Does the timing of treatment affect the clinical and psychological outcomes of Twin-block therapy?

DESIGN: This study is a multicenter, two-arm parallel randomized control trial carried out across 3 hospital orthodontic units. A total of 75 patients participated in the study, with 41 randomly allocated in an Immediate Treatment Group (ITG) and 34 randomly allocated into an 18-month delayed Later Treatment Group (LTG). The patients were aware as to which group they were being allocated, as were the clinicians. Both patient groups received the same twin block appliance for use during the study. The appliance was to be worn full time, including eating, but to be removed while participating in

Influence of conventional and skeletal anchorage system supported fixed functional appliance on maxillo-mandibular complex and temporomandibular joint: A preliminary comparative cone beam computed tomography study.

AIM: Fixed functional appliance (FFA) used in the treatment of Class II malocclusion, lead to mandibular anterior teeth proclination, thus limiting the skeletal effects of the appliance. To counter this side effect, FFA is anchored in the lower anterior region of the mandible using the skeletal anchorage system. This pilot study was done to evaluate treatment and one-year post-treatment maxillo-mandibular, and temporomandibular joint (TMJ) effects of conventional fixed functional appliance (FFA) and skeletal anchorage system supported fixed functional appliance (SAS-FFA) using cone beam comput

Orthodontic treatment for prominent upper front teeth (Class II malocclusion) in children and adolescents.

BACKGROUND: Prominent upper front teeth are a common problem affecting about a quarter of 12-year-old children in the UK. The condition develops when permanent teeth erupt. These teeth are more likely to be injured and their appearance can cause significant distress. Children are often referred to an orthodontist for treatment with dental braces to reduce the prominence of their teeth. If a child is referred at a young age, the orthodontist is faced with the dilemma of whether to treat the patient early or to wait and provide treatment in adolescence. OBJECTIVES: To assess the effects of orth

Apparent life-threatening events could be a wake-up call for sleep disordered breathing.

INTRODUCTION: Polysomnographic recordings of children with an apparent life-threatening event (ALTE) have often displayed signs of partial or complete obstruction during sleep. Various studies have focused on facial dysmorphia in infants with ALTE and tried to establish a correlation between ALTE and obstructive sleep apnoea. Our study evaluates the phenotypic characteristics and the presence of sleep disorders in pre-school children who had at least one ALTE in the first year of life. MATERIALS AND METHODS: We analyzed a group of pre-school children (mean age 5.21 ± 0.90 years) who were refe

Orthodontic Consideration in Patients with Beta-Thalassemia Major: Case Report and Literature Review.

OBJECTIVE: Beta Thalassemia (βT) patients present a unique facial appearance and specific craniofacial, jaw and dental patterns. Although this anomaly often requires orthodontic management, βT patients have received scant attention in the orthodontic and dental literature over the past 50 years. The aim of this article is to review the characteristic craniofacial and dental manifestation pattern of βT patients and to emphasize their preferred orthodontic management protocol by presenting a βT orthodontic treated patient. CASE REPORT: A 10 year old patient presented with a complaint of severe

Follow-up problems with fixed appliances in pediatric dentistry.

Fixed appliances are commonly used in managing early orthodontic problems. Despite their widespread use, they have the potential to impinge on the soft tissues, interfere with the eruption of adjacent teeth and become dislodged or broken. These two case reports present the poor outcomes of fixed appliance treatments if the patient fails to attend follow-up appointments. A successful outcome of treatment with fixed appliances depends upon proper patient selection and the communication skills of the dentist to help patients/parents understand the importance of regular checkups.

Unusual orthodontic approach to a maxillary canine-premolar transposition and a missing lateral incisor with long-term follow-up.

In this case report, we present an unusual approach that was used to treat a 14-year-old patient with a bilateral canine-premolar transposition and a missing maxillary lateral incisor. The orthodontic treatment involved maintaining the transposition and creating a space for lateral incisor replacement. Toward the end of the treatment, accentuated buccal root torque was performed, and lateral group function was established to improve the treatment outcome. Finally, an ideal esthetic and functional occlusion was achieved. The 8-year posttreatment follow-up records show the stability of this trea

Treatment of class II division 1 malocclusion with myofunctional trainer system in early mixed dentition period.

Dentofacial growth interferences, caused by abnormal lip and tongue function in the mixed dentition period are a common clinical condition. The main purpose of the treatment in such cases is to eliminate oral dysfunction, establish muscular balance and correct or diminish maxillary incisor protrusion. The position of the teeth is determined by the lip and tongue pressure. Lip function may have been the cause of forcing teeth in to malposition they occupy. The case report is presented where the myofunctional trainer T4K is used during growth phase.

Maximum anchorage in orthodontics with the palatal implant: maximum anchorage in orthodontics with the palatal implant.

Maximum anchorage in the maxilla can only be achieved by using headgear worn full-time or by using an implant. As full time wear requires considerable patient cooperation, we describe a technique for use of the Straumann Ortho Implant, exemplified by a case presentation. The cephalometric analysis at the end of therapy confirms that the upper molars did not slip forward during treatment (no anchorage loss).

The impact of mandibular advancement on articulation, resonance and voice characteristics in Flemish speaking adults: a pilot study.

Bilateral saggital split osteotomy (BSSO) of the mandible is a frequently performed mandibular orthognathic procedure, used to resolve mandibular disharmonies. Literature review showed contradictory findings regarding the effect of the orthognathic surgery on speech characteristics. The purpose of the present study was to determine a detailed analysis of the articulation, resonance and voice characteristics after BSSO with mandibular advancement for the treatment of Class II malocclusions using objective and subjective assessment techniques (perceptual evaluations, Dysphonia Severity Index, na

Periodontal disease associated with interdental osteotomies after orthognathic surgery.

PURPOSE: One of the main reasons for orthodontics and orthognathic surgery is the prevention of dental loss caused by periodontal disease. Until now there have been no published data describing the periodontal situation near interdental osteotomies after orthognatic surgery. The purpose of this study was to evaluate this situation. PATIENTS AND METHODS: Thirty patients with Class II malocclusions were studied 4 to 10 years after segmental orthognathic surgery, analyzing the periodontal condition near osteotomies with the aid of periapical and panoramic radiographs. RESULTS: Fifty-one patholo

Forceps extraction of teeth with severe internal root resorption.

Many treatment plans require a dental practitioner to maintain the entire labial cortical plate of bone when removing an anterior maxillary tooth. A tooth with an undermined root secondary to an endodontic perforation or internal (Idiopathic) resorption can present a surgical challenge to the general practitioner. This article describes a new technique for extracting a severely undermined anterior maxillary tooth while maintaining the entire labial cortex of bone.
