Optic nerve involvement in neuro-Behcet's disease.

BACKGROUND: To report the ocular manifestations of neuro-Behcet's disease. CASES: A 34-year-old woman had recurrent meningitis. She developed diplopia, headache, and ataxia, and was diagnosed as having neuro-Behcet's disease. OBSERVATIONS: Imaging revealed two infarct foci in the transitional midbrain and pons. After treatment with prednisolone and colchicine, the diplopia resolved. Two years later, a scotoma developed in the right eye, in which the best-corrected visual acuity was 4/200. Papillitis and a prepapillary vitreous opacity were seen in the right fundus. These findings disappeared…

[Mitochondrial encephalomyopathy of late presentation with progressive ophthalmoplegia, tremor and diffuse leukoencephalopathy].

A 60-year-old woman had developed ptosis, progressive external ophthalmoplegia and action tremor over the last ten years. Physical examination also revealed short stature and retinal pigmentation. Anaerobic forearm exercise test showed increase of basal lactate and rise of lactate/piruvate index. Biceps biopsy displayed numerous ragged red fibers. Respiratory chain studies were consistent with complex I deficiency. Point mutations or deletions in mitochondrial DNA were not found. MR identified a diffuse leukoencephalopathy over both cerebral hemispheres, mesencephalon, pons and cerebellum. The…

Pontine lesions in idiopathic narcolepsy.

Three patients had longstanding (37 to 50 years), highly disabling narcolepsy, poorly controlled by treatment. The clinical histories were typical, consisting of sleepiness, cataplexy, sleep paralysis, hypnagogic hallucinations, disturbed nocturnal sleep, and HLA-DR2 tissue typing. Polygraphic findings confirmed the diagnosis. Neurologic examination, spinal fluid, and evoked potentials were normal. On MRI scanning, all three patients showed overlapping bilateral and symmetric brainstem T2 hyperintensities circumscribed to the ventrolateral aspect of the midrostral pons. The nature of the lesio…

Infiltration of helper/inducer T lymphocytes heralds central nervous system damage in human T-cell leukemia virus infection.

Cellular infiltrates in new and old lesions in two cases of human T-cell leukemia virus associated myelopathy (HAM) were analyzed with anti-CD3 antibody and OPD4 antibody recognizing CD4 + CD45RO + T lymphocytes. A subset of CD4 lymphocytes with helper/inducer function and labeled with OPD4 constitutes up to 65% of CD3 cells in new lesions in the pons and the cervical cord. In contrast, nonhelper cells and macrophages were dominant in long-standing spinal cord lesions of these HAM cases and inflammatory lesions in two cases of Japanese encephalitis. Thus, unlike in viral infections, the centra…

[Central pontine myelinolysis].

One week after relapsing a 44-year-old female alcoholic and drug addict developed a psycho-organic syndrome. Later she had swallowing and speech dysfunctions and became quadriplegic, the result of central pontine myelinolysis. The signs became most marked in the 5th week of illness and then gradually improved, but an independent existence became impossible. There were no electrolyte disorders. Computed tomography demonstrated the typical lesion (a hypodense focus in the pons, 1 cm in diameter) after the 3rd week, findings confirmed by magnetic resonance imaging. The acoustic evoked potentials …

Dysphagia following brain-stem stroke. Clinical correlates and outcome.

We studied 23 individuals (16 men, seven women; mean age 57 years) who had brainstem strokes confirmed by computed tomography of the head or magnetic resonance imaging. Videofluoroscopic modified barium-swallowing examination showed aspiration in 15 of 23 patients. Of the 15 aspirating patients, the majority had bilateral strokes with multiple lesion loci, most often in association with large-vessel disease. All 15 patients had involvement of the pons or medulla. Statistical analyses revealed a significant association between aspiration and pharyngeal residue observed under videofluoroscopy, c…

Stimulation of the wrist acupuncture point PC6 for preventing postoperative nausea and vomiting: a network meta-analysis.

RATIONALE: Postoperative nausea and vomiting (PONV) are common complications following surgery and anaesthesia. Antiemetic drugs are only partially effective in preventing PONV. Stimulating the PC6 acupoint(s) on the wrist offers an alternative approach, but the effectiveness of the various common techniques is unclear. OBJECTIVES: To update and compare the effects and safety of PC6 acupoint stimulation with or without antiemetic drug(s) versus sham or antiemetic drug(s) for preventing postoperative nausea (PON) and postoperative vomiting (POV) in people undergoing surgery, and to identify th

[Moebius syndrome after maternal misoprostol use].

This case report describes a newborn with clinical and radiological features compatible with Moebius syndrome following first-trimester maternal use of misoprostol intended for medical abortion. Despite early vaginal bleeding, the pregnancy continued and the parents opted to proceed. Prenatal ultrasound showed bilateral clubfeet, and postnatal examination revealed cranial nerve dysfunction, hypotonia, and feeding difficulties. An MRI of the cerebrum demonstrated hypoplasia of the pons, thalamic fusion, and absence of the nervus abducens and nervus facialis, findings that support a diagnosis of…

An MSA-P patient presenting with preserved glucose metabolism in the putamen, cerebellar hypometabolism and pronounced loss of presynaptic dopamine transporter in the striatum.

