Changing perspective on gastrointestinal complications in patients undergoing cardiac surgery.

There has been a documented shift towards increasing age and severity of illness in the patient population undergoing cardiac surgery. To determine if there was a coincident change in frequency, gastrointestinal (GI) complications were prospectively recorded in a consecutive series of 5,438 patients undergoing cardiac surgery from 1983 to 1991. There were 73 complications in 69 patients (incidence = 1.4%) defined as any GI condition that required transfer to an acute care unit, surgical intervention, blood transfusion, or treatment that prolonged the hospital course. Fourteen patients died, a

[A case report of nutritional therapy guided by metabolic cart monitoring in a patient with extracorporeal membrane oxygenation].

Patients supported by extracorporeal membrane oxygenation (ECMO) are often in a state of severe hypermetabolism and hypercatabolism, making nutritional support difficult, and there are significant deviations in estimating energy consumption by traditional formulas. On May 1, 2025, a 42-year-old male patient with acute extensive anterior myocardial infarction was admitted to the First Dongguan Affiliated Hospital of Guangdong Medical University. After admission, the patient underwent coronary stent implantation, and subsequently developed severe complications such as cardiac failure, cardiogeni

Assessing the validity of Society of Thoracic Surgeons (STS) score in predicting stroke risk among patients undergoing cardiothoracic surgery at a tertiary hospital in Pakistan: a retrospective cohort study.

BACKGROUND: Postoperative stroke is one of the most serious consequences of cardiac surgery. Morbidity risk assessment is critical for preoperative risk assessments and resource allocation. In this article, we aim to investigate the predictive value of Society of Thoracic Surgeons (STS) score's effectiveness in stroke risk in cardiothoracic surgery patients in our population. METHODS: This retrospective cohort study was conducted at Aga Khan University Hospital (AKUH) using a consecutive sampling technique. The study included all eligible patients aged 18 years or older who underwent cardiac

Single venous cannulation for bi-atrial venoarterial extracorporeal membrane oxygenation in bridge to transplantation: a two-in-one approach.

Venoarterial extracorporeal membrane oxygenation (VA-ECMO) often leads to left ventricular (LV) overload, particularly in patients with biventricular cardiogenic shock. This increased afterload can hinder cardiac muscle recovery and cause pulmonary edema, thereby impairing systemic oxygenation. Although devices such as the Impella and intra-aortic balloon pump (IABP) are used to manage this issue, they require complex procedures and pose risks for bleeding and vascular complications. A transseptal approach for left atrial decompression using a single multistage femoral venous cannula provides

Nationwide one-year outcomes in acute myocardial infarction-related cardiogenic shock with a focus on broad-anterior myocardial infarction: Insights from a Japanese registry.

BACKGROUND: The one-year prognosis for patients with acute myocardial infarction complicated by cardiogenic shock (AMI-CS) undergoing percutaneous coronary intervention (PCI) has not been sufficiently characterized, particularly in nationwide registry analyses. Moreover, data comparing outcomes by infarct location, such as broad-anterior myocardial infarction (BA-MI) vs. non-BA-MI, remain scarce. METHODS AND RESULTS: We analyzed 2489 AMI-CS patients from the J-PCI OUTCOME registry, classifying them into BA-MI and non-BA-MI groups. BA-MI was defined as a culprit lesion in the left main trunk o

Early outcomes in heart transplantation using donation after circulatory death donors in patients bridged with durable left ventricular assist devices.

OBJECTIVE: Donation after circulatory death heart transplantation potentially increases donor allografts, especially for patients with lower listing status. We assessed the outcomes of donation after circulatory death heart transplantation in patients bridged with durable left ventricular assist devices. METHODS: The United Network for Organ Sharing database was queried for adult heart transplants using donation after circulatory death donors from 2019 to 2022. Patients were stratified between those with durable left ventricular assist devices and those with intra-aortic balloon pump, inotrop

Features and outcomes of female and male patients requiring postcardiotomy extracorporeal life support.

