[Bone densitometry in inflammatory rheumatic diseases : Characteristics of the measurement site and disease-specific factors].

Bone densitometry should be performed earlier in patients with inflammatory arthritis, since factors such as inflammation and drug therapy, in particular treatment with glucocorticoids, have an important impact on the development of osteoporosis. DXA (Dual energy X-ray Absorptiometry) is considered the gold standard for bone densitometry. According to the German guidelines for osteoporosis, bone densitometry plays a crucial role in the choice of therapy. In patients with rheumatoid arthritis, measurement of peripheral bone (forearm) density in addition to lumbar spine and hip is recommended, si

[Spontaneous healing of a juvenile bone cyst of the tibia after pathological fracture].

Up to the present only few cases of spontaneous healing of juvenile bone cysts after pathological fractures were published. Most authors will not accept the possibility of a "healing fracture" in bone cysts because of missing documentation. In our clinic we could observe spontaneous healing of a juvenile bone cyst of the tibia after a pathological fracture in a ten-year old boy. Therefore healing of bone cysts after fracture is proven. On the other hand healing of bone cysts following conservative treatment after a fracture is not sure. Conservative treatment depends on the localization and ty

Solid variant of an aneurysmal bone cyst (giant cell reparative granuloma) of the 3rd lumbar vertebra.

A 9-year-old girl with a solid variant of an aneurysmal bone cyst in the 3rd lumbar vertebra showed a good response to low-dose radiation as the only treatment. The solid variant of aneurysmal bone cyst is thought to be a reactive response to intraosseous hemorrhage and is also called giant cell reparative granuloma or giant cell reaction. These lesions in the jaw and the short tubular bones of the hands and feet frequently recur after surgery. Aneurysmal bone cysts of the spine also show a fairly high recurrence rate after incomplete resection or radiation therapy. However, 7 previous cases o

Giant Cell Reparative Granuloma in the Thumb Distal Phalanx: A Case Report.

Giant cell reparative granulomas (GCRG) often affect the bones of the hands and the feet. Treatment of this lesion depends on the exact location and amount of localized bony destruction. Ours is the first case report to discuss the nuances of treating this lesion in the thumb distal phalanx. A 19-year-old male presented with lytic, destructive expansion of his left thumb distal phalanx; imaging was suggestive of an aneurysmal bone cyst. Open biopsy was interpreted as giant cell reparative granuloma. Curettage and bone grafting resulted in complete healing of the distal phalanx with an excellen

Aspirin effect on bone remodeling and skeletal regeneration: Review article.

Bone tissues are one of the most complex tissues in the body that regenerate and repair themselves spontaneously under the right physiological conditions. Within the limitations of treating bone defects, mimicking tissue engineering through the recruitment of scaffolds, cell sources and growth factors, is strongly recommended. Aspirin is one of the non-steroidal anti-inflammatory drugs (NSAIDs) and has been used in clinical studies for many years due to its anti-coagulant effect. On the other hand, aspirin and other NSAIDs activate cytokines and some mediators in osteoclasts, osteoblasts and t

Osteocalcin as a hormone regulating glucose metabolism.

The number of patients with osteoporosis and diabetes is rapidly increasing all over the world. Bone is recently recognized as an endocrine organ. Accumulating evidence has shown that osteocalcin, which is specifically expressed in osteoblasts and secreted into the circulation, regulates glucose homeostasis by stimulating insulin expression in pancreas and adiponectin expression in adipocytes, resulting in improving glucose intolerance. On the other hand, insulin and adiponectin stimulate osteocalcin expression in osteoblasts, suggesting that positive feedforward loops exist among bone, pancre

Renal osteodystrophy in predialysis patients without stainable bone aluminum. A cross-sectional bone-histomorphometric study.

Histomorphometry was performed on transiliac bone biopsies, double-labeled with tetracycline, from 60 consecutively admitted patients (20 women) at various stages of chronic renal failure (CRF). Eleven patients (1 woman) had normal bone resorption and formation indices. Bone resorption and osteoid formation increased with progression of renal failure, but abnormal values were seen even at slightly elevated creatinine levels. Mineralization lag time increased with CRF duration; prolonged values were only seen in patients with polycystic kidney disease or chronic pyelonephritis with advanced CRF

[Apparent pseudarthrosis of the thumb].

Pseudarthrosis in the hand are defined as a non-union of a fracture after 6 month. Reasons might be biological disorders, mechanical instability or infection after fracture treatment. Pseudoarthrotic bone demands stable osteosynthesis and often bone grafting. The presented case is about a dislocated osseous fragment attached to the Aponeurosis dorsalis mimicking clinical and radiological a Pseudarthrosis of the proximal phalanx of the thumb. We would like to draw attention that osseous fragments may present very similarly to a pseudoarthrosis of a phalanx.

The hands in Saethre-Chotzen syndrome.

