Regression-Based Analysis of Vestibular Laboratory Tests for the Prediction of Unilateral Vestibular Schwannoma.

OBJECTIVE: The diagnostic work-up for vestibular pathologies involves a battery of tests designed to quantify the functioning of the otolith organs and semicircular canals. Clinical data from video head-impulse tests, vestibular-evoked myogenic potentials, subjective visual verticality, and caloric tests are usually collected. Our study applied regression analyses to predict the affected side of a patient group with vestibular schwannoma, learning from laboratory vestibular tests, to assess their relative predictive capacity in predicting the tumor side. Technology or Method: The dataset was p

Vestibular Functional Patterns in Sudden Sensorineural Hearing Loss: A Systematic Review and Pooled Analysis.

OBJECTIVES: To identify the most prevalent vestibular dysfunction patterns in idiopathic sudden sensorineural hearing loss (SSNHL) and their potential clinical significance. DESIGN: A systematic search was conducted in PubMed, SCOPUS, and Google Scholar (October 2024), following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Studies assessing vestibular function in SSNHL using the video head impulse test, cervical vestibular evoked myogenic potentials, and ocular vestibular evoked myogenic potentials were included. Exclusion criteria encompassed studies without

Vestibular Neuritis: Recent Advances in Etiology, Diagnostic Evaluation, and Treatment.

Purpose of Chapter: This chapter highlights the recent advances in etiology, diagnostic evaluation, and management of vestibular neuritis (VN). Recent Findings: The viral hypothesis has been strengthened with new evidence as the main etiology of VN. Recent evidence indicates that bedside oculomotor findings play a critical role in differentiating VN from stroke. The implementation of cervical and ocular vestibular evoked myogenic potential, and video head impulse test in vestibular function testing has made it possible to diagnose selective damage of the vestibular nerves. The management of th

[Comparative analysis of audiovestibular testing results in vestibular schwannomas].

Objective:This study aimed to analyze the results of auditory and vestibular function tests in patients with unilateral vestibular schwannoma and explore their association with tumor size. Methods:Clinical data from 81 patients diagnosed with unilateral vestibular schwannoma who underwent pure-tone audiometry(PTA), cervical and ocular vestibular evoked myogenic potentials(c/oVEMP), as well as video head impulse test(vHIT), and subsequently underwent surgical treatment, were retrospectively analyzed. Patients were categorized into groups based on tumor size: small (≤ 15 mm), medium(16-30 mm), a…

Post headshake nystagmus: further correlation with other vestibular test results.

BACKGROUND: It is generally accepted that headshake nystagmus (HSN) is generated from an asymmetrical peripheral vestibular input and a correlation exists between HSN and canal paresis. There have been limited reports, however, how HSN correlates with the more recently introduced tests of vestibular function. AIMS/OBJECTIVES: To evaluate the correlation between HSN and unilateral weakness on caloric testing, high-frequency vestibulo-ocular reflex (VOR) function on video head impulse testing (VHIT) and otolith function determined by vestibular evoked myogenic potentials (VEMPs). METHODS: A re

Vestibulary rehabilitation--election treatment method for compensating vestibular impairment.

OBJECTIVE: This paper aims to reveal the actual benefit of vestibular rehabilitation (VR) in patients with unilateral vestibular loss. PATIENT AND METHODS: Case report of a young female patient with acute unilateral vestibular loss due to facial nerve schwannoma developed above the internal auditory canal (IAC) from where it seems to have entered the IAC. Betahistine associated to VR treatment was recommended due to persisting imbalance after tumor removal. The benefit of the combined therapy was evaluated objectively (sensory organization test) and subjectively (questionnaires regarding self

Potential application of ocular and cervical vestibular-evoked myogenic potentials in Meniere's disease: a review.

By stimulating the ear with air-conducted sound or bone-conducted vibration stimuli, vestibular-evoked myogenic potential (VEMP) can be recorded on the contracted neck muscles, termed cervical VEMP (cVEMP), and on the extraocular muscles, termed ocular VEMP (oVEMP). These two electrophysiological tests expand the test battery for clinicians to explore the dynamic otolithic function, adding a potential usefulness to the sacculocollic reflex and vestibulo-ocular reflex, respectively. The inner ear test battery, including audiometry, and cVEMP, oVEMP and caloric tests, is designed for complete ev

[Some characteristics of vertigo in vestibular neuronitis].

The authors present a detailed clinical analysis of objective neurological symptoms and vertigo in patients with vestibular neuronitis. Diagnostic criteria are specified allowing differentiation between vertigo and dizziness, pathognomonic signs of vestibular neuronitis are outlined. Peripheral location of the pathological process in vestibular neuronitis is suggested. How rotating vertigo is forming in patients with vestibular neuronitis is hypothesized.

[Vestibular disorders in the minimal cerebral dysfunсtion in adults].

Vestibular dysfunction is the most frequent symptom of minimal cerebral dysfunction in adults. Objective of the research was to study the etiopathogenesis of minimal cerebral dysfunction based on the analysis of the latency of different types of induced nystagmus. Threshold stimulation by physiological stimuli was used to study vestibular function (functional load tests) for the evaluation of neurodynamical plasticity of all parts of vestibular analyzer. Sixty males with manifestations of posterior cervical sympathetic syndrome, consequences of traumatic brain injury and neuroinfection, histor

Recent advances in idiopathic bilateral vestibulopathy: a literature review.

BACKGROUND: Idiopathic bilateral vestibulopathy (IBV) is an acquired bilateral peripheral vestibular dysfunction of unknown etiology, with p … perior vestibular nerve (SVN) system. However, recently, with the development of more sophisticated vestibular function tests of the otolith organs and vertical semicircular canals, it has been revealed that IBV involves peripheral vestibular lesions other than those already identified in the LSCC and the SVN system. Furthermore, novel subtypes of IBV that do not involve b …

Morphological correlation between caloric tests and vestibular hydrops in Ménière's disease using intravenous Gd enhanced inner ear MRI.

The purpose of this study was to prove the hypothesis that caloric response in Ménière's disease (MD) is reduced by hydropic expansion of the vestibular labyrinth, not by vestibular hypofunction, by evaluating the correlation morphologically using an intravenous Gadolinium (IV-Gd) inner ear MRI. In study I, the prevalence of abnormal video Head Impulse Test (vHIT) results among the patients with definite unilateral MD (n = 24) and vestibular neuritis (VN) (n = 22) were investigated. All patients showed abnormal canal paresis (CP) (> 26%) on caloric tests. The prevalence of abnormal vHIT in pat

Significance of Vestibular Testing on Distinguishing the Nerve of Origin for Vestibular Schwannoma and Predicting the Preservation of Hearing.

BACKGROUND: Determining the nerve of origin for vestibular schwannoma (VS), as a method for predicting hearing prognosis, has not been systematically considered. The vestibular test can be used to investigate the function of the superior vestibular nerve (SVN) and the inferior vestibular nerve (IVN). This study aimed to preoperatively distinguish the nerve of origin for VS patients using the vestibular test, and determine if this correlated with hearing preservation. METHODS: A total of 106 patients with unilateral VS were enrolled in this study prospectively. Each patient received a caloric
