[Macrodystrophia lipomatosa: a case report].

Macrodystrophia lipomatosa is a rare form of congenital localized gigantism. A forty-year-old male patient presented with complaints of swelling and deformity in the right toe and difficulty in wearing shoes. Conventional radiographic examination, ultrasonography, and computed tomography showed dorsal and medial deviations in the right toe, hypertrophy and degeneration in bone structure, increased density in the medullary bone, and a lipomatous mass with infiltration to the adjacent soft tissue muscles. Partial removal of the lipomatous tissues was performed. Microscopic examination of the spe

Occipital condyle fractures.

STANDARDS: There is insufficient evidence to support diagnostic standards. GUIDELINES: Computed tomographic imaging is recommended for establishing the diagnosis of occipital condyle fractures. Clinical suspicion should be raised by the presence of one or more of the following criteria: blunt trauma patients sustaining high-energy craniocervical injuries, altered consciousness, occipital pain or tenderness, impaired cervical motion, lower cranial nerve paresis, or retropharyngeal soft tissue swelling. OPTIONS: Magnetic resonance imaging is recommended to assess the integrity of the craniocer

[A case of adenocarcinoma arising in female urethral diverticulum].

An 81-year-old woman was admitted with a chief complaint of bloody discharge from the urethra. On physical examination an elastic-soft mass was palpable beneath the anterior vaginal wall. Magnetic resonance imaging and computed tomographic scan revealed the mass around the urethra. Transvaginal needle biopsy was performed and the histopathologic finding was adenocarcinoma. The tumor was excised with the uterus, the ovary and the anterior vaginal wall. The macroscopic appearance suggested that the tumor arose in the urethral diverticulum. She had intravesical recurrence at 8 months after the op

Anterior lumbosacral dislocation: case report and review of the literature.

BACKGROUND: Anterior lumbosacral dislocation is a rare traumatic anterior spondylolisthesis. METHODS: We report a new case surgically treated and review the eight cases previously reported. RESULTS: The mechanism of injury is hyperflexion-distraction. Neurological signs are inconsistent. Indirect radiological signs such as bilateral multiple transverse process fractures are strongly suggestive of the diagnosis. This complete soft tissue injury induces a definitive instability and requires an open reduction with posterior fixation. CONCLUSIONS: Suspicion of lumbosacral dislocation requires a

Cavernous malformation of the internal auditory canal. A case report.

A 51 year-old male, complaining of progressive left-sided hearing loss, tinnitus, and unsteady gait, underwent surgery with a probable diagnosis of intracanalicular acoustic neuroma, based on neurological, neurotologic, Computed Tomography (CT), and Magnetic Resonance Imaging (MRI) findings. During surgery, the Internal Auditory Canal (IAC) was opened and a reddish-colored, soft, vascular lesion was found within the VII and VIII cranial nerve complex. The lesion, which resulted to be a cavernous malformation, was removed without any postoperative deficits. This report stresses the diagnostic d

A European fitting philosophy for aspheric, high-Dk RGP contact lenses.

Microbial keratitis, the most serious complication of contact lens wear, is most frequently associated with soft contact lenses. Rigid gas permeable (RGP) lens wearers are less often affected. Increased use of high-Dk RGP lenses could reduce the number of cases of microbial keratitis. In Europe, aspheric high-Dk RGP lenses are very popular. This article presents practical guidelines for computer-aided fitting of aspheric RGP lenses. Base curve selection using the keratometer to determine the corneal topography is described. This method, which is inexpensive and time-saving, is suitable for fit

Medical surveillance for carpal tunnel syndrome in workers.

Carpal canal size was examined as a risk factor associated with carpal tunnel syndrome in the workplace. Seven of 14 electricians had symptoms of carpal tunnel syndrome. When cross-sectional areas were measured by computed tomography (CT), affected workers had a cross-sectional area of 1.75 +/- 0.21 cm2; control values were 2.53 +/- 0.15 cm2 (p less than 0.05). Subjects with a subclinical syndrome had an area of 1.83 +/- 0.22 cm2, which was similar to the group with symptoms. Wrist circumference was not a predictor of the smallest carpal canal area. Unusual bony and soft tissue structures with

Leiomyosarcoma: computed tomographic findings.

The computed tomographic (CT) findings in 118 patients with the diagnosis of leiomyosarcoma were reviewed. The tumor masses visualized in these patients were often quite large; extensive necrotic or cystic change was a frequent finding. Calcification was not observed in these tumors. The liver was the most common site of metastasis in these patients, with marked necrosis of the liver lesions a common finding. Other manifestations of tumor spread included pulmonary metastases, mesenteric or omental metastases, retroperitoneal lymphadenopathy, soft-tissue metastases, bone metastases, splenic met

Idiopathic orbital myositis: report of six cases.

