Corneal perforation by an astigmatic keratotomy performed with an optical coherence tomography-guided femtosecond laser.

UNLABELLED: We present a case of corneal perforation secondary to an intrastromal astigmatic keratotomy performed with an optical coherence tomography-guided femtosecond laser. The keratotomy was concomitant with cataract surgery and resulted in a flat anterior chamber prior to the start of lens extraction. Interrupted nylon sutures were placed to seal the keratotomy prior to phacoemulsification. Escape of cavitation bubbles into the anterior chamber or the liquid interface can alert the surgeon to the possibility of unintended perforation of the endothelium or the epithelium, respectively. Is topography-guided ablation profile centered on the corneal vertex better than wavefront-guided ablation profile centered on the entrance pupil?

PURPOSE: To illustrate the hypothesis that corneal vertex centration is superior to entrance pupil centration when guiding an ablation by wavefront. METHODS: In one case example of therapeutic retreatment for treatment zone decentration after primary radial keratotomy (RK) centered on the entrance pupil (line of sight), both a whole-eye wavefront-guided ablation profile (WASCA data) and a topography-guided ablation profile (Atlas data) were generated using the CRS-Master (Carl Zeiss Meditec) and compared. The patient had a large vertical angle kappa. Corneal topography demonstrated that the z

Multizone/multipass photorefractive keratectomy: six month results.

A multizone/multipass photorefractive keratectomy (MP-PRK) technique was used to treat 315 eyes with the 193 nm VISX 20/20 excimer laser. Algorithms were developed to treat the full range of myopia, including 170 low myopic eyes (-1.0 to -6.0 diopters [D]), 105 moderately myopic eyes (-6.0 to -10.0 D), and 40 highly myopic eyes (-10.0 to -27.0 D). Preoperative mean myopia was -6.69 D; astigmatism equal to or greater than -0.5 D (mean -1.18 D) was present in 193 eyes. At one month postoperatively, most eyes were slightly hyperopic (mean +0.8 D); this regressed to a mean of -0.16 D. At six month

Long-Term Evaluation of Ocular Surface and Meibomian Gland Function after Laser-Assisted in situ Keratomileusis Surgery.

INTRODUCTION: Laser-assisted in situ keratomileusis (LASIK) is a common refractive surgery. But it may lead to temporary dry eye due to reversible damage to the corneal subbasal nerve plexus. However, evidence on chronic ocular surface changes are unclear. This study reports the ocular surface changes, partial blinking rates, and meibomian gland statuses in eyes that underwent LASIK for at least 48 months. METHODS: Cross-sectional, matched case-control comparison study, including 48 post-LASIK patients and 48 sex-, age-, smoking-, and axial length-matched healthy controls, recruited from a co
