Increased median nerve latency at the carpal tunnel of patients with "trigger finger": comparison of 62 patients and 13 controls.

An association between symptomatic compression neuropathy of the median nerve at the carpal tunnel and "trigger finger" has been reported in endocrine and metabolic disorders. We assessed the incidence of increased median nerve latency in subjects with "trigger finger". 62 consecutive patients with "trigger finger" and no signs or symptoms of median nerve compression underwent nerve conduction studies of the median nerve. 13 healthy adults served as controls. 39/62 patients had increased distal motor latency in the median nerve. Only 1 of 13 subjects in the control group had a borderline value

Carpal tunnel syndrome in patients on long-term haemodialysis.

The purpose of this study was to describe the pathophysiology of carpal tunnel syndrome (CTS) in patients on long-term haemodialysis. We examined 110 patients, who had been having haemodialysis for chronic renal failure and had CTS, to clarify the clinical features and electrophysiological changes in peripheral nerves. There was a significant correlation between the incidence of CTS and the duration of haemodialysis. Compared with idiopathic CTS, CTS caused by long-term haemodialysis had relatively limited postoperative improvement. Symptoms recurred postoperatively in 11 patients (19%) of tho

Comparative diagnostic sensitivities of orthodromic or antidromic sensory inching test in mild carpal tunnel syndrome.

OBJECTIVES: To compare the reliability, sensitivity, and specificity of the "inching test" (IT) or "centimetric test," performed orthodromically (OIT) and antidromically (AIT). METHODS: Incremental palmar study of the sensory fibers of the median nerve was evaluated over 10cm across the wrist in 20 patients with mild carpal tunnel syndrome (CTS) and in 20 controls. Mild CTS was defined as clinical features of CTS with normal electrophysiologic findings by standard methods. The CTS patients were preselected with abnormal orthodromic median-ulnar latency difference of the fourth digit (mean .66

Medical brachial cutaneous nerve conduction study.

There is no nerve conduction study for the medial brachial cutaneous nerve in the literature. This study was done in the military hospital # 501 from feb 1st 1997 to june 1st 1999. Three hundred healthy adult (220 male) with a mean age of 40 +/- 13 (range, 14-66) years were studied. At first the sensory nerve action potential of median and ulnar and medial antebrachial cataneous nerves were recorded to show these roots C8-T1 are intact. Then the medial brachial cutaneous nerve was stimulated at the medial border of arm on the line that connects axilla to medial epicondyle (parallel with mid ax

Ulnar neuropathy at the elbow due to unusual sleep position.

Abnormal strain of the ulnar nerve over the sulcus due to an unusual sleep position is a rare cause of ulnar neuropathy at the elbow. A 57-year-old patient with Mandelung's deformity developed progressive weakness in the flexion of fingers 4 and 5 and in finger straddling on the left side. Additionally, there was slight wasting of the left hypothenar and the left interossei muscles. Motor and sensory nerve conduction studies of the left ulnar nerve showed delayed conduction velocities over the left ulnar sulcus. He preferred to sleep in a left lateral position with his head lying on a headrest

Quantitative sensory testing of thermal and vibratory perception in familial dysautonomia.

Familial dysautonomia (FD) is an inherited disorder that is known to affect both sensory and autonomic functions as a result of incomplete neuronal development and progressive loss but the degree to which patients are affected differs greatly. To determine if quantitative vibration and thermal testing refined the assessment of severity, 23 familial dysautonomia patients were evaluated by clinical examination, measurements of median, peroneal and sural nerve conduction velocities (NCV), and assessment of vibration thresholds at two body sites and of warm and cold perception thresholds at 6 body

Fracture of the distal part of the radius associated with severed ulnar nerve.

We report a case of a severed ulnar nerve after fracture of the distal part of the radius. The most likely hypothesis is stretching of the ulnar nerve fixed by Guyon's canal and severed on the sharp edge of the proximal radius. Although very rare, this lesion must be investigated particularly in cases with marked displacement, especially ulnar and/or volar.

Suppression of diaphragmatic activity during spontaneous ponto-geniculo-occipital waves in cat.

It has been reported that spontaneous ponto-geniculo-occipital (PGO) waves, which occur during REM sleep in the cat, are associated with a brief inhibition of diaphragmatic activity (Orem, 1980). This report was preliminary and not supported by a detailed analysis. We report here analysis of the relationship between PGO waves and diaphragmatic activity based on 3073 PGO waves recorded simultaneously with diaphragmatic activity. The results show that there is indeed an inhibition of diaphragmatic activity during PGO waves. This inhibition has an amplitude up to 20% of background, and a duration

Recovery of nerve conduction following microvascular decompression for trigeminal neuralgia.

OBJECTIVE: To assess the function of trigeminal nerve before and after microvascular decompression for trigeminal neuralgia. BACKGROUND: To date there is no direct evidence that microvascular decompression of the trigeminal root restores normal conduction in the nerve. METHODS: The authors examined 10 patients with trigeminal neuralgia in whom preoperative MRI and MR angiography demonstrated neurovascular contact. During microvascular decompression, the trigeminal nerve was monitored by recording early scalp trigeminal evoked potentials immediately before, during, and after decompression. Di

Chronic inflammatory polyradiculoneuropathy associated with alopecia universalis.

We report a patient with concurrent chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) and alopecia universalis. A dramatic improvement in strength accompanied the administration of prednisone, although no regrowth of hair was noted. CIDP and alopecia universalis are two relatively rare autoimmune disorders. The simultaneous occurrence of these disorders suggests a common pathogenetic mechanism, including the possibility of shared antigens between peripheral nerve myelin and the hair follicle.

