Iliac bone-block autograft for posterior shoulder instability.

INTRODUCTION: Posterior shoulder instability is a rare condition, representing only 4% of all shoulder-joint instabilities. Numerous surgica … o return to their preoperative sports activity level. Three required an additional procedure, 2 for hardware removal and 1 for posterior deltoid repair, which all lead to an uneventful evolution. Imaging at follow-up (X-ray or CT) did not show any instance of bone-block pseudoarthrosis or osteolysis nor did it exhibit glenohumeral early degenerative changes. In al …

[Tibia nonunion after intramedullar nailing for fracture: decortication and osteosynthesis by medial plating].

PURPOSE OF THE STUDY: Intramedullar nailing is now widely used for fractures of the tibia. Extension of nailing indications to proximal, distal, and comminuted fractures has led to a significant rate of complications. The purpose of this study was to analyze decortication and medial plating for the treatment of tibial nonunion after intramedullary nailing. MATERIAL AND METHODS: Eighteen cases of aseptic tibial nonunion after nailing for fracture were treated in three women and fifteen men, mean age 39 years (19-57). The initial classification was open (n = 8), segmental (n = 3), single-focus

Factors associated with infection following open distal radius fractures.

Open fractures are often classified according to a system described by Gustilo and Anderson. However, this system was applied to open long b … Anderson type II and Swanson type I open fracture treated with one debridement and plate fixation. Hardware removal, debridement, and antibiotics resolved the infection. Three contaminated fractures that healed uneventfully received two debridements. Statistical analysis revealed a correlation with infection and contamination (p = 0.0331). The number of initial d …

Percutaneous Femoral Derotational Osteotomy in the Skeletally Immature Patient.

UNLABELLED: Percutaneous femoral derotational osteotomies are performed in both adult and pediatric patients for excessive symptomatic femoral anteversion or retroversion1,2. The aim of the procedure is to correct version abnormalities with use of a minimally invasive technique3. DESCRIPTION: This is a percutaneous procedure that involves creation of femoral drill holes at the osteotomy site prior to reaming the canal4. External fixator pins are placed proximal and distal to the osteotomy site prior to completing the osteotomy. These pins are derotational markers for the surgeon and act to ho

Risk assessment and quality management in AIO based on CT-linac for nasopharyngeal carcinoma: An improved FMEA and FTA approach.

BACKGROUND: All-in-one radiotherapy workflow (AIO) is a novel one-stop solution that integrates the multiple conventional radiotherapy steps from simulation, contouring, planning, image guidance, beam delivery, and in vivo dosimetry into a single device (integrated computed tomography linac, the uRT-linac 506c), making the treatment process more efficient and convenient while reducing errors for cancer patients' initial radiotherapy. Despite its numerous advantages, the implementation of AIO faces challenges such as interdisciplinary coordination, software and hardware complexity, and reliance

Surgical results and complications in a series of 71 consecutive cervical spondylotic corpectomies.

BACKGROUND: Cervical corpectomy is a common procedure in spondylosis. It is normally a well-tolerated surgery and clinical improvement is widely described. However, it is associated with potential risky complications (subsidence, clinical deterioration, vertebral artery injury...); thus, a judicious surgical indication and a good technique are required. METHODS: We revised retrospectively the clinical evolution and complications of 71 spondylotic corpectomies in a series of 100 consecutive patients operated on due to different ethiological causes from January 2001 to September 2007 at our hos

Reconstruction of the distal aspect of the radius with use of an osteoarticular allograft after excision of a skeletal tumor.

Twenty-four patients had reconstruction of the distal aspect of the radius with use of an osteoarticular allograft, between 1974 and 1992, after excision of a giant-cell tumor (twenty patients), a desmoplastic fibroma (two patients), a chondrosarcoma (one patient), or an angiosarcoma (one patient). Nine giant-cell tumors were recurrent lesions, and eleven were extracompartmental primary lesions that had extended through the cortex or subchondral bone. The average age of the patients was 31.5 years (range, fifteen to sixty-one years); thirteen patients were female and eleven were male. Seventee

[Ventral interbody spondylodesis in injuries of the cervical spine. Indications, surgical technique and results].

Lower cervical spine injuries with instability of the anterior and/or posterior column can be treated by anterior interbody fusion and plate fixation. Plates available for anterior instrumentation of the lower cervical spine can be divided into locking or non-locking systems with uni- or bicortical screw purchase. Our biomechanical comparative testing of different screw fixation systems demonstrates improved stability with the use of bicortical purchase. Clinical studies, however, have proven high fusion rates without loss of correction and a low implant related morbidity with the use of unico

The changing paradigm of spine motion preservation: A bibliometric analysis of dynamic posterior stabilization.

OBJECTIVE: Traditional rigid posterior lumbar fusion frequently induces adjacent segment disease due to non-physiological stress concentrations. While dynamic and semi-rigid posterior stabilization systems were introduced to preserve segmental motion and share biomechanical loads, the expanding global literature remains fragmented. This study aims to perform a comprehensive bibliometric and computational science mapping analysis to decode the global intellectual architecture, collaborative networks, and shifting research frontiers of dynamic posterior spinal stabilization from 1999 to June 202

Opening-wedge high tibial osteotomy performed with locking plate fixation (TomoFix) and early weight-bearing but without filling the defect. A concise follow-up note of 48 cases at 10 years' follow-up.

INTRODUCTION: Use of a locking plate during medial opening high tibial osteotomy (HTO) eliminates the need to fill the defect and its associated complications. It also allows early weight-bearing. Since long-term data with this type of construct are rare, we wanted to report the outcomes of a 51-patient cohort evaluated after a mean follow-up of 10.2years. The goals were to determine (1) the HTO survival and whether the correction was maintained between 2 and 10years later, (2) the stability of the functional outcomes, (3) the complication rate related to use of locking plates. HYPOTHESIS: Th

Total hip arthroplasty after failed fixation of a proximal femur fracture: Analysis of 59 cases of intra- and extra-capsular fractures.

INTRODUCTION: The indications for total hip arthroplasty (THA) after failed internal fixation of a proximal femur fracture vary. Published studies on this topic are broad-ranging and do not distinguish between intracapsular and extracapsular fractures. This led us to conduct a retrospective analysis comparing the clinical outcomes, radiological outcomes, technical problems and complications between these two types of fractures. HYPOTHESIS: The functional outcomes of THA after an extracapsular fracture will be worse than the ones after an intracapsular fracture. MATERIAL AND METHODS: This was

Does the Utilization of Allograft Tissue in Medial Patellofemoral Ligament Reconstruction in Pediatric and Adolescent Patients Restore Patellar Stability?

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction is one of several surgical procedures used to treat patellofemoral instability. Use of allograft tissue can preserve autogenous tissue and may be preferable in patients with connective tissue disorders or ligamentous laxity. Although there are successful reports in adults, it is unclear if the use of allograft tissue in MPFL reconstruction can restore patellofemoral stability in children and adolescents. QUESTIONS/PURPOSES: (1) Does allograft tissue in MPFL reconstruction in pediatric and adolescent patients restore patellar sta