OBJECTIVES: Although cardiogenic shock requiring extracorporeal life support after cardiac surgery is associated with high mortality, the impact of sex on outcomes of postcardiotomy extracorporeal life support remains unclear with conflicting results in the literature. We compare patient characteristics, in-hospital outcomes, and overall survival between females and males requiring postcardiotomy extracorporeal life support. METHODS: This retrospective, multicenter (34 centers), observational study included adults requiring postcardiotomy extracorporeal life support between 2000 and 2020. Pre

Continuous near-infrared reflectance spectroscopy monitoring to guide distal perfusion can minimize limb ischemia surgery for patients requiring femoral venoarterial extracorporeal life support.

BACKGROUND: Patients requiring femoral venoarterial (VA) extracorporeal life support (ECLS) are at risk of distal lower limb hypoperfusion and ischemia of the cannulated leg. In the present study, we evaluated the effect of using continuous noninvasive lower limb oximetry with near-infrared reflectance spectroscopy (NIRS) to detect tissue hypoxia and guide distal perfusion catheter (DPC) placement on the rates of leg ischemia requiring surgical intervention. METHODS: We performed a retrospective analysis of patients who had undergone femoral VA-ECLS at our institution from 2010 to 2014 (pre-N

GPIIb/IIIa Receptor Antagonism Using Small Molecules Provides no Additive Long-Term Protection after Percutaneous Coronary Intervention as Compared to Clopidogrel Plus Aspirin.

BACKGROUND: There is some controversy as to whether tirofiban or eptifibatide, two small anti-aggregating drugs (AAD), may reduce the incidence of composite ischemic events within one year in patients undergoing percutaneous coronary intervention (PCI) in the real clinical world. METHODS: We compared consecutive patients on oral double AAD (with clopidogrel and aspirin) who underwent PCI (n=207) and patients who were on single AAD and received a second AAD, just prior to PCI, and either high-dose tirofiban or double-bolus eptifibatide (double AAD plus small molecules group, n=666). Clinical review: practical recommendations on the management of perioperative heart failure in cardiac surgery.

Acute cardiovascular dysfunction occurs perioperatively in more than 20% of cardiosurgical patients, yet current acute heart failure (HF) classification is not applicable to this period. Indicators of major perioperative risk include unstable coronary syndromes, decompensated HF, significant arrhythmias and valvular disease. Clinical risk factors include history of heart disease, compensated HF, cerebrovascular disease, presence of diabetes mellitus, renal insufficiency and high-risk surgery. EuroSCORE reliably predicts perioperative cardiovascular alteration in patients aged less than 80 year

Temporary Mechanical Circulatory Support in Acute Myocardial Infarction-Related Cardiogenic Shock: Lessons from Contemporary Randomized Trials.

Background: Acute myocardial infarction-related cardiogenic shock (AMI-CS) remains associated with high mortality despite advances in revascularization, critical care, and temporary mechanical circulatory support (MCS). Recent randomized trials have challenged the concept of routine device-based therapy and highlighted the importance of matching MCS strategy to shock phenotype, timing, and patient risk profile. Methods: We reviewed contemporary randomized clinical trials and prespecified or post hoc analyses evaluating temporary MCS strategies in adult patients with AMI-CS. PubMed was searched

Simplified Parsonnet risk scale identifies limits to early patient discharge.

BACKGROUND: Fast-track recovery after coronary artery bypass surgery has influenced patient care positively. Predicting patients who fall off track and require prolonged (> or =7 days) hospitalization remains uncertain. The Parsonnet risk assessment score is effective in predicting length of stay, but is limited by inaccurate subdivision of risk categories. We simplified the Parsonnet risk scale to better identify patients eligible for fast-track recovery. METHOD: The cases of 604 consecutive patients who underwent isolated coronary artery bypass grafting (CABG) using cardiopulmonary bypass (