Idiopathic orbital myositis, a subgroup of inflammatory orbital pseudotumor, occurred in six patients with acute onset periorbital pain, diplopia, and in most cases, eyelid swelling. Proptosis, ductional restrictions, and responsiveness to oral prednisone administration were characteristic. Computed tomography and orbital ultrasonography showed enlargement of one or more extraocular muscles, with sparing of other orbital soft tissues. One patient had involvement of all recti muscles in both orbits. The distinction of orbital myositis from Graves' ophthalmopathy is made on clinical grounds, wit

A Rare Presentation of a Postauricular Dermoid Cyst: A Case Report and Literature Review.

Dermoid cysts are rare congenital lesions composed of ectodermal and mesodermal elements, typically developing along embryonic fusion lines. They account for about 7% of cysts in the head and neck region, although they are uncommon in the postauricular area. This is a case of a 19-year-old male presenting with right postauricular swelling for one year. Computed tomography demonstrated a well-defined subcutaneous cyst without intracranial extension. A complete surgical excision was performed, and histopathological analysis confirmed a dermoid cyst. Postauricular dermoid cysts present a diagnost

A Case Report of Facial Swelling and Crepitus Following a Dental Procedure.

UNLABELLED: Subcutaneous emphysema (SE) is a rare but significant complication following dental procedures characterized by the presence of air in the subcutaneous tissue. This case report presents a 67-year-old male who developed right-sided facial swelling after tooth extraction, a procedure executed with high-power dental tools. Clinical findings included facial swelling and crepitus with no lip involvement, differentiating SE from an allergic reaction and hematoma. Diagnostic imaging through computed tomography (CT) and chest X-ray confirmed SE, showing diffuse subcutaneous air in facial a

Mesenteric cystic lymphangiomas in an 11-year-old female: A rare case report.

Mesenteric cystic lymphangiomas are a rare benign abdominal malformation of lymphatic vessels, with an estimated incidence of 1 per 250,000. Clinical presentation ranges from asymptomatic masses to acute abdominal pain. Diagnostic investigation includes ultrasound, abdominal computed tomography, or magnetic resonance imaging. Complete surgical excision is the recommended treatment. We present an 11-year-old female with abdominal cramps, and a 6-month history of gradually developing distension, constipation, and polyuria, without the occurrence of vomiting. Clinical examination revealed a soft,

Orbital Metastasis From Gastric Cancer Presenting as Orbital Cellulitis With Ptosis.

We report a rare case of orbital metastasis that originated from gastric carcinoma, which presented as orbital cellulitis with ptosis. Orbital metastasis accounts for about only 1% to 13% of orbital tumors. Orbital metastasis in orbital soft tissue or bony structures is very uncommon.A female patient with advanced gastric cancer with multiple metastases was referred to our clinic. She showed mild swelling and ptosis in her left eye. Contrast enhanced computed tomography imaging suggested orbital metastasis from gastric cancer in the superolateral aspect of the orbit. Based on her general condi

Maxillary, mandibular, and chin advancement: treatment planning based on airway anatomy in obstructive sleep apnea.

Surgical correction of obstructive sleep apnea (OSA) syndrome involves understanding a number of parameters, of which the 3-dimensional airway anatomy is important. Visualization of the upper airway based on cone beam computed tomography scans and automated computer analysis is an aid in understanding normal and abnormal airway conditions and their response to surgery. The goal of surgical treatment of OSA syndrome is to enlarge the velo-oropharyngeal airway by anterior/lateral displacement of the soft tissues and musculature by maxillary, mandibular, and possibly, genioglossus advancement. Imatinib mesylate in a patient with metastatic disease originating from a dermatofibrosarcoma protuberans of the scalp.

Dermatofibrosarcoma protuberans is a soft-tissue tumor that may recur locally and rarely causes metastases to vital organs. Dermatofibrosarcoma protuberans has specific chromosomal abnormalities involving the platelet-derived growth factor beta-chain locus that may render these tumors responsive to targeted therapy with the tyrosine kinase inhibitor imatinib mesylate. A patient with locally recurrent and metastatic dermatofibrosarcoma protuberans who had already undergone surgery 22 times was initially treated with imatinib mesylate 400 mg/day. The treatment dose was increased after 7 days to
