A Cross-sectional Survey of the Clinical Manifestations and Underlying Illness of Cough.

AIM: The aim of this study was to identify factors affecting the final diagnosis of cough. MATERIALS AND METHODS: This study recruited 463 consecutive patients who visited five Japanese general hospitals due to cough from October 2006 to September 2007. Of these, 418 patients (90%) who completed a questionnaire designed to acquire data regarding clinical manifestations of cough were included. RESULTS: Most patients with bronchial asthma had cough with seasonal variation and wheezing. Patients with gastro esophageal reflux disease suffered from heartburn and cough without daily or seasonal va

Comparison of the clinical and laboratory characteristics of pertussis or viral lower respiratory tract infections.

INTRODUCTION: Whooping cough-like respiratory tract infections (WCLRTI) caused by factors other than the Bordetella pertussis are available. Clinical picture is difficult to differentiate between the B. pertussis and viral respiratory infections. METHODOLOGY: Eighty-five patients with the diagnosis of WCLRTI were divided into 3 groups. Group 1 involved patients with pertussis shown by nasopharyngeal aspirate culture (NAC) and/or PCR. Group 2 consisted of patients who B. pertussis was not detected by NAC however, clinicians still evaluated them as potential patients of pertussis. Group 3 invol

Clinical characteristics and risk factors for severe pertussis in infants aged less than 3 months.

BACKGROUND: Pertussis is a globally prevalent infectious disease caused by Bordetella pertussis. Although extensive research has examined pertussis in children, studies specifically focusing on infants aged less than 3 months remain limited. The aim of this study was to analyze the clinical characteristics of pertussis and the risk factors associated with severe pertussis in infants aged less than 3 months. METHODS: Clinical data from infants aged less than 3 months who were hospitalized with pertussis were retrospectively analyzed. Multivariate logistic regression analysis was conducted to d

How well do ICD-9 physician claim diagnostic codes identify confirmed pertussis cases in Alberta, Canada? A Canadian Immunization Research Network (CIRN) Study.

BACKGROUND: Rates of Bordetella pertussis have been increasing in Alberta, Canada despite vaccination programs. Waning immunity from existing acellular component vaccines may be contributing to this. Vaccine effectiveness can be estimated using a variety of data sources including diagnostic codes from physician billing claims, public health records, reportable disease and laboratory databases. We sought to determine if diagnostic codes from billing claims (administrative data) are adequately sensitive and specific to identify pertussis cases among patients who had undergone disease-specific la

Atypical Presentation of Bordetella pertussis in a Six-Month-Old with Cleaved Lymphocytes and Mild Respiratory Distress.

Bordetella pertussis, the causative agent of whooping cough, typically presents in infants with paroxysmal coughing, inspiratory whoop, and post-tussive vomiting; however, atypical presentations are increasingly recognized, particularly in partially or recently immunized individuals. We present the case of a six-month-old previously healthy, fully immunized female infant who was evaluated for a three-day history of low-grade fever, feeding difficulties, increased irritability, and mild respiratory symptoms, in the absence of the characteristic pertussis cough or respiratory distress. A data driven clinical algorithm for differential diagnosis of pertussis and other respiratory infections in infants.

BACKGROUND: Clinical criteria for pertussis diagnosis and clinical case definitions for surveillance are based on a cough lasting two or more weeks. As several pertussis cases seek care earlier, a clinical tool independent of cough duration may support earlier recognition. We developed a data-driven algorithm aimed at predicting a laboratory confirmed pertussis. METHODS: We enrolled children <12 months of age presenting with apnoea, paroxistic cough, whooping, or post-tussive vomiting, irrespective of the duration of cough. Patients underwent a RT-PCR test for pertussis and other viruses. Thr

Multifractal signatures of infectious diseases.

Incidence of infection time-series data for the childhood diseases measles, chicken pox, rubella and whooping cough are described in the language of multifractals. We explore the potential of using the wavelet transform maximum modulus (WTMM) method to characterize the multiscale structure of the observed time series and of simulated data generated by the stochastic susceptible-exposed-infectious-recovered (SEIR) epidemic model. The singularity spectra of the observed time series suggest that each disease is characterized by a unique multifractal signature, which distinguishes that particular

[Cellular and acellular anti-pertussis vaccines].

Since 1940, vaccination with whole cell vaccines, composed of Bordetella pertussis extracts, has dramatically reduced the incidence of whooping cough. However, the occurrence of side effects has stimulated research that has resulted in the design of new, acellular vaccines. These vaccines are composed of adhesins, such as filamentous haemagglutinin and pertactin, and detoxified pertussis toxin. Detoxification can be achieved by genetic engineering resulting in alterations of specific amino acid residues that are involved in the enzymatic activity or in the target-cell receptor binding activity

Novel Strategies to Inhibit Pertussis Toxin.

