Fourteen neonates 18 hours to 32 days old with transposition of the great arteries (TGA) and virtually intact ventricular septum (IVS) underwent arterial switch operations under deep hypothermic circulatory arrest. Preoperative left ventricular to right ventricular peak systolic pressure ratios ranged from 0.7 to 1.0 (mean, 0.92), and the echocardiogram showed a centrally positioned ventricular septum in 10 patients and a rightward displaced ventricular septum in 4. At the time of initial balloon atrial septostomy a patent ductus arteriosus was found with angiography in 39 of 81 infants with transposition of the great arteries with intact ventricular septum. Twnety-nine cases of aneurysm of the membranous septum in association with various cardiac abnormalities are described. These aneurysms are more common than has been previously thought (10% of the total associated anomalies managed surgically). The patients' ages ranged from 3 months to 28 years. Anatomical correction by arterial "switch" procedure was performed in a patient with transposition of the great arteries, intact ventricular septum, and left ventricular hypertension due to distal pulmonary stenosis. The patient died from left ventricular failure. The reasons for the failure of this procedure are discussed.

The mortality and morbidity of children with pulmonary atresia/intact ventricular septum (PA/IVS) are linked to the degree of right ventricular (RV) hypoplasia. Opening up the pulmonary valve (PV) in fetal life could result in improved growth of the RV making it amenable to biventricular repair postnatally. Successful valvulotomy of the PV was performed in a fetus with heart failure at 28 weeks. Following the procedure there was significant growth of the tricuspid valve and RV. Ventricular tachycardia ablation in the post-surgical patients is complicated by difficult epicardial access. Endocardial-only ablation may lead to failure which can be prevented by mapping and ablating inside the coronary venous system. Radiofrequency ablation inside the coronary venous system is dependent on anatomical and biophysical factors. Herein we report a ventricular tachycardia case necessitating bipolar ablation between the middle cardiac vein and the left ventricular endocardium.

To compare usefulness of a ventricular extrastimulus (VES) from the right ventricular (RV) apex versus the RV septum in patients with a septal accessory pathway (AP), VES was applied from the RV apex and the summit of the RV septum in patients with septal APs. Fifteen patients with pulmonary atresia or critical pulmonary stenosis and intact ventricular septum were studied. All were operated on in the neonatal period, with valvotomy or a systemic to pulmonary arterial shunt, or both.
