Scaphoid proximal pole destruction remains a surgical challenge owing to its high propensity for nonunion and osteonecrosis. The hemi-hamate graft has shown promising results in addressing this issue. However, long-term results of non-vascularized composite grafts remain uncertain. The purpose of this study was to investigate the feasibility of a vascularized hemi-hamate osteo-chondro-ligamentous pedicled flap for the reconstruction of the proximal pole of the scaphoid. Thirty fresh cadaveric wrists were used to harvest the hamate proximal pole on the dorsal intercarpal arch. A loss of substan

BACKGROUND: Scapho-lunate advanced collapse (SLAC) and scaphoid nonunion advanced collapse (SNAC) of the wrist are the most common types of wrist arthritis. We compared the union rate and complication profile of patients with SLAC and SNAC wrist undergoing 4 corner arthrodesis with partially threaded or fully threaded headless compression screws.
METHODS: A single-center retrospective review was conducted to identify all patients treated for SLAC and SNAC with 4 corner fusion using headless compression screws from 2016 to 2021. A total of 33 patients undergoing surgery on 35 wrists were ident

Correct interpretation of scaphoid axial length and carpal malalignment is difficult owing to the complex geometry of the scaphoid. Traditional measurements, such as the scapholunate angle and radiolunate angle, have shown limited reproducibility. To improve the assessment of these measurements, we used multiplanar reformation computed tomography with added average intensity projection. Four measurements for scaphoid morphology and carpal alignment were independently measured by four observers on computed tomography scans of 39 consecutive patients who were treated conservatively for scaphoid

Background: Fractures of the proximal pole of the scaphoid can be difficult to heal because of its limited vascular supply. Furthermore, nonunion with avascular necrosis and secondary fragmentation makes surgical reconstruction a challenging procedure. Method: We describe a technique and report the outcome of a proximal hamate autograft to treat a proximal pole scaphoid nonunion with avascular necrosis. Results: At 3.5 years post reconstruction, the patient remains asymptomatic with union of his scaphoid reconstruction without any evidence of scapholunate instability. Conclusion: Proximal hama

Removal of a cannulated headless screw following scaphoid fixation, when necessary, can prove to be difficult due to loss of bony integrity and the shape of the screw. This can result in greater exposure and more extensive soft tissue dissection for removal of the hardware. We describe a simple technique for percutaneous extraction of this hardware that can be done reliably with no additional equipment.

INTRODUCTION: Acutrak 2 screws are commonly used for scaphoid fracture fixation. To our knowledge, the variation in compressive force along the screw has not been investigated before. The objectives of our study were to measure variance in compression along the length of the standard Acutrak 2 screw, to identify the region of the screw which produces the greatest compression and to discuss the clinical relevance of this to the placement of the screw for scaphoid fractures.
MATERIALS AND METHODS: A laboratory model was set up to test the compressive force at 2mm intervals along the screw, usin

A Thai mother and son with distal symphalangism and other associated abnormalities are reported. Distal and middle phalanges of fingers and toes 2-5 were either aplastic/hypoplastic or fused between the corresponding digits. The second fingers and fourth fingernails were most severely affected in both patients. The mother's hands were less severely affected; the middle and distal phalanges of her hands were malformed and fused. Besides the absence of fusion lines, the shape of the fused middle and distal phalanges was quite different from that of other types of fusion, i.e., fused bones in bot

Background: The trapezoid is the least commonly fractured carpal bone, comprising 4% of all carpal fractures. To date, few articles have been published on isolated trapezoid fractures. Mechanisms of injury have typically been reported as an axial load, with or without forced wrist flexion/extension, that is transmitted from the second metacarpal indirectly to the trapezoid. Case Reports: Two patients presenting with symptoms of nonspecific wrist pain after acute trauma were initially worked up with plain film x-rays. Physical examinations identified nonspecific wrist pain in both patients. Mec

PURPOSE: Untreated scapholunate dissociation can lead to pain and eventually arthritis. There has been minimal limited success of soft tissue surgical procedures to repair or functionally replicate the torn structures. In this study, we evaluated a new surgical repair for scapholunate dissociation using a bioresorbable polymer approved for human applications.
METHODS: Eight cadaver wrists were tested in a wrist joint motion simulator. Each wrist was moved in continuous cycles of flexion-extension and radioulnar deviation. Kinematic data for the scaphoid and lunate were recorded for each wrist

