Copper foreign body in the lens without damage of iris and lens capsule.

PURPOSE: The objective is to report a rare case of asymptomatic penetrating injury with a copper foreign body found during an examination for cataract surgery. METHOD: A case report. RESULTS: A 73-year-old woman had vision loss. Examination by pupil dilation disclosed a dark-brown metallic mass located under the anterior capsule of the lens, as well as nuclear sclerosis of the lens. Although no history of traumatic injury was reported by the patient, careful examination revealed a dot corneal opacity. This finding, and the past history of the patient that she had worked in the fabrication of

Asymmetry of blinking.

PURPOSE: Too investigate asymmetry in eyelid movements with blinking, the stability of the asymmetry, and its modifiability in normal humans. METHODS: Differences in the start time and amplitude between the two eyelids were assessed for voluntary blinks and reflex blinks evoked by supraorbital trigeminal nerve stimulation. These variables were also measured before and up to 18 months after 2 hours of unilateral upper lid restraint. RESULTS: With voluntary blinks, one eyelid consistently began to close earlier and made a larger eyelid movement than the other eyelid. Stimulation of the supraor

[Bronchospasm during upper gastrointestinal endoscopy under sedation].

We experienced a case of bronchospasm during upper gastrointestinal endoscopy under sedation. An 80-year-old man came to our hospital with abdominal distension with pain, nausea and vomiting. He has the history of splenectomy, cholecystectomy for hemolytic anemia and thyroidectomy for thyroid cancer, surgery for bilateral shoulder joints and diabetes. Abdominal X-ray suggested obstruction of the small intestine. On the third hospital day, gastrointestinal endoscopy was scheduled for insertion of a long ileus tube. Under sedation with diazepam 10 mg and local anesthesia of the pharynx with lido

Lateral cervical epidural catheter placement for continuous unilateral upper extremity analgesia and sympathetic block.

BACKGROUND AND OBJECTIVES: The use of the laterally directed cervical epidural catheter for the treatment of acute and chronic pain has not been previously described. We have used this technique in a series of 30 patients to produce unilateral upper extremity analgesia and sympathetic block. We present a case report of a patient treated with this technique and a description of the technique and results from the series. METHODS: We placed an epidural catheter at the C6-7 vertebral interspace and directed the needle bevel and catheter to the affected side in a woman with a history of complex re

Safety of neck rotation for ear surgery in children with Down syndrome.

OBJECTIVE: Seek information about spinal cord safety for children with Down syndrome positioned for ear surgery. STUDY DESIGN: Prospective consecutive patients, each serving as his or her own control. METHODS: Somatosensory evoked potentials were recorded from 17 children who were undergoing elective otolaryngological surgery. None of the patients had neurological symptoms or physical examination findings suggesting cervical spinal cord embarrassment. Specifically, muscle tone was normal or mildly reduced globally, consistent with Down syndrome, and deep tendon reflexes were normal and not a

[Steinert's myotonic dystrophy: severe neonatal form with unknown family history].

UNLABELLED: A child with severe generalized hypotonia and respiratory insufficiency, with an unknown positive family history for Steinert's disease, is referred. We want to point out the importance of correct anamnesis and of physical examination during pregnancy to suspect and diagnose rare and incurable fetal pathologies. CASE REPORT: The child was born after caesarean section. Polyhydramnios and decreased fetal movements were noticed during pregnancy. At birth, the baby presented asphyxia (Apgar 4/6) and respiratory insufficiency: he was then intubed. He received assisted ventilation for 3

Atypical presentation of stroke in a diabetic hemodialysis patient.

Cerebrovascular disease is a major cause of death in patients with end-stage renal disease, particularly in those with diabetes mellitus. Cardiac disease frequently presents itself atypically in diabetic patients. This awareness has led to earlier detection and treatment of cardiac disease in diabetic patients. Whether cerebrovascular disease may also present itself in a highly atypical fashion in the diabetic patient with end-stage renal disease has never been addressed. We report the case of a diabetic hemodialysis patient who had an extremely unusual manifestation of stroke. A 67-year-old d

A groove observed in the face of an old man who probably died from acute asphyxia.

A continuous, horizontal groove about 0.7 cm wide was observed on the nape and lower part of the face of a 93-year-old man. The groove started from one corner of the mouth, traversed the nape, and reached the other corner of the mouth. It was most conspicuous at the nape. The mark was considered to have been produced by an electric cord. No facial congestion was noted, however, a few petechiae were present in the conjunctivae. There were no wounds in the front of the neck, while small hemorrhages were noted in the neck muscles. The right 2nd through 6th ribs were fractured and slight hemorrhag

Measurement of carotid body blood flow in cats by use of radioactive microspheres.

