Transfusion and bleeding risk associated with terbutaline administration for external cephalic version among patients delivering the same day: a retrospective cohort study using the Premier Healthcare Database (2016-2020).

INTRODUCTION: The administration of pre-procedural terbutaline is associated with increased success for external cephalic version, and its use for this purpose is supported by the American College of Obstetricians and Gynecologists. However, terbutaline's uterine relaxant effect may potentially increase the risk of bleeding in the postpartum period, though this association has not been studied. METHODS: Using the Premier Healthcare Database, need for blood transfusion and postpartum hemorrhage incidence were examined in singleton pregnancies with an external cephalic version on the date of de

Ultrasonographic evaluation of myometrial thickness and prediction of a successful external cephalic version.

OBJECTIVE: To test the hypothesis that myometrial thickness predicts the success of external cephalic version. METHODS: Abdominal ultrasonographic scans were performed in 114 consecutive pregnant women with breech singletons before an external cephalic version maneuver. Myometrial thickness was measured by a standardized protocol at three sites: the lower segment, midanterior wall, and the fundal uterine wall. Independent variables analyzed in conjunction with myometrial thickness were: maternal age, parity, body mass index, abdominal wall thickness, estimated fetal weight, amniotic fluid ind

Evaluation of an improved tool for non-invasive prediction of neonatal respiratory morbidity based on fully automated fetal lung ultrasound analysis.

The objective of this study was to evaluate the performance of a new version of quantusFLM®, a software tool for prediction of neonatal respiratory morbidity (NRM) by ultrasound, which incorporates a fully automated fetal lung delineation based on Deep Learning techniques. A set of 790 fetal lung ultrasound images obtained at 24 + 0-38 + 6 weeks' gestation was evaluated. Perinatal outcomes and the occurrence of NRM were recorded. quantusFLM® version 3.0 was applied to all images to automatically delineate the fetal lung and predict NRM risk. The test was compared with the same technology but u

Evisceration as fetal destructive operation: an art revisited.

Fetal destructive operation is a vanishing art today. In an era of increasing cesarean deliveries it has become a historic event. Incidence of destructive operation has varied from various Indian hospitals 0.09-0.28%. Evisceration is one of the rarest of all destructive operations, performed in cases of cephalopelvic disproportion with large fetal abdominal or thoracic tumors and fetal malformations, which are incompatible with life. Less than 50 cases of fetal evisceration have been reported in the literature so far. We are presenting a case of gross fetal abdominal malformation in a multigra

Non-reassuring fetal heart rate and associated factors among laboring mothers at southern public hospitals in Ethiopia: a poisson regression model.

Non-reassuring fetal heart rate is a critical indicator of fetal well-being. It is used to predict fetal hypoxia and metabolic acidosis due to insufficient fetal oxygenation and is the major contributor to perinatal mortality and morbidity. This study aimed to assess the magnitude of non-reassuring fetal heart rate status and its associated factors among laboring mothers at South Omo public Hospitals, in Southern Ethiopia. An institution-based cross-sectional study was conducted among randomly selected 448 laboring mothers from June 11 to August 30, 2023. Data were collected using a pre-tested

Randomized trial of anaesthetic interventions in external cephalic version for breech presentation.

BACKGROUND: Successful external cephalic version (ECV) for breech presenting fetus reduces the need for Caesarean section (CS). We aimed to compare the success rate of ECV with either spinal anaesthesia (SA) or i.v. analgesia using remifentanil. METHODS: In a double-phased, stratified randomized blinded controlled study we compared the success rates of ECV, performed under spinal anaesthesia (SA), i.v. analgesia (IVA) using remifentanil or no anaesthetic interventions. In phase I, 189 patients were stratified by parity before randomization to ECV, performed by blinded operators, under SA usin

Clinical use of nitric oxide donors and L-arginine in obstetrics.

Nitric oxide (NO) is a free radical that plays a fundamental role in human physiology, being involved in the homeostasis of different functions. In obstetrics this molecule is determinant in the physiology of labor and cervical ripening; it possibly plays a fundamental role in the etiology of preeclampsia and intrauterine growth restriction, and it could also be utilized in view of its ability to induce smooth muscle relaxation. Several clinical trials have ascertained the ability of the topical application of NO donors to promote cervical ripening, and also labor induction. There is much less

[External cephalic version close to term in the management of breech presentation (author's transl)].

In 16 cases from a total of 36 (44%) the external cephalic version was successful. 18 times the relaxation of the uterus was performed by general anaesthesia with a successrate of 50% and 17 times by Fenoterol with a successrate of 35%. A V-para had a successful external cephalic version without any relaxation of the uterus. In average the external cephalic version was performed in the 39th week of pregnancy. From these 16, in whose case an external cephalic version was successful, there were 6 caesarian deliveries because of fetal indication. From 5 of these 6 caesarian deliveries it was disc

Maternal and perinatal outcomes in external cephalic version: prognostic factors for the success of the technique.

Background We aimed to analyze the success rate of external cephalic version (ECV) for breech presentations performed in our center between March 2011 and March 2016. We evaluated factors related to a successful ECV, delivery mode, complications and newborn status after ECV. Methods Analysis of assembled data of 327 consecutive ECVs in the third trimester was done. Results The total success rate was 56.6%. After a successful ECV, 85.9% of the fetuses were delivered vaginally. Logistic regression analysis of background factors leading to a successful ECV showed that tocolysis with ritodrine and