[Neurophysiological assessment of children with obstetric brachial plexus palsy].

INTRODUCTION: The objectives of the neurophysiological evaluation of infants with brachial plexus palsy are to determine the time of occurrence of the lesion, to locate the lesion and to determine its course. METHODS AND CONCLUSIONS: These objectives are achieved by studying affected upper extremity muscles by needle electromiography (EMG) and affected nerves by motor and sensory conduction studies. EMG is performed in the first week of life in those patients with brachial plexus palsy of unknown etiology to determine the age of the lesion for medico-legal reasons. EMG is performed before sur

Reversal by methylene blue of tetanic fade induced in cats by nitric oxide.

Previous data from our laboratory have indicated that nitric oxide (NO) acting at the presynaptic level increases the amplitude of muscular contraction (AMC) of the phrenic-diaphragm preparations isolated from indirectly stimulated rats, but, by acting at the postsynaptic level, it reduces the AMC when the preparations are directly stimulated. In the present study we investigated the effects induced by NO when tetanic frequencies of stimulation were applied to in vivo preparations (sciatic nerve-anterior tibial muscle of the cat). Intra-arterial injection of NO (0.75-1.5 mg/kg) induced a dose-

Strategies for analyzing nerve conduction data: superiority of a summary index over single tests.

We compared three strategies for diagnosing carpal tunnel syndrome: using a single test; requiring one, two, or three of three tests to be abnormal; or utilizing a single summary variable incorporating data from three tests. Sixty-five hands of subjects without clinical carpal tunnel syndrome (CTS) were compared with 66 hands with clinical CTS. Three latency differences were measured: median-ulnar (8 cm) midpalmar orthodromic (palmdiff); median-ulnar ring finger (14 cm) antidromic (ringdiff); and median-radial thumb (10 cm) antidromic (thumbdiff). The combined sensory index (CSI) was the sum o

Chronic hepatitis B and neurogenic muscle disease: case report.

A 17 year-old boy with chronic hepatitis B who developed left-sided muscle wasting is reported. When other possible known diseases as the cause of the neurogenic muscle disease were excluded it was hypothesised that there was a relation between the chronic hepatitis B infection and the neurogenic muscle disease. An immunopathogenesis could be explained by the presence of HBsAg in the cerebral spinal fluid.

Familial juvenile focal amyotrophy of the upper extremity (Hirayama disease). Superoxide dismutase 1 genotype and activity.

BACKGROUND: Juvenile focal amyotrophy of the arm is an unusual focal motor neuron disease that is rarely familial. Its pathogenesis is unknown. We recently described a family with amyotrophic lateral sclerosis associated with a mutation in the superoxide dismutase 1 (SOD1) gene substituting an aspartate for an alanine (D9OA). One of the carriers of this mutation had focal and nonprogressive amyotrophy of the arm, suggesting that focal amyotrophy might be associated with SOD1 mutations. OBJECTIVES: To describe the phenotype of 2 brothers with juvenile focal amyotrophy of the upper extremity an

Exercise training improves functional recovery and motor nerve conduction velocity after sciatic nerve crush lesion in the rat.

OBJECTIVE: To observe the effects of exercise training on recuperation of sensorimotor function in the early phase of regeneration, and to monitor the long-term effects of exercise on electrophysiological aspects of the regenerating nerve. DESIGN: After sciatic nerve crush in 20 male Wistar rats, one random selected group was subjected to 24 days of exercise training, whereas the other group served as sedentary controls. INTERVENTIONS: Exercise training was induced for 24 days, starting the first postoperation day, by placing bottles of water at such a height that the exercising rats had to

Functional consequences of isolated nerve stretch: experimental long-term static loading.

The maximum longitudinal tension that can be applied to the stumps of a divided peripheral nerve during surgical repair is not precisely defined. Uncertainty about the threshold of unacceptable nerve tension may be due to the lack of studies on isolated and reproducible stretch, i.e., on quantified elongation without concomitant compression, crush, or transection. The authors devised a simple method for producing isolated, long-term static loading of the nerve in vivo without concomitant compression, crush, or transection. Tension was applied to intact and crushed sciatic nerves in the rat, st

Clinical manifestations of peripheral nervous system involvement in human chronic Chagas disease.

We conducted a clinical and electromyographical study in patients with Chagas' disease in the indeterminate or chronic stages of the illness. Altogether 841 patients were examined. Only 511 were admitted within the protocol; the remainder patients were rejected because they showed other causes able to damage the nervous system. Fifty two (10.17%) out of the 511 patients showed signs and symptoms of peripheral nervous system involvement in the form of sensory impairment and diminished tendon jerks suggesting the presence of neuropathy. Forty five of them were submitted to a conventional electro

Motor nerve conduction velocities calculated by F tacheodispersion in patients with anterior horn diseases.

F tacheodispersion, a new neurophysiological technique based on F waves, was used to determine the motor nerve conduction velocities in 16 patients with motor neuron disease and 19 patients with sequelae following paralytic poliomyelitis. The findings were compared to two age-matched groups, each consisting of 20 healthy volunteers. In each subject the F wave maximal, minimal, mean conduction velocities and the difference between maximal and minimal velocities of the ulnar and peroneal nerves were calculated. M response distal latency, amplitude and conduction velocity were also measured. Ther
