We determined the site of the focal illumination from the Zeiss OPMI-6 operating microscope on the retina of the phakic and aphakic human cadaver eye by directly observing the illuminating element image on the posterior scleral surface of the globe. With the eye straight ahead and the operating microscope level, the focal oval area of retinal illumination was located superior to the foveola in both the phakic and aphakic eye. Tilting the operating microscope 10 degrees toward the surgeon displaced the entire illuminating element image 0.50 mm below the foveola in the phakic eye and 0.25 mm bel

We undertook a follow-up case study of 126 patients (146 eyes) to determine success rate and replacement rate of the Bausch & Lomb CW 79 aphakic extended wear contact lens. One hundred eighteen eyes (81%) were successful in wearing the contact lens, and 16 eyes (11%) were reported as contact lenses dropouts. Twelve eyes (8%) elected to receive intraocular lenses (IOLs). A total of 134 lenses were replaced, yielding a replacement rate of 0.3 lenses per eye per year. The average length of follow-up for successful patients was 3.7 years.

When silicone oil fills the vitreous cavity of the phakic eye or the entire aphakic eye, the refractive correction can be expected to change by 5-9 diopters and sometimes as much as 14 diopters. Aphakic eyes become less hyperopic when filled with silicone oil, whereas phakic eyes become more hyperopic when the vitreous cavity is filled with silicone oil. Previously emmetropic aphakic eyes that are filled with silicone oil have only 4-6 diopters of hyperopia, whereas the same eyes have 10-12 diopters of hyperopia before insertion or after removal of silicone oil. Phakic eyes develop 5-7 diopter

Infants born with a unilateral congenital cataract have always been considered a particularly difficult group of patients to treat. Our experience in fitting contact lenses to such children, after cataract removal, is described. Contact lenses must be specially designed, to account for the significant differences in size and shape of the infant eye from that of an adult eye. A group of 15 children born with a unilateral cataract, were fit with contact lenses, and have been treated for amblyopia for an average of less than 2 years. Eight patients have obtained acuities measured by preferential

The clinical findings, surgical techniques, anatomic results and visual recovery in 71 pseudophakic and 71 aphakic eyes with retinal detachment repaired concurrently were compared. Preoperative visibility of the peripheral retina was significantly reduced in the pseudophakic group. Intraoperative identification of retinal breaks was useful for the pseudophakic eyes. The surgical techniques used in the two groups were similar. Anatomic success was achieved in 65 pseudophakic eyes (92%) and 63 aphakic eyes (89%); however, the visual recovery after a mean follow-up period of 18 months was poorer

One case of aphakic retinal detachment with increased intraocular pressure is described. Overnight the intraocular pressure fell abruptly to non-measurable values, and simultaneously the detachment had advanced to the optic disc border. The hypothesis is formulated that decreased intraocular pressure in eyes with retinal detachment is brought about because intraocular fluid is leaking out of the eye through a subretinal route to the perioptic connective tissue. Additional support for this posterior aqueous outflow is obtained from clinical studies showing that maximum hypotony is reached when
