BACKGROUND: Isolated triceps tendon injuries are infrequent, and their combination with a medial ligament avulsion is even rarer. Eccentric force on the triceps tendon following a fall on the outstretched hand, combined with a valgus torque at the elbow joint, exposes the medial side to stretching forces, potentially disrupting the ulnar collateral ligament (UCL). Furthermore, compressive forces on the lateral side could result in injury to the radial head. Currently, there is no standardized surgical approach for managing such combined injuries.
INDICATIONS: The complex triad injury presente

A Fukuda retractor is placed through the split to hold the humeral head laterally. The labrum is elevated, and the glenoid is prepared with rasp. BACKGROUND: Pediatric femoral neck fractures require stable fixation to avoid complications. It remains unclear whether fixation with the Proximal Humeral Internal Locking System (PHILOS) can serve as an alternative to cannulated screw fixation. The purpose of this study was to compare the biomechanical properties of PHILOS and cannulated screws for stabilizing unstable pediatric femoral neck fractures using a synthetic bone model.
MATERIALS AND METHODS: Twelve fourth-generation synthetic composite femurs were randomly assigned to screw fixation (Group S) or PHILOS fixation (Group P) (n = 6 e

Rotator cuff pathology is a common musculoskeletal condition that disproportionately affects older adults, as well as patients with diabetes mellitus and chronic kidney disease. It is known that increased age and kidney dysfunction have been correlated to acidotic states, which may be related to the increased incidence of rotator cuff injury. In order to investigate the potential relationship between acidosis and rotator cuff composition and mechanics, this study utilizes a 14-day murine model of metabolic acidosis and examines the effects on the supraspinatus tendon-humeral head attachment co…

There is no consensus on how to measure shoulder joint laxity and results reported in the literature are not well systematized for the avail … for measuring glenohumeral laxity and stiffness. Most arthrometers applied an external force to the humeral head or superior humerus by a manual-assisted mechanism. Glenohumeral laxity and stiffness were mostly assessed in the sagittal plane. There is substantial heterogeneity in glenohumeral laxity values that is mostly related to the arthrometer used and the …

Rotator cuff calcific tendinopathy (RCCT) is a very common condition, characterized by calcium deposition over fibrocartilaginous metaplasia of tenocytes, mainly occurring in the supraspinatus tendon. RCCT has a typical imaging presentation: in most cases, calcific deposits appear as a dense opacity around the humeral head on conventional radiography, as hyperechoic foci with or without acoustic shadow at ultrasound and as a signal void at magnetic resonance imaging. However, radiologists have to keep in mind the possible unusual presentations of RCCT and the key imaging features to correctly …

Instead, a point-of-care ultrasound was performed, demonstrating posterior displacement of the humeral head relative to the glenoid rim, confirming the presence of a posterior shoulder dislocation. The patient received procedural sedation, and the shoulder was reduced with real-time ultrasound visualization. The patient tolerated the procedure well, and h …

Transosseous repair of a supraspinatus tendon (SSP) defect (Patte size II) can be difficult if the tendon is retracted and the muscle atrophied. In this situation alternatives are margin convergence techniques, local tendon transfers or distant tendon transfers in massive tears. The object of this study was to compare two local tendon transfers in terms of the feasibility of the shift, the area covered by the shift and the force needed to accomplish the shift. Thirteen fresh-frozen cadaver shoulders were used. First a supraspinatus defect extending to the apex of the humeral head (Patte size I…

Successful treatment of acute fractures of the proximal humerus with prosthetic replacement is a therapeutic challenge to the orthopaedic surgeon, and requires proper elevation of the patient, proper surgical technique, and meticulous rehabilitation. Fractures that require prosthetic replacement as the definitive treatment include 4-part fractures and fracture dislocations, head-split fractures with > 40% articular surface involvement, and selected 3-part fractures. The trauma series of radiographs (including true anteroposterior and lateral views in the scapular plane, and axillary view) is e

BACKGROUND: The bioabsorbable suture anchor is probably one of the most commonly used tools in arthroscopic shoulder operations. However, there is controversy about whether the bioabsorbable anchor is replaced by bone. The object of this study is to evaluate bone ingrowth into the micropore bioabsorbable suture anchor and the differences in the biomechanical properties of a micropore anchor and a nonpore anchor.
MATERIALS AND METHODS: A total of 16 microsized holes (diameter, 250 ± 50 μm; depth, 0.2 mm) were made on the bioabsorbable anchors with a microdrill. Twelve adult New Zealand White r

OBJECTIVE: To characterize bone morphogenetic protein (BMP) signaling in cuff tear arthropathy (CTA) using a modified rat model (mCTA) and determine whether inhibition of DMH1-sensitive BMP type I receptor signaling attenuates CTA pathology.
DESIGN: The rat mCTA model was generated by resection of the supraspinatus and infraspinatus tendons, the superior joint capsule, and the long head of the biceps tendon. Single-cell RNA sequencing (scRNA-seq) was performed to characterize BMP ligand expression in the humeral head. BMP-2, BMP-7, and phosphorylated Smad1/5 (p-Smad1/5) were evaluated by immu…

INTRODUCTION: Shoulder osteoarthritis is an inflammatory and degenerative condition affecting the joint between the humeral head and the glenoid. It is estimated to affect up to 20% of patients aged 60 or older. In our setting, the use of hyaluronic acid is becoming increasingly employed for the treatment of shoulder osteoarthritis. Therefore, this consensus aims to review the current literature to determine whether treatment with hyaluronic acid infiltration is more effective than corticosteroid infiltration in terms of pain outcome for the treatment of shoulder osteoarthritis in adults.
