The 3-in-1 procedure for recurrent dislocation of the patella in skeletally immature children and adolescents.

BACKGROUND: Recurrent patellar dislocations are common injuries in children and adolescents. The subjective and functional results of soft tissue surgical management in a population that was skeletally immature at the time of surgery have not been reported. HYPOTHESIS: The 3-in-1 procedure is an effective treatment for recurrent patellar dislocation in skeletally immature patients. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Twenty-five skeletally immature patients (age at operation, 13.5 +/- 3.8 years) who were practicing sports and suffering from recurrent unilateral patella

Rehabilitation and nonoperative treatment of patellar instability.

Patellofemoral instability can be a difficult condition for clinicians to manage. Differentiation needs to be made as to whether the problem is an acute injury where a traumatic incident has usually precipitated the dislocation or whether the problem is a recurrent instability where the patellofemoral joint is unstable during everyday activities. This review defines instability, discusses the factors affecting instability, and provides assessment procedures and nonoperative intervention strategies for the clinician.

Osteochondral injury to the mid-lateral weight-bearing portion of the lateral femoral condyle associated with patella dislocation.

We report on a series of 7 patients who presented with patella dislocation and were found to have osteochondral fracture (OCF) of the weight-bearing portion of the lateral femoral condyle either through preoperative studies or by identification of the lesion during arthroscopy. Treatments included open reduction and internal fixation, debridement, and microfracture of the lateral femoral condyle. Although not as common as osteochondral injuries to the medial facet of the patella and the anterior lateral portion of the lateral femoral condyle, osteochondral injury to the weight-bearing portion

A new technique for reconstructing the medial patellofemoral ligament.

Many surgical techniques exist for treating patella instability. Over recent years, attention has focussed on reconstruction of the medial patellofemoral ligament (MPFL). We report a procedure involving a lateral release, distal realignment of the tibial tubercle and anatomic reconstruction of the MPFL using a graft composed of a redundant strip of medial retinacular tissue. Nineteen patients (twenty-one knees) with chronic patellofemoral instability who had failed conservative treatment underwent this procedure and were assessed clinically and radiologically at an average of 23 months post-op

Evaluation of patients presenting with knee pain: Part II. Differential diagnosis.

Knee pain is a common presenting complaint with many possible causes. An awareness of certain patterns can help the family physician identify the underlying cause more efficiently. Teenage girls and young women are more likely to have patellar tracking problems such as patellar subluxation and patellofemoral pain syndrome, whereas teenage boys and young men are more likely to have knee extensor mechanism problems such as tibial apophysitis (Osgood-Schlatter lesion) and patellar tendonitis. Referred pain resulting from hip joint pathology, such as slipped capital femoral epiphysis, also may cau

Kinematic MRI of the knee using a specially designed positioning device.

To evaluate the benefits of a specially designed positioning device, kinematic MR studies of the patellofemoral joint and the cruciate ligaments were performed. Handling of the device and positioning of the patient's leg were easy and took only a few minutes. The positioning device was helpful for patients to guide their joint motion and to obtain sufficient image quality for kinematic MR studies of both cruciate ligaments and the patellar tracking.

Radiographic analysis of patellar tilt.

We describe the radiographic measurement of the angle of tilt of the patella and relate it to malalignment of the extensor mechanism. The tilt angle is defined as the angle subtended by a line joining the medial and lateral edges of the patella and the horizontal. The radiograph (Merchant type) is taken with the foot pointing up, the lower edge of the film parallel to the ground, and the knee at 30 degrees flexion. The mean tilt angle of a group of patients with signs and symptoms suggesting patellofemoral malalignment was 12 degrees (+/- 6 degrees); in a similar group of control subjects it w

Lateral patellar instability: treatment with a combined open-arthroscopic approach.

