Management of bone defects due to infected non-union or chronic osteomyelitis with autologous non-vascularized free fibular grafts.

OBJECTIVES: Bone defects as a result of infected non-union or chronic osteomyelitis are difficult to manage. The purpose of this study was to present the results of treatment of bone defects of < 6 cm due to a previous infected non-union or chronic osteomyelitis with autologous non-vascularized fibular grafts in a 2-stage surgery. PATIENTS AND METHODS: The records of patients who were treated with autologous non-vascularized fibular grafts for bone defects of < 6 cm due to a previous infected non-union or chronic osteomyelitis between 2008 and 2013 were retrospectively reviewed. Primary compl

Distal tibial triplane fractures: long-term follow-up.

The cases of 35 patients in whom a triplane fracture had been treated at the Ramón y Cajal Hospital were reviewed. The diagnosis of biplane and triplane fracture can now be made by standard radiograph. However, plain radiographs alone did not accurately demonstrate the configuration of the fracture, and computed tomography must be performed. Closed reduction should be attempted first. Failure to obtain or maintain (or both) an adequate closed reduction is an indication for operative treatment. Prognosis is surprisingly good. Only when adequate reduction (< 2 mm displacement) has not been achie

Enterococcus thailandicus as an uncommon pathogen in severe lower extremity trauma: a case report.

BACKGROUND: Enterococcus thailandicus is a recently described enterococcal species with uncertain clinical significance. While Enterococcus faecium and Enterococcus faecalis are well recognized as nosocomial pathogens, clinical reports of E. thailandicus remain exceedingly rare. CASE PRESENTATION: We report the case of a 46-year-old Thai male construction worker in Taiwan who sustained a severe forklift-related lower extremity injury in January 2025, including extensive degloving, tibialis anterior tendon rupture, and fibula fracture. Despite multiple surgical debridements and soft tissue reco

Leg Impalement - A Rare Injury Pattern Case Report.

INTRODUCTION: Impalement injuries are defined as rare, high-energy lesions caused by foreign bodies, usually steel bars or wooden objects, which pierce body cavities or extremities and remain interposed in the perforated body region. They usually occur with road accident or civil construction falls. CASE REPORT: A 24-year-old male patient was admitted at the emergency department after a motorcycle accident, resulting a left leg impalement with a wooden object. A partial deep peroneal nerve palsy and a proximal third fibula fracture were observed. The foreign body removal and wound debridement

Free non-vascularized fibular graft reconstruction after resection of metacarpal unicameral bone cyst: A case report.

Unicameral bone cyst (UBC) or simple bone cyst (SBC) is a benign cystic lesion commonly found in the humerus and femur that is mainly encountered during childhood. The currently available treatments for UBC of the hand commonly involve curettage, bone grafting, partial resection with or without grafting, multiple drilling, fracture immobilization and observation alone, and steroid injection. We herein report a case treated with total resection of the cyst and non-vascularized fibular graft in a 9-year-old right-handed female patient. The patient presented with a chief complaint of a large, fas

Prolonged central apnoea after intravenous morphine administration in a 12-year-old male with a UGT1A1 loss-of-function polymorphism.

ADVERSE EVENT: Repeated and prolonged episodes of central apnoea and hypoxia after receiving intravenous morphine for analgesia and ketamine for sedation. DRUG IMPLICATED: Intravenous morphine sulfate. THE PATIENT: Previously healthy 12-year-old male with no history of sleep apnoea who presented with distal tibia and fibula fracture. EVIDENCE THAT LINKS DRUG TO EVENT: Pharmacogenomic testing revealed that the patient was homozygous for the T allele at the rs887829 SNP in UGT1A1, an enzyme involved in the metabolism of morphine. This polymorphism is a loss-of-function variant, leading to imp

A Previously Discounted Flap Now Reconsidered: MatriDerm and Split-Thickness Skin Grafting for Tendon Cover Following Dorsalis Pedis Fasciocutaneous Flap in Lower Limb Trauma.