To resolve the controversy regarding carotid body blood flow, we used the radioactive microsphere technique for determination of tissue blood flow. We also measured the blood flow to several other tissues in the cat. Blood flow experiments were performed on 13 cats that were anesthetized, paralyzed, and mechanically ventilated with air. Different numbers of differently labeled 9-, 15-, and 25-micron microspheres were injected via a catheter into the left atrium. It was determined that one injection of 5 x 10(6) 15-micron microspheres was appropriate for the determination of carotid body blood

CONSERVATIVE PHYSIOTHERAPY MANAGEMENT OF CERVICAL SPONDYLOTIC RADICULOPATHY WITH RADIAL NERVE TENSION SIGNS: A CASE REPORT

Background: Cervical spondylosis with radiculopathy is a common cause of neck pain with referredupper-limb symptoms in middle-aged adults. Conservative physiotherapy is the recommended first-linemanagement strategy. Accurate clinical differentiation of cervical radiculopathy from peripheralentrapment neuropathies, shoulder pathology, and vascular causes requires systematic neurodynamictesting, segmental provocation assessment, and appropriate imaging.Case Presentation: A 45-year-old male office worker presented with a one-week history of right-sidedneck pain radiating along the radial nerve di

Total Ophthalmoplegia as an Early Manifestation of Herpes Zoster Ophthalmicus in an Elderly Patient With Asthma: A Case Report.

Herpes zoster ophthalmicus (HZO) may cause ocular surface disease, but ocular motor cranial neuropathy and total ophthalmoplegia are rare. These neurological deficits generally develop after the cutaneous eruption, making their presence at initial presentation unusual. Asthma has recently gained attention as a risk factor for herpes zoster, potentially through Th2-skewed inflammation, relative Th1-mediated antiviral immune insufficiency, and impaired cellular control of latent varicella-zoster virus (VZV). We report a 71-year-old man with poorly controlled asthma who presented with a one-day h

Methotrexate-induced leukoencephalopathy presenting as acute-onset limb weakness in a child: a case report.

BACKGROUND: Methotrexate is an essential medicine used to treat childhood malignancies including acute lymphoblastic leukemia. Neurotoxicity manifesting as leukoencephalopathy is an important adverse effect of methotrexate. Methotrexate-induced leukoencephalopathy classically demonstrates sub-acute-onset neurological manifestations that include learning disability, progressive dementia, drowsiness, seizures, ataxia, and hemiparesis. These are rare in children and are generally reported following intrathecal or intravenous use of methotrexate. In contrast, acute onset neurotoxicity with oral us

Sudden cardiac arrest during cesarean section -- a possible case of amniotic fluid embolism.

Amniotic Fluid Embolism (AFE) is a rare obstetric catastrophe that occurs in approximately 1/50,000 pregnancies and has a mortality rate in excess of 80%. AFE is a condition that is poorly understood and often difficult to diagnose. We report a case of a healthy 27-yr-old gravid two, 35 wk gestation parturient with a previous Cesarean section two years previously, and presently admitted for emergent Cesarean section due to premature uterine contractions. Induction of general anesthesias was performed with no problem and a male preterm infant with Apgar 8 at 1 min was delivered. Amniotic fluid

Monocular elevation deficiency associated ptosis misdiagnosed as simple congenital ptosis: management with Müller Muscle-Conjunctival Resection.

Purpose: Monocular elevation deficiency (MED), formerly termed double elevator palsy, is a rare ocular motility disorder characterized by restriction of elevation in all gaze positions. Ptosis frequently accompanies MED but may be mistaken for simple congenital ptosis, potentially resulting in suboptimal surgical planning. While frontalis sling and levator resection are the most frequently employed techniques for MED-associated ptosis, these procedures carry a considerable risk of lagophthalmos and suboptimal cosmetic outcomes. To date, the use of Müller muscle - conjunctival resection (MMCR)

Effect of concentric exercise-induced fatigue on proprioception, motor control and performance of the upper limb in handball players.

BACKGROUND: The last phases of a competitive game are when shoulder injuries most commonly happen, and fatigue is thought to be a major contributing factor, perhaps because of reduced proprioception and motor control. The purpose of this study was to investigate the effect of concentric fatigue on proprioception, motor control, and performance of the upper limb in handball players. METHODS: Forty-six right-handed handball players (all males, age 26.1 ± 5.54 years) were included in this test-retest laboratory experiment. Proprioception was assessed using joint reposition sense (JRS), threshold

[Hyperlexia in an adult patient with lesions in the left medial frontal lobe].

A 69-year-old right-handed woman developed a transcortical motor aphasia with hyperlexia following resection of a glioma in the left medial frontal lobe. Neurological examination revealed grasp reflex in the right hand and underutilization of the right upper extremity. An MRI demonstrated lesions in the left medial frontal lobe including the supplementary motor area and the anterior part of the cingulate gyrus, which extended to the anterior part of the body of corpus callosum. Neuropsychologically she was alert and cooperative. She demonstrated transcortical motor aphasia. Her verbal output b

Peripheral and Autonomic Neuropathy Status of Young Patients With Type 1 Diabetes Mellitus at the Time of Transition From Pediatric Care to Adult-Oriented Diabetes Care.

INTRODUCTION: The prevalence of neuropathic lesions in young patients with type 1 diabetes mellitus (T1DM) at the time of transition from pediatric care to adult-oriented diabetes care is poorly studied. A comparative study with healthy volunteers to assess the possible neuropathic condition of this special population and to identify the potential early screening needs has not been performed yet. The results may provide important feedback to pediatric diabetes care and a remarkable baseline reference point for further follow up in adult diabetes care. PATIENTS AND METHODS: Twenty-nine young p
