Staging of untreated nasopharyngeal carcinoma with PET/CT: comparison with conventional imaging work-up.

PURPOSE: We prospectively compared PET/CT and conventional imaging for initial staging of nasopharyngeal carcinoma (NPC). METHODS: A total of 111 patients with histologically proven NPC were investigated with PET/CT and conventional imaging (head-and-neck MRI, chest X-ray, abdominal ultrasound, and bone scan) before treatment. The respective findings were reviewed independently and then compared with each other. RESULTS: With regard to T staging, PET/CT showed a discrepancy with head-and-neck MRI in 36 (32.4%) of the study subjects. With regard to N staging, PET/CT showed a discrepancy with

Combined transconjunctival/intranasal endoscopic approach to the optic canal in traumatic optic neuropathy.

Surgical decompression of the optic canal is indicated in patients with traumatic optic neuropathy who fail to respond to corticosteroids. Traditional surgical approaches to the orbital apex have been effective in achieving optic nerve decompression but require either a craniotomy, provide limited exposure with late identification and protection of the optic nerve, or require external incisions. The combined transconjunctival/intranasal endoscopic approach to the optic canal offers sufficient exposure, allows early identification and protection of the optic nerve, provides space for the use of

Clinical spectrum and uncommon features of McCune-Albright syndrome in children: a cohort study from a National Referral Center.

INTRODUCTION: McCune-Albright syndrome (MAS) is a rare disease caused by somatic gain-of-function variants in the GNAS gene that lead to constitutive activation of the G protein alpha subunit (Gsα). Pathologic consequences can involve several tissues. Fibrous dysplasia (FD), café-au-lait skin macules and hyperfunctioning endocrinopathies are classic manifestations. However, the phenotypic spectrum of MAS is considerably wider and more complex. METHODS: We performed a pediatric retrospective study from our National Referral Center between 2007 and 2021 to describe the clinical spectrum of MAS

DeepFocus: a transnasal approach for optimized deep brain stimulation of reward circuit nodes.

Objective.Transcranial electrical stimulation (TES) is an effective technique to modulate brain activity and treat diseases. However, TES is primarily used to stimulate superficial brain regions and is unable to reach deeper targets. The spread of injected currents in the head is affected by volume conduction and the additional spreading of currents as they move through head layers with different conductivities, as is discussed in Forssellet al(2021J. Neural Eng.18046042). In this paper, we introduce DeepFocus, a technique aimed at stimulating deep brain structures in the brain's 'reward circu

Optic Canal Decompression: Concepts and Techniques: 2-Dimensional Operative Video.

The optic canal (OC) is a bony channel that transmits the optic nerve (ON) and ophthalmic artery (OphA) as they course through the lesser wing of the sphenoid bone to the orbital apex. The OC is involved in a variety of intracranial and extracranial pathologies,1 and opening of the canal may be necessary in order to achieve adequate exposure, better disease control, and vision preservation.2 Depending on the location of the pathology and its relationship with the optic nerve, the OC may be decompressed through an open transcranial approach or an endoscopic endonasal approach.1,3 OC drilling ca…

Craniofacial resection for paranasal sinus cancers.

BACKGROUND: Combined anterior craniofacial resection (CFR) has been in use for more than 25 years. The advent of the free revascularized tissue transfer flap in l980 permitted safe resection of tumors that had spread beyond the confines of the paranasal sinuses with immediate reconstruction of the sino-orbital cranial defect. The purpose of this study was to examine the outcomes and morbidity of a management policy of primary CFR and postoperative radiotherapy for paranasal sinus cancers infiltrating the skull base over a 21-year period. METHODS: Seventy-three patients with paranasal sinus ca

Endoscopic approach to the trigeminal nerve: an anatomic study.