OBJECTIVE: The dorsalis pedis flap has reliable vascularity; however, its use is limited by reports of donor site morbidity including infection, delayed healing, exposure of tendons, and later contractures. The purpose of this study was to demonstrate its continued role in lower limb trauma when the donor site is reconstructed with MatriDerm to avoid complications. METHODS: A 65-year-old man presented with a displaced, Gustilo 3b open transverse fracture of his left distal fibula. He had a 2 cm(2) open wound over his lateral malleolus. RESULTS: Following review of possible local options, a d

Benign fibrous histiocytoma of bone in a paediatric population: a report of 6 cases.

Case records and radiological investigations of six children with benign fibrous histiocytoma were studied retrospectively. BFH occurred in the femur (n = 2), tibia (n = 2) and fibula (n = 2). Clinically, patients reported pain from the lesion lasting several months (mean 6 months). The pain was not associated with pathological fracture in any patient. On X-rays, the lesions appeared as lytic and sharply demarcated with a sclerotic rim and fine trabeculations. The reported cases were located in the metaphysis and the diaphysis of the long bones. The tumour was restricted to bone, without perio

Benign fibrous histiocytoma of bone: a report of ten cases and review of literature.

The benign fibrous histiocytoma is a rare tumor with only a few descriptions. In order to better define the optimal therapeutic procedure and the necessity of surgery we retrospectively analyzed the patients of the orthopedic department. Benign fibrous histiocytoma occurred in the femur (n = 3), pelvis (n = 2), humerus, tibia, fibula, rib and spine. Pain as the cardinal symptom and a median age of 28 years are factors that differentiate benign fibrous histiocytoma from other metaphyseal fibrous lesions such as the nonossifying fibroma. Radiographic investigation of this entity showed osteolyti

Comparative effectiveness of nitinol staple-only fixation versus antiglide plate fixation for Weber type B distal fibular fractures.

BACKGROUND: Interest in less invasive surgeries for Weber type B distal fibular fracture has increased recently. This study aimed to demonstrate that nitinol staple-only fixation is less invasive compared to antiglide plate fixation. MATERIALS AND METHODS: This retrospective review involved 59 patients with Weber type B fibular fractures who underwent surgery between 2018 and 2023. Twenty-eight patients underwent antiglide plate fixation, whereas 31 underwent multiple nitinol staple-only fixation. The intraoperative assessment included skin incision length and operative time. The radiographic

Fat grafting for resurfacing an exposed implant in lower extremity: A case report.

RATIONALE: Although numerous reconstruction protocols have been reported for lower leg trauma, those for distal leg trauma remain few. We present the case of a woman with an implant exposure wound, who was successfully treated through fat grafting, without major flap surgery. PATIENT CONCERNS: An 83-year-old woman with an exposed implant in lower extremity received reconstruction surgery once and the surgery failed. She refused additional major surgery and negative pressure wound therapy. DIAGNOSES: The diagnosis of a tibia and fibula shaft open fracture (type IIIA) complicated with an expos

Late results of free-muscle flaps and delayed bone grafting in the secondary treatment of open distal tibial fractures.

Since no current studies reflect the long-term function of patients accurately categorized and uniformly managed for the same degree and location of tibial injury, seven individuals with type IIIB open distal tibial fractures were studied prospectively for 2 to 4 years after debridement, free-muscle transfer, and delayed autologous bone grafting. Lymphedema, transient drainage, poor ankle motion, limb shortening, nonunion, and delayed union were all significant problems. On average, the patients endured over six operations, 2 months of hospitalization, and a year's course of physical rehabilit

A classification manager for compositional concept systems exemplarily shown by the AO/ASIF classification of fractures of long bones.

Conventional classification and coding systems represent concept systems by strict hierarchical enumeration and are supported by meaningful codes. Compositional classification is a means for representing concept systems by semantic descriptions. Classification is based on the structure of concept descriptions and explicit hierarchical relationships between their constituents. A classification manager will be presented which is based on the BERNWARD model [1]. BERNWARD is a conceptual graphs formalism and allows the constrained composition of concept descriptions by primitive concepts and roles
