BACKGROUND: Human hands play an important role in overcoming routine daily life. As a consequence of limitations in the function of the hand due to rheumatic diseases, various manual activities can become an enormous challenge or even become absolutely impossible.
MATERIAL AND METHODS: This review article discusses the possibilities of hand ergotherapy and explains the most important assist devices.
RESULTS AND CONCLUSION: The main area of occupational and hand therapy interventions in patients with rheumatoid arthritis is the postoperative or conservative treatment of wrist and finger joint…

Dupuytren contracture of the proximal interphalangeal (PIP) joint can be reversed by an extension torque transmitted from an external device, the Digit Widget, by skeletal pins to the middle phalanx. This extension torque, generated by the same elastic bands dentists use to align teeth, gradually restores length to soft tissues palmar to the PIP joint's axis of rotation. Simultaneously, tissues dorsal to the joint's axis will shorten toward normal length as the PIP progressively straightens. Although the contractile nodules and bands of Dupuytren disease may be excised either before or after r

We present a modification to the widely used lead hand retraction system that allows for improved surgical access to a digit with a severe flexor contracture. A simple construct using surgical adhesive tape and a silk suture can be made and applied to the digit and lead hand. Our technique is simple and inexpensive to use, and we recommend its use to improve surgical access without causing unnecessary trauma to the digit.

Pachydermodactyly is an uncommon variant of digital fibromatosis that has previously been related to knuckle pads. It is a benign condition that needs no investigation for an underlying systemic disease. We present a new case of pachydermodactyly and comment on the available literature.

Tenotomy of the dorsal apparatus over the middle phalanx was done in 13 digits for treatment of posttraumatic hyperextension of the distal interphalangeal joint associated with a boutonniere deformity. All but one patient was improved with distal interphalangeal joint flexion averaging 45 degrees at follow-up. Four digits had a 5 degree to 10 degree extensor lag. This procedure is simple and reliable and should be considered when increased distal interphalangeal joint flexion is desired.

The control of joint stiffness is a fundamental mechanism used to control human movements. While many studies have observed how stiffness is modulated for tasks involving shoulder and elbow motion, a limited amount of knowledge is available for wrist movements, though the wrist plays a crucial role in manipulation. We have developed a computational framework based on a realistic musculoskeletal model, which allows one to calculate the passive and active components of the wrist joint stiffness. We first used the framework to validate the musculoskeletal model against experimental measurements o

PURPOSE OF THE STUDY Arthrodesis of interphalangeal joints of the hand is a method used to treat conditions associated with joint destruction, instability and pain. Our study aims to evaluate the outcomes of the treatment of sequelae of septic arthritis of interphalangeal joints by arthrodesis with external fixation. This topic is hardly covered in literature. Moreover, it compares the outcomes of application of this method in diabetic patients and non-diabetic population. MATERIAL AND METHODS Arthrodesis by means of Stellbrink external fixator was performed in 17 patients who had suffered sep

Isolated volar metacarpophalangeal dislocations of fingers are extremely rare. There are few cases published in English and French literature. In this article, we aim to review the literature and present a case of isolated open volar dislocation of a finger. We treated this dislocation first by a dorsal approach alone and later, after recurrence, using a combined dorsal and volar approach. This report emphasizes the pathology of such injuries, clarifies the mechanisms, and outlines the treatment options of the dislocations. Close reduction can be achieved and maintained if done early. It must

We report a boy with Desbuquois dysplasia type 1. He had the typical skeletal changes: a "Swedish key" appearance of the proximal femora; advanced carpal ossification and other distinctive features of the hand, including an extra-ossification center at the base of the proximal phalanx of the index and middle fingers; dislocation of the metacarpophalangeal joint of the index finger; and bifid distal phalanx of the thumb. In addition, he presented with very severe prenatal growth failure, respiratory distress as a neonate, subsequent failure to thrive and susceptibility to airway infection, and

