Reirradiation for patients with recurrence head and neck squamous cell carcinoma: a single-institution comparative study.

BACKGROUND AND OBJECTIVE: In the last decade, the number of publications that report on the use of external beam radiotherapy and high-dose-rate brachytherapy (HDR-BRT) in the treatment of recurrent head and neck cancer has increased, but no studies compare external beam radiotherapy and HDR-BRT. The aim of this study was to evaluate and to compare the efficacy and toxicity of the three-dimensional conformal radiotherapy (3D-CRT) and HDR-BRT in the treatment of recurrent head and neck cancer. MATERIAL AND METHODS: A total of 64 patients with head and neck cancer recurrence were randomly assig

Long-term outcome of hypofractionated radiotherapy to the whole breast of Japanese women after breast-conserving surgery.

BACKGROUND: In Japan, there are still no reports of long-term outcome for hypofractionated radiotherapy to the whole breast after breast-conserving surgery (BCS). We report our institution's results from evaluation of the efficacy and safety of hypofractionated radiotherapy for Japanese women. METHODS: Data in the medical records of 327 patients were retrospectively reviewed. The patients were treated with hypofractionated radiotherapy between January 2003 and December 2006 at the Kawasaki Medical School Hospital and were followed for more than 3 years. The median age was 54 years old (the ag

Effect of a topical treatment in organotypic culture of human breast skin after exposure to gamma-rays.

The early radiation of epidermal reactions can lead to healing of the lesion or radiation necrosis. There is no general agreement for either the prevention and/or treatment of radiation skin response, also as little is known about the immediate phases of this phenomenon. We investigated the early effects exerted by Healing and Wound Emulsion (HWE) on human skin response after ionizing radiation. Epidermal morphology, Heat Shock Protein (HSP) 70, and Transforming Growth Factor-beta1 (TGF-beta1) gene expression were investigated in organotypic human skin cultures undergoing a double dose of gamm

Genetic predictors of acute toxicities related to radiation therapy following lumpectomy for breast cancer: a case-series study.

INTRODUCTION: The cytotoxic effects of radiation therapy are mediated primarily through increased formation of hydroxyl radicals and reactive oxygen species, which can damage cells, proteins and DNA; the glutathione S-transferases (GSTs) function to protect against oxidative stress. We hypothesized that polymorphisms encoding reduced or absent activity in the GSTs might result in greater risk for radiation-associated toxicity. METHODS: Women receiving therapy in radiation units in Germany following lumpectomy for breast cancer (1998-2001) provided a blood sample and completed an epidemiologic

Advice on hair and scalp care during cranial radiotherapy: a prospective randomized trial.

BACKGROUND: The advice on hair washing during brain irradiation is aimed at minimizing radiation induced skin toxicity. We performed a prospective randomized trial to assess the effect of advice on scalp care on the local skin reaction in patients undergoing cranial radiotherapy. METHODS: One hundred and nine patients undergoing cranial radiotherapy were randomized into two groups. Patients in group 1 were advised not to wash hair during treatment and patients in group 2 to maintain normal pattern of hair washing. They were assessed weekly over a period of 10 weeks from the start of treatment

Colonic J pouch reconstruction of the radiation-damaged neorectum.

BACKGROUND: Postoperative radiotherapy after restorative resection for rectal carcinoma can cause fibrosis, stricture and proctitis. There are few accounts in the literature of the surgical approach to the radiation-damaged neorectum. METHODS: Reported are four patients who underwent resection of the radiation-damaged neorectum with colonic J pouch reconstruction between September 1996 and July 1997. The operative and postoperative course is described. Parameters of bowel function were assessed by standardized interview 6 months after ileostomy reversal. Ongoing follow up has occurred to the

[Principles in the assessment of radiation exposure in interventional measures].

Interventional procedures undoubtedly involve a high radiation exposure, for the patient. For some interventional procedures the frequencies of procedures, radiation exposures and considerations to the radiation risk are shown and discussed. Radiation risk is calculated with the currently common assumptions but always in consideration of the age and expectation of life of the patient. The risk coefficient used for the age-distribution of the common population is 5% per Sv, but for the risk coefficient for, e.g., the group of PTA patients 1.0% per Sv can be derived. That's why the risk of cance

Reconstruction of extensive defects of the parotid region: experience with the pectoralis major and free latissimus dorsi flaps.

Large defects of the parotid region resulting from excision of malignant tumours, or necrotic tissue due to radiotherapy, should not always be closed with local tissue, for several reasons. Occasionally, myocutaneous flaps are indicated, giving better results. We describe the problems of such tissue defects and our experience over a 10-year period using 2 different flap reconstruction techniques. A total of 28 cases, 25 with malignant tumours of the parotid or the external ear, 2 with extensive radionecrosis of the parotid region and 1 with a burn, were evaluated. The defects were restored wit

Increased cutaneous toxicity to ionizing radiation in HIV-positive patients. Military Medical Consortium for the Advancement of Retroviral Research (MMCARR).