Positron emission tomography (PET) of the brain using [18F]fluorodeoxyglucose (FDG) is becoming increasingly important for the diagnosis and differential diagnosis of atypical parkinsonian syndrome such as multiple system atrophy (MSA), which is characterized by hypometabolism of the putamen, pons, and cerebellum. We report on a patient with clinically established MSA based on a rapidly progressive, poorly levodopa-responsive parkinsonian syndrome, multidomain autonomic failure, and imaging findings where hereditary spastic paraplegia was discussed as a differential diagnosis. PET images revea…

Rapidly growing glioblastoma multiforme at the cervicomedullary junction - a diagnostic and neuroradiological challenge.

Brainstem gliomas in adults are a rare and heterogeneous group of brain tumours, which account for less than 2% of all adult gliomas and usually involve the pons (60-63%). The medulla oblongata and the midbrain are the least common locations of brainstem gliomas (25% and 12-15%, respectively). Survival and overall prognosis depend on the underlying pathology and pathological tumour grade. The authors present a female patient with medulla oblongata rapidly growing brainstem glioma (documented in a series of brain MRI examinations), which resulted in quick progression of neurological symptoms wi…

Physiotherapy Approach to a Bilateral Pontine Infarct With Acute Ischemic Stroke: A Case Report.

Strokes are prevalent, although only some impact the pons, a brainstem region that performs essential functions. Because pontine infarcts are uncommon, there isn't sufficient research to back up efficient therapies that give patients functional advantages. This case report presents a 75-year-old male who was brought to the casualty with a history of falls and developed weakness on the left side of the body, dysarthria, and dysphagia, for which he was referred to the neurophysiotherapy department after performing the necessary investigations. A neurological examination was carried out, and a ph…

Case report: A case of tetanus in a dog: cranial nerve involvement and imaging findings.

An 11 years old male Labrador cross presented with unilateral vestibular signs, ipsilateral facial paresis, moderate obtundation, ptyalism, and paraparesis. MRI of the brain revealed diffuse, multifocal T2/FLAIR hyperintense changes throughout various regions of the brain including the medulla, midbrain, pons, thalamus and right cerebral hemisphere with mild multifocal contrast enhancement. The patient progressed to trismus with generalized increased extensor tone and risus sardonicus. A diagnosis of generalized tetanus was made and the patient was started on antibiotics, skeletal muscle relax…

Life without a brain: Neuroradiological and behavioral evidence of neuroplasticity necessary to sustain brain function in the face of severe hydrocephalus.

A two-year old rat, R222, survived a life-time of extreme hydrocephaly affecting the size and organization of its brain. Much of the cortex was severely thinned and replaced by cerebrospinal fluid, yet R222 had normal motor function, could hear, see, smell, and respond to tactile stimulation. The hippocampus was malformed and compressed into the lower hindbrain together with the hypothalamus midbrain and pons, yet R222 showed normal spatial memory as compared to age-matched controls. BOLD MRI was used to study the reorganization of R222's brain function showing global activation to visual, olf…

Entrapment of the basilar artery within a clivus fracture: Case report and literature review.

A review of the literature revealed that basilar artery (BA) entrapment is a very rare (17 cases published) and severe pathological condition, which often leads to death. We report the case of a 72-year-old man who presented with a longitudinal clivus fracture associated with a basilar artery entrapment. This entrapment was responsible for a basilar artery dissection, which led to an ischemic stroke in the pons. The patient was managed with medical treatment, mainly to avoid a progression towards an ischemic stroke. It consisted of heparin therapy followed by antiplatelet therapy, which finall…

Blood, brain and binocular vision.

A man aged 51 years presented with sudden onset, horizontal, binocular, double vision and right facial weakness. Ocular motility examination demonstrated a right horizontal gaze palsy pattern in keeping with a one-and-a-half syndrome. Since this was associated with a concomitant, ipsilateral, lower motor neuron (LMN) facial (VIIth) cranial nerve palsy, he had acquired an eight-and-a-half syndrome. Diffusion-weighted MRI confirmed a small infarcted area in the pons of the brainstem which correlated with anatomical location of the horizontal gaze centre and VIIth cranial nerve fasciculus. As a r…

Serial brain MRI findings in a rare survivor of rabies encephalitis.

Rabies is a neurotropic viral illness, almost always fatal, that is equally dreaded by healthcare practitioners and patients due to the dism … rical non-enhancing areas of T1 and T2 hyperintensity in the basal ganglia, thalami, mid brain, and pons along with T2 hyperintensity and restricted diffusion in fronto-parietal cortical grey matter and left hippocampus. Subsequent MRI scans at 2 months and 5 months revealed progressive brain atrophy, leukoencephalopathy, and gliosis.

[Brain stem compression preceded by trigeminal neuralgia in a patient with vertebro-basilar and bilateral carotid dolichoectasia].

We described a patient who had left trigeminal neuralgia by vertebro-basilar dolichoectasia, who underwent microvascular decompression separating the basilar artery of the trigeminal nerve by interposing a vascular graft piece. Symptoms resolved completely after surgery. Nine years later, he has a recurrence of facial pain associated with rapidly progressive brainstem compressive symptoms. The brain MRI showed the vertebro-basilar dolichoectasia exerting compression on the ventral-lateral aspect of the pons and the medulla. In cerebral angiography confirmed the presence of dilated tortuous ver…

The interhuman transmission, low level of personal hygiene, coprophagic habits, among others, may contribute to the dissemination of enteroparasitosis in individuals with mental diseases. The object of this paper was to verify the occurrence of enteroparasitosis in individuals with mental diseases assisted at São Marcello Nursing Home in Aracaju, Sergipe State, in the period between February and May at 2006, and track epidemiological aspects of the transmission. So, coproparasitologic exams were performed, and samples were manipulated by Hoffmann, Pons or Janner (1932) technique. The epidemiol…