The aim of this study was to investigate the correlation between the severity of Dupuytren contracture and disability. The American Medical Association (AMA) guidelines were used to assess the impairment for each involved hand. The disability of the arm, shoulder and hand (DASH) questionnaire was used for evaluating the disability. Eighty patients with 102 involved hands were enrolled. The mean total flexion contracture was 119 degrees /hand or 65 degrees /ray. The mean DASH score was 15 (range 0-69). There was no significant correlation between the DASH score on one hand and the total flexion

Dislocated metacarpal V fractures, fixed by percutaneous Kirschner wires or other implants near the metacarpophalangeal joint, often show impaired movement despite excellent radiologic findings. We therefore chose an alternative method using intramedullary Kirschner wire osteosynthesis according to Foucher. Twenty-five patients were operated on in a 3-year period and movement of the fingers and complications were examined. Six weeks after operation all fractures were consolidated in anatomical reduction. One patient suffering from an early stage of Sudeck's dystrophy was healed, and no other c

Six patients with severe hand and forearm injuries involving open wounds and exposed structures were treated with reverse radial forearm fascial flaps and split-thickness skin grafts for soft tissue cover. There were five men and one woman aged between 16 and 36 years. Injuries included soft tissue avulsion on the dorsum of the hand and fingers, extensive flexor and extensor tendon damage, multiple phalangeal fractures, a grade IIIB open dislocation of the index to little carpometacarpal joints, a grade III open metacarpal fracture and a finger amputation. The average wound size was 9 cm in le

BACKGROUND: Musculoskeletal ultrasound is increasingly used in the evaluation of joint inflammation. Power Doppler can identify increased hyperemia which indicates inflammation.
OBJECTIVES: To measure the prevalence and level of positive Power Doppler signals at the hand joints of scleroderma patients.
METHODS: Ten scleroderma patients were examined by ultrasound of the dominant hand. Power Doppler signals were measured at the wrist as well as the metacarpophalangeal joints (MCPS), proximal interphalangeal joints (PIPS) and distal interphalangeal joints (DIPS) at the second to fifth fingers.

Pachydermodactyly is a superficial benign fibromatosis of unknown etiology; it is rare, more frequent in adolescent males, and characterized by painless swelling of the proximal interphalangeal joints(PIP) of the hands. Histologic examination of the skin shows epidermal hyperplasia and increased number of dermal fibroblasts and collagen fibers. We report the case of a 16-year-old adolescent boy who presented swelling of the lateral and dorsal regions of all the metacarpophalangeal and PIP joints of the left hand and PIP and metacarpophalangeal joints of the second and fifth fingers of the right

From 1983 to 1990, 30 free toe transplantations were performed in 30 children suffering from congenital malformations of the hand. Especially the two severe forms of symbrachydactylia presented major indications. In addition to toe transplantation onto the ulnar ray, lengthening of the radial ray was frequently necessary. Whereas 25 transplantations were successful, there were one necrosis and four partial necroses of the toe. In the monodactylous form, all 15 children with successful transplantation showed good and very good functional results. Children suffering from the peromelic form could

OBJECTIVE: This study aimed to characterize patterns of joint involvement and clinical features in patients with difficult-to-treat rheumatoid arthritis (D2T RA).
METHODS: A cross-sectional analysis was conducted using the FRANK Registry, a multicentre cohort (n = 3770). D2T RA was defined based on the 2021 European Alliance of Associations for Rheumatology criteria. Propensity score matching by age and disease duration was applied to compare D2T RA patients with matched non-D2T RA. Joint involvement was assessed using 28-joint counts and Paired Joint Pathology Scores. Patient satisfaction wa

Distal arthrogryposis IIB is characterized by contractures of the distal joints (especially of the fingers and toes) and ptosis. We recently encountered a father and son with these manifestations. The father was reported 54 years ago as a case of amyoplasia congenita (arthrogryposis multiplex congenita). Both father and son have distal joint contractures, most severe in the hands and feet, as well as ptosis and ophthalmoplegia. In addition, these patients have an unusual distribution of hair loss, and conical teeth. Whether these latter findings are related to the type of distal arthrogryposis