OBJECTIVE: To describe an endoscopic perspective of the surgical anatomy of the trigeminal nerve. METHODS: Nine adult cadaveric heads were dissected endoscopically. RESULTS: Opening the pterygopalatine fossa is important because many key anatomical structures (V2, pterygopalatine ganglion, vidian nerve) can be identified and traced to other areas of the trigeminal nerve. From the pterygopalatine ganglion, the maxillary nerve and vidian nerve can be identified, and they can be traced to the gasserian ganglion and internal carotid artery. An anteromedial maxillectomy increases the angle of app

The endoscopic supraorbital approach to tumors of the middle cranial base.

BACKGROUND: Access to tumors of the middle cranial base has traditionally required wide surgical exposures via open craniotomies. These open techniques often require the use of potentially disfiguring skin incisions and are often associated with a significant degree of brain retraction and potential morbidity. We report our experience with the use of a minimally invasive supraorbital endoscopic approach through the eyebrow for excision of middle cranial base tumors in 2 cases. METHODS: We describe 2 patients with large-sized middle cranial fossa tumors (a medial sphenoid wing meningioma measu

Biomechanical mechanisms of orbital wall fractures - a transient finite element analysis.

As the biomechanical mechanisms of orbital wall fractures are still under research, three different fracture mechanisms were tested in a finite element based investigation. In contrast to earlier studies, a finer skeletal model and a transient dynamic simulation were used to test pure hydraulic, pure buckling and a mixed force transmission. Results showed that each set-up led to different orbital fracture patterns, which correlate well with clinical findings. Therefore the conclusion is that different mechanisms may act together explaining the variety of clinical fracture situations. Biomechan

Lateral supraorbital keyhole approach to clip unruptured anterior communicating artery aneurysms.

OBJECT: Clipping of an anterior communicating artery (A-com A) aneurysm requires various working angles for safe manipulation and observation. The lateral supraorbital keyhole approach provides a more lateral subfrontal corridor to observe and clip an A-com A aneurysm than the standard Perneczky method. METHODS: Preoperative planning was individualized in each patient based on three-dimensional computed tomography (3D-CT) angiography and 3D-CT osteotomy planning images. The procedure consisted of a 40-50 mm periorbital skin incision, partial dissection of the anterior portion of the temporal

Role of Multidetector Computed Tomography in the Evaluation of Maxillofacial Trauma.

INTRODUCTION: Maxillofacial fractures are among the commonest injuries occurring in trauma patients. Multislice computed tomography (CT) is a widely used radiological investigation that accurately reveals the number, location, and extent of the fractures as well as concomitant soft tissue injuries and has been found to be superior in the diagnosis of maxillofacial fractures owing to high sensitivity and specificity. This study was performed to assess the efficacy of axial, coronal, sagittal, and three-dimensional (3D) reformatted images in the detection of fractures in maxillofacial trauma. M

New Insights Into Cranial Base Ontogeny in Modern Humans Through Morphometric Analysis.

OBJECTIVES: The cranial base is located at the interface between the face, the brain, and the vertebral column. Understanding its growth is essential for elucidating key aspects of craniofacial development, neurodevelopment, and locomotor function. This study aims to examine the developmental trajectories of the three cranial fossae, anterior, middle, and posterior, in terms of both size and shape. MATERIALS AND METHODS: 203 CT scans of individuals aged from birth to 15 years were segmented and analyzed. Traditional morphometric analyses based on linear measurements were first conducted to as

Surgical management of congenital median perisellar transsphenoidal encephaloceles with an extracranial approach: a series of 6 cases.

OBJECTIVE: Median perisellar congenital transsphenoidal encephalocele (CTE) is a rare entity associated with multiple endocrine, visual, and respiratory deficits. The most likely causative factor for these pathological alterations is distension of neural structures (hypothalamic-pituitary system, optic pathway), resulting in protrusion of the dural sac through a sphenoid bone defect into the pharynx. The continuity with the extracranial space can be associated with an increased risk of tearing of the sac, with consequent cerebrospinal fluid leakage and subsequent increase in the risk of infect…