Lateral patellar maltracking, subluxation, and dislocation could arguably be grouped together as varying degrees of lateral patellar instability. Besides common anatomical etiological factors, such as genu valgum, patella alta, tibial rotation, and others, the author has found that many of these patients exhibit patellar hypermobility, a seldom mentioned physical finding. Lateral retinacular release fails to address the hypermobility, whereas traditional, more extensive open operations often carry significant postoperative morbidity and disability. This article suggests a surgical approach tha

Rehabilitation exercises for common sports injuries.

Residual weakness after joint injury is a risk factor for recurrent injury. A rehabilitation program helps patients recover strength and helps prevent further injuries. Orthopedic injuries account for 10 percent of visits to family physicians, yet many primary care physicians do not routinely prescribe rehabilitation exercises for injured patients. Illustrations of exercises for the hip, knee, ankle and shoulder are included as a reference for family physicians to use when prescribing rehabilitation exercises.

[An experimental study on early changes of articular cartilages in subluxated patella of rabbits].

The pathogenesis and clinical progress of chondromalacia patellae have not been well studied in experimental models. We, therefore, studied the patellar subluxation produced experimentally in rabbit knees. In this model, sequential changes of the patellar cartilage were followed by simple X-rays, CT, macroscopic, histologic and scanning electron microscopic examinations up to 32 weeks. Pathological changes extending from horizontal fissures in the middle to the deep layers of the patellar cartilage and thinning of the subchondral bone were found at about six weeks after operation. The early ch

Congenital snapping knee. Habitual anterior subluxation of the tibia in extension.

We describe a rare form of congenital snapping knee. In six knees in four children, the tibia subluxed anteriorly on the femur when the knee was extended and reduced spontaneously on flexion. The abnormal movements were seen and felt as sudden snaps or clunks at about 30 degrees of flexion. All six knees showed similar dysplastic features, although the patients had different clinical syndromes. The mechanism of the subluxation and its management are discussed.

The diagnostic potential of vibration arthrography.

The detection and interpretation of vibration emission from the locomotor system is a sensitive, noninvasive method for the objective study of human joints. The projects reviewed here aimed to identify and categorize vibration signals from various joints, chiefly the neonatal hip and the adult knee. Using a vibration detection and computerized analysis system, various studies have suggested that vibration arthrography will be a potentially valuable diagnostic service for the orthopedic surgeon of the 1990s.

The anterior drawer sign: what is it?

1. The anterior drawer is a test of anterior cruciate ligament integrity. 2. The anterior cruciate ligament is composed of two parts: the anteromedial band and the posterolater bulk. 3. The anteromedial band is the primary check against anterior drawer. 4. Other structures may contribute to the resistance of anterior drawer only if the anteromedial band has been injuried. 5. The posterior cruciate ligament is a check against posterior drawer. 6. In long term follow-up, the presence of anterior drawer is associated with poor overall results.

Early morbidity and mortality after one-stage bilateral shoulder arthroplasty.

PURPOSE: One-stage bilateral shoulder arthroplasty has the advantage of requiring a single hospital stay and a single anaesthesia. The topic has been little reported, unlike one stage bilateral hip and knee arthroplasty, which have demonstrated their interest. The aim of the present study was to determine peri- and early post-operative morbidity and mortality after this procedure. The study hypothesis was that peri- and early post-operative morbidity and mortality in one stage bilateral shoulder arthroplasty is low in selected patients and that satisfaction is high. METHODS: A single-centre r

A feasibility study for a pragmatic randomised controlled trial comparing cast immobilisation versus no immobilisation for patients following first-time patellar dislocation.

BACKGROUND: The purpose of this feasibility study was to begin to determine the efficacy of cylinder cast immobilisation compared to no form of immobilisation for patients following first-time patellar dislocation (FTPD). MATERIALS AND METHODS: Participants were patients who attended a National Health Service Accident and Emergency department following a FTPD. Patients were randomised to receive immobilisation for four weeks in a cylinder cast followed by rehabilitation, versus no immobilisation and early rehabilitation. All patients were followed-up over a three month period, assessed using