The purpose of this study was to assess the therapeutic effects of intraoperative direct pillar corticosteroid injection in patients with carpal tunnel syndrome(CTS) who underwent open carpal tunnel release (OCTR) surgery. A prospective randomized controlled trial was conducted on 62 patients with carpal tunnel syndrome. Two intraoperative pillar injection solutions were compared: the lidocaine with triamcinolone acetonide and the lidocaine alone. Injections were administered at two points each at the scaphoid and trapezium bones in the radial pillar area, and at the hamate and pisiform bones

Hand injuries contribute a considerable workload for any individual accident and emergency department. Not many studies are found in the Indian literature as far as the epidemiology of hand fractures is concerned. A prospective survey of the medical charts of all 10,977 patients attending the accident and emergency department of the Kasturba Medical College, Manipal, from August 1, 2004, to July 31, 2006 was performed. Of these, 2,836 sustained one or more fractures, and hand fractures accounted for 5.4% of all fractures. This study compared the prevalence of hand fractures with that of all fr

Wrist injuries are an extremely common occurrence. The origin of these injuries may be due to a fracture of the two bones of the forearm or the wrist bones, which may sometimes require specific X-rays views. They are rarely responsible for a dislocation of the wrist. Strains wrists may be misdiagnosed and may result in instability of the wrist. The initial examination of all wrist injuries should systematically include the following four views: (1) a PAview of the wrist in pronation (with the palm facing downward). (2) a lateral view according to the Meyrueis technique. (3) an oblique view. (4

Scapholunate (SL) ligament injuries are a common cause of wrist pain and instability but can be diagnostically challenging. Standard radiographs, ultrasound, computed tomography, and magnetic resonance imaging have been used for many years to assist in the diagnosis of these injuries, and all of them have benefits and limitations. Advanced dynamic modalities, such as real-time magnetic resonance imaging and four-dimensional computed tomography, can distinguish functional from static injuries but are not widely available. The emergence of artificial intelligence applications is promising to imp

A male patient in his 50s presented with severe bilateral wrist pain caused by scapholunate advanced collapse (SLAC). He did not wish to undergo a salvage treatment, and consecutively both affected scaphoids were replaced by 3D-modelled prostheses. 3 and 2 years postoperatively, the patient had significant pain reduction, good wrist function and grip strength. Final X-rays demonstrated no signs of dislocation of the implant or signs of periprosthetic arthritis. Studies have shown that once SLAC occurs, the treatment should consist of either a proximal row carpectomy or scaphoidectomy and four-

Thirty-six patients were assessed after scapholunate ligament reconstruction using a portion of the extensor carpi radialis brevis through a dorsal approach. The median age was 53 years. Most (27/38) were graded as scapholunate advanced collapse Grade I. At a median of 47 months after treatment, hand function using the Disabilities of Arm, Shoulder and Hand Questionnaire was 12. The postoperative range of wrist flexion and extension movement was 77% and grip strength 92% compared with the uninjured side. The median patient satisfaction was rated as 9/10. Median pain scores without and with loa

We aimed to assess the influence of fracture location and comminution on acute scaphoid fracture displacement using three-dimensional CT. CT scans of 51 adults with an acute scaphoid fracture were included. Three-dimensional CT was used to assess fracture location, comminution and displacement. Fracture location was expressed as the height of the cortical breach on the volar and dorsal side of the scaphoid relative to total scaphoid length (%), corresponding to the fracture's entry and exit point, respectively. We found a near-linear relation between dorsal fracture location and displacement.

INTRODUCTION: Post-traumatic lesions of the carpus (scapholunate rupture, scaphoid non-union) frequently evolve into disabling osteoarthritis (scapholunate advanced collapse or scaphoid non-union advanced collapse: SLAC or SNAC wrist). Proximal row carpectomy (PRC) is a reliable option but with poorer prognosis in case of osteoarthritis of the distal radius or capitate head. In such situations, radiocarpal arthrodesis may be necessary, sacrificing wrist motion. To circumvent this limitation, we propose a new procedure consisting in arthroscopic PRC and replacing the proximal row by tendon graf
