by the anaesthesist is mandatory. NPPE, also addressed as post-obstructive pulmonary edema (POPE) presents in most cases as a complex of symptoms with rapid onset, consisting of acute respiratory failure with dyspnea, tachypnea, and strained respiratory efforts. Additional signs are paradoxe ventilation, pink frothy sputum,

The function of the eustachian tube is ventilation of the middle ear to equilibrate air pressure in the middle ear with atmospheric pressure. Obstruction of the middle ear produces a negative pressure in the middle ear due to absorption of the oxygen from the middle ear cavity, often leading to otalgia. This study was done on 30 patients complaining of otalgia. They were 16 males and 14 females. They were found to have eustachian tube obstruction by examination, and negative middle ear pressure by tympanometry. All patients were treated with a mixture of equal amounts of nitrous oxide and oxyg

BACKGROUND: The relationship between indices of mechanical ventilation and pulmonary artery pressures remains ill-defined in ARDS. As our understanding of mechanical ventilation has progressed, there is now a greater appreciation of the impact of high driving pressures and mechanical power in perpetuating lung injury. However, the relationship between the newer derived indices of mechanical ventilation and pulmonary artery pressure is unclear. We performed a post hoc analysis of the Fluid and Catheters Treatment Trial (FACTT) trial to investigate the associations between mechanical ventilation

Non invasive ventilation (NIV), primarily applied in cardiogenic pulmonary edema, decompensated COPD and hypoxemic respiratory failure, has also found a wide application in the postoperative period. The expanding indications to the transcatheter treatment of diseased left heart valves have led to an increase in cardiac interventional and diagnostic procedures in severely fragile cardiac patients. As an essential part of post cardiac surgery care is ventilatory support, NIV use has expanded to cardiosurgical patients. The objective of this study was to investigate the application and the result