BACKGROUND: There are reports of increased reactions in HIV-1+ patients to ultraviolet light sometimes in association with medication. In addition, there are also reports of increased morbidity associated with radiation therapy in HIV-1+ patients. METHODS: Three HIV-1+ patients developed cutaneous toxic reactions to radiation therapy; two with Kaposi's sarcoma (KS) and one with non-Hodgkin's lymphoma. CONCLUSIONS: Although the mechanisms which resulted in these reactions are not clear, they may be related to depletion of endogenous scavengers and may be accentuated by the pattern of immune d

[The treatment of cutaneous radiation-induced fibrosis with pentoxifylline and vitamin E. An empirical report].

BACKGROUND: Radiation fibrosis represents a severe complication of radiation therapy; standardized treatment protocols are lacking so far. Surgical excision rarely results in complete healing. PATIENT AND METHODS: We report on a 58-year-old female patient who developed a squamous cell carcinoma within the fibrotic area of the irradiation field on the right chest, resulting from a radiotherapy following mastectomy for breast cancer 17 years ago. After surgical excision of the carcinoma a combined treatment with pentoxifylline tablets (3 x 400 mg/d p.o.) and vitamin-E capsules (1 x 400 mg/d p.o

Techniques for reducing interventional neuroradiologic skin dose: tube position rotation and supplemental beam filtration.

PURPOSE: To limit the side effects of interventional neuroradiologic radiation, such as epilation, by applying a technique involving tube position rotation and by adding a supplemental inexpensive primary beam filter; and to show the dose effect of modifying technical factors. METHODS: Combined skin dose from fluoroscopy and digital subtraction angiography was measured with an array of 16 thermoluminescent dosimeters during interventional neuroradiologic procedures in 12 control subjects, in 18 patients whose procedures included addition of an inexpensive primary beam filter (0.5 mm aluminum/

Clinical experience with electron pseudoarc therapy.

Between November 1986 and June 1990, 24 patients were treated with electron pseudoarc therapy at McGill University. There were 21 females and three males aged 27 to 81 years (median 62 years). 17 patients, nine of whom had received previous conventional locoregional irradiation, were treated palliatively for locally extensive breast carcinoma. Eight of these 17 patients achieved a complete, and six a partial, response to treatment; nine subsequently developed evidence of progressive disease within the treatment field after intervals of one to 27 months (median 5 months) following therapy. Eigh

Mucocutaneous complications of intraarterial 5-bromodeoxyuridine and radiation.

5-Bromodeoxyuridine (BUDR), a halopyrimidine thymidine analogue, is incorporated into the DNA of dividing cells and causes photoradiosensitization. Twenty-five patients with malignant astrocytomas were treated with continuous intracarotid BUDR radiosensitization and radiotherapy for 8 1/2 weeks. Unique dose-limiting mucocutaneous complications were encountered. Ipsilateral facial dermatitis with epilation of eyebrows and eyelashes, ocular irritation, and bilateral nail dystrophy developed in all patients. Less common reactions included oral ulceration in six patients, body exanthem on the trun

[Radiotherapy of vulvar cancer. Treatment results of electron therapy in 446 patients 1956 to 1978].

446 patients with malignant tumour of the vulva were treated from 1956 to 1978. In 432 patients with invasive squamous cell carcinoma, a five-year survival rate of 43.1% was obtained after exclusive exposure to electron radiation. Severe complications were seen in 10.9% of the treated patients. On comparing the results with those obtained after electrocoagulation or vulvectomy, it was seen that a therapy based on electron radiation only yielded more favourable results, especially in cases with advanced stage carcinomas. In younger women with early tumour stages, surgery should be given prefere

N-CWS promotes the repair of radiation-induced skin injury by enhancing angiogenesis.

PURPOSE: Radiation-induced skin injury is a common complication that seriously affects the follow-up treatment and life quality of tumor patients. Nocardia rubra cell-wall skeleton (N-CWS) has been reported to have pro-angiogenesis effects, and its role on RISI remains unclear. The aim of this study was to investigate its effect on repair of radiation induced skin injury. MATERIALS AND METHODS: After exposure to 45 Gy X-rays, the irradiated areas of SD rats were treated by N-CWS every 3 days. The radioprotective effects of N-CWS were evaluated by body weight changes, skin scores, H&E staining

Exploring Black Soybean Extract Cream for Inflammatory Dermatitis-Toward Radiation Dermatitis Relief.

We aimed to evaluate the effect of black soybean extract cream (BSEC) on 2,4-dinitrochlorobenzene (DNCB)-induced dermatitis in murine models mimicking inflammatory dermatitis observed in humans. In this DNCB-induced model, BALB/c mice were spread with 100 μL of 2% DNCB twice a week for two weeks to induce skin inflammation on the shaved back skin; then, a placebo or BSEC that consisted of the volatile fraction derived from the seeds of Glycine max (L.) Merr. was applied to the DNCB-sensitized mice for 7 days. Gross visual analysis was conducted to assess the impact of BSEC on dermatitis, and a