The hand radiographs of fifteen patients with a diagnosis of Saethre-Chotzen syndrome were reviewed by a radiologist with an interest in skeletal dysplasia along with the craniofacial team. Eleven patients exhibited a range of bone abnormalities of the following sites: thumbs, fingers, metacarpals, and the radius. The epiphysis of the distal phalanx of the thumb was most commonly involved site with 7 of the 15 patients affected. In addition to the structural abnormalities all cases before puberty (14 out of 15 patients in this series) showed variable delay of the bone age.

Longitudinal epiphyseal bracket: associated foot deformities with implications for treatment.

The longitudinal epiphyseal bracket is a rare ossification defect of bones of the hands and feet and results in an abnormal epiphysis that is C-shaped, longitudinally oriented, and vertically extends into the diaphysis. The longitudinal epiphyseal bracket may exist as an isolated deformity, but is commonly associated with digital deformities such as polydactyly, duplicated digits, congenital hallux varus, and clinodactyly. Five case histories of children with the longitudinal epiphyseal bracket in different bones of the foot are described with three distinct clinical presentations. The develop

Pulmonary anthrax caused by contaminated sacks.

A 54-year-old Jamaican employed as a grinding machine operator developed pulmonary anthrax and died within two days. In the eight days before his illness he had been grinding sterilized bone charcoal delivered in second-hand sacks, some of which had been used to import the raw bone before its sterilization. Bacillus anthracis was isolated from four out of six sacks examined and is considered to have been the source of the infection.

Gene dosage of the transcription factor Fingerin (bHLHA9) affects digit development and links syndactyly to ectrodactyly.

Distal limb deformities are congenital malformations with phenotypic variability, genetic heterogeneity and complex inheritance. Among these, split-hand/foot malformation is an ectrodactyly with missing central fingers, yielding a lobster claw-like hand, which when combined with long-bone deficiency is defined as split-hand/foot malformation and long-bone deficiency (SHFLD) that is genetically heterogeneous. Copy number variation (CNV) consisting of 17p13.3 duplication was identified in unrelated pedigrees, underlying SHFLD3 (OMIM 612576). Although the transcription factor Fingerin (bHLHA9) is

Bone regeneration after pedicled periosteal dorsal metacarpal flap. Reconstructive technique.

The dorsal metacarpal artery (DMCA) flap is considered as one of the working horses to cover exposed extensor tendon or bone of dorsal digits. The periosteal composite DMCA reverse flap (pcDMCAr flap) is described as a fast and safe solution to manage this kind of trauma. A 35-year-old male had a trauma to his left hand from a circular saw. The resultant injury was localized to the proximal middle finger with a dorsal bone loss. A vascularized composite flap, including 3th metacarpal periosteum, was elected as the most appropriate option. Postoperative follow-up at 6 months confirmed bony rege

The metacarpal index revisited: a brief overview.

Metacarpal index (MCI) is combined cortical thickness (both sides)) normalized with regard to outer bone diameter of the measuring site, the midshaft of the second metacarpal, or the three midmetacarpals of both hands. MCI is reduced with age, particularly in postmenopausal women. It correlates with axial bone mass in group studies. Measurement of the MCI in its modern version, digital X-ray radiogrammetry (DXR), requires only a plain analog radiograph, a PC, a film scanner, and reliable software. MCI can be used diagnostically and longitudinally for monitoring changes. MCI measured with DXR h

Bone age delay in Perthes disease and transient synovitis of the hip.

Patients with transient synovitis of the hip were found to have a bone age delay similar to the one found in patients in the active stages of Perthes disease. This finding led to a study on the relationship between the bone and chronologic ages in patients affected by either disease, in activity, and in the residual stage through to the end of growth. Bone age of all patients was established by means of a radiograph of both wrists and hands and the quantified data were compared with the Greulich and Pyle Atlas. All the patients in the active stage of both diseases revealed a bone age delay wit

Increased bone apposition in primary hyperparathyroidism: measurements based on short interval tetracycline labeling of bone.

Bone biopsies of 23 subjects suffering from primary hyperparathyroidism, which was proven by the demonstration of parathyroid pathology during neck surgery, were studied by morphometric analysis and short interval sequential tetracycline labelling and compared with those of 13 patients without metabolic bone disease. In 19 of these patients, abnormalities were found in the trabecular volume, resorption surface, formation surface, or in various combinations. However, no constant pattern of bone changes was observed. In 4 patients, the bone morphometric parameters were normal. The bone appositio

A variant of Desbuquois dysplasia characterized by advanced carpal bone age, short metacarpals, and elongated phalanges: report of seven cases.

We present the clinical and radiological findings of seven patients with a seemingly new variant of Desbuquois dysplasia (DBQD) and emphasize the radiographic findings in the hand. All cases showed remarkably accelerated carpal bone ages in childhood, but none of the patients had an accessory ossification center distal to the second metacarpal, or thumb anomalies, instead, there was shortness of one or all metacarpals, with elongated appearance of phalanges, resulting in nearly equal length of the second to fifth fingers. The two sibs followed for 20 years showed narrowing and fusion of the in