Pertussis, also known as whooping cough, is a respiratory disease caused by infection with Bordetella pertussis, which releases several virulence factors, including the AB-type pertussis toxin (PT). The characteristic symptom is severe, long-lasting paroxysmal coughing. Especially in newborns and infants, pertussis symptoms, such as leukocytosis, can become life-threatening. Despite an available vaccination, increasing case numbers have been reported worldwide, including Western countries such as Germany and the USA. Antibiotic treatment is available and important to prevent further transmissi

What is chronic cough in children?

The cough reflex is modulated throughout growth and development. Cough-but not expiration reflex-appears to be absent at birth, but increases with maturation. Thus, acute cough is the most frequent respiratory symptom during the first few years of life. Later on, the pubertal development seems to play a significant role in changing of the cough threshold during childhood and adolescence resulting in sex-related differences in cough reflex sensitivity in adulthood. Asthma is the major cause of chronic cough in children. Prolonged acute cough is usually related to the long-lasting effects of a p

[Tracheal compression by the innominate artery (author's transl)].

A five week old male infant is treated under suspicion of whooping cough because of pertussis like cough, cyanosis, and attacks of reflex apnea. These attacks are seen up to 40 times per day. After exclusion of a cerebral genesis there was found a ventral indentation of the trachea. It was also seen by tracheoscopy, but this part of the trachea was not pulsating. In the angiography the compriming structure was identified as the innominate artery. The genesis of this anatomic variant, the often threatening symptoms in this case without inspiratoric stridor, and the diagnostic and therapeutic st

Adaptive immune response to bordetella pertussis during vaccination and infection: emerging perspectives and unanswered questions.

INTRODUCTION: Whooping cough, also known as pertussis, remains a significant challenge as a vaccine-preventable disease worldwide. Since the switch from the whole-cell Pertussis (wP) vaccine to the acellular Pertussis vaccine (aP), cases of whooping cough have increased in countries using the aP vaccine. Understanding the immune system's response to pertussis vaccines and infection is crucial for improving current vaccine efficacy. AREAS COVERED: This review of the literature using PubMed records offers an overview of the qualitative differences in antibody and T cell responses to B. pertussi

A comprehensive resource for Bordetella genomic epidemiology and biodiversity studies.

The genus Bordetella includes bacteria that are found in the environment and/or associated with humans and other animals. A few closely related species, including Bordetella pertussis, are human pathogens that cause diseases such as whooping cough. Here, we present a large database of Bordetella isolates and genomes and develop genotyping systems for the genus and for the B. pertussis clade. To generate the database, we merge previously existing databases from Oxford University and Institut Pasteur, import genomes from public repositories, and add 83 newly sequenced B. bronchiseptica genomes.

Temporal analysis of French Bordetella pertussis isolates by comparative whole-genome hybridization.

Bordetella pertussis, a gram-negative beta-proteobacterium, is the agent of whooping cough in humans. Whooping cough remains a public health problem worldwide, despite well-implemented infant/child vaccination programs. It continues to be endemic and is observed cyclically in vaccinated populations. Classical molecular subtyping methods indicate that genome diversity among B. pertussis isolates is limited. Although the whole bacterial genome has been studied by pulsed-field gel electrophoresis, the genes implicated in the diversity have not been identified. We developed a B. pertussis whole-ge

[Childhood diseases, infectious diseases, and fever as potential risk factors for cancer?].

OBJECTIVE: We investigated the assertion that past childhood diseases, acute and febrile infections as well as allergies have a preventive effect on cancer. Former studies on this topic show controversial results and methodical deficits. PATIENTS AND METHODS The investigation was conducted as a retrospective case control study with 111 cancer patients and a group of 109 control persons from 3 hospitals in Berlin. RESULTS: A significant change in the risk of developing cancer could only be observed for mumps (OR = 2.6; increased risk), whooping cough (OR = 2.7; increased risk), and colds in th

Pertussis: a reemerging infection.

Pertussis, also known as whooping cough, is an acute respiratory tract infection that has increased in incidence in recent years. The initial catarrhal stage presents with nonspecific symptoms of malaise, rhinorrhea, sneezing, lacrimation, and mild cough. During the paroxysmal stage, severe outbreaks of coughing often lead to the classic high-pitched whooping sound patients make when gasping for breath. The paroxysmal stage is followed by the convalescent stage and resolution of symptoms. Complications vary by age, with infants more likely to experience severe complications such as apnea, pneu

Postinfectious cough: ACCP evidence-based clinical practice guidelines.

BACKGROUND: Patients who complain of a persistent cough lasting >3 weeks after experiencing the acute symptoms of an upper respiratory tract infection may have a postinfectious cough. Such patients are considered to have a subacute cough because the condition lasts for no >8 weeks. The chest radiograph findings are normal, thus ruling out pneumonia, and the cough eventually resolves, usually on its own. The purpose of this review is to present the evidence for the diagnosis and treatment of postinfectious cough, including the most virulent form caused by Bordetella pertussis infection, and mak
