Dynamics of Awake Hippocampal-Prefrontal Replay for Spatial Learning and Memory-Guided Decision Making.

Spatial learning requires remembering and choosing paths to goals. Hippocampal place cells replay spatial paths during immobility in reverse and forward order, offering a potential mechanism. However, how replay supports both goal-directed learning and memory-guided decision making is unclear. We therefore continuously tracked awake replay in the same hippocampal-prefrontal ensembles throughout learning of a spatial alternation task. We found that, during pauses between behavioral trajectories, reverse and forward hippocampal replay supports an internal cognitive search of available past and f

Neural mechanisms of social influence in adolescence.

During the transformative period of adolescence, social influence plays a prominent role in shaping young people's emerging social identities, and can impact their propensity to engage in prosocial or risky behaviors. In this study, we examine the neural correlates of social influence from both parents and peers, two important sources of influence. Nineteen adolescents (age 16-18 years) completed a social influence task during a functional magnetic resonance imaging (fMRI) scan. Social influence from both sources evoked activity in brain regions implicated in mentalizing (medial prefrontal cor

Amphetamine-induced sensitization has little effect on multiple learning paradigms and fails to rescue mice with a striatal learning defect.

Behavioral sensitization to psychostimulants such as amphetamine (AMPH) is associated with synaptic modifications that are thought to underlie learning and memory. Because AMPH enhances extracellular dopamine in the striatum where dopamine and glutamate signaling are essential for learning, one might expect that the molecular and morphological changes that occur in the striatum in response to AMPH, including changes in synaptic plasticity, would affect learning. To ascertain whether AMPH sensitization affects learning, we tested wild-type mice and mice lacking NMDA receptor signaling in striat

Inhibition related impairments of coherent motion perception in the attention-induced motion blindness paradigm.

A striking effect of selective attention on perception of first- and second-order motion has been termed 'attention-induced motion blindness' or AMB (Sahraie et al., 2001). The AMB paradigm is based on a rapid serial visual presentation (RSVP) task and causes a severe transient impairment of the detection of coherent motion in a random dot kinematogram (RDK). The effect crucially depends on irrelevant motion intervals (distractors) prior to the motion target. To account for this phenomenon, both psychophysical and electrophysiological studies point to the existence of a post-perceptual gate op

Speech disturbances and gaze behavior during public speaking in subtypes of social phobia.

Twenty-four social phobics with public speaking anxiety and 25 nonphobic individuals (controls) gave a speech in front of two people. Subjective anxiety, gaze behavior, and speech disturbances were assessed. Based on subjects' fear ratings of social situations, phobics and controls were divided into the generalized and nongeneralized subtype. Results showed that generalized phobics reported the most, and nongeneralized controls the least anxiety during public speaking. All subjects had longer and more frequent eye contact when delivering a speech than when talking with an experimenter or sitti

Role of imaging in temporal bone and cerebellopontine-angle lesions. A surgeon's perspective.

In recent years, with the advances in imaging technology, the gold standard for diagnosis of lesions of the temporal bone and cerebellopontine angle has shifted from histologic identification of the lesion from surgical specimens or autopsy material to noninvasive identification, localization, and characterization of the tissue by imaging. The symptoms caused by such lesions usually provide clues as to their location and probable nature. Clinical tests such as the vestibular evaluation, audiometry, auditory brain stem response, and otoacoustic emissions further help with topodiagnosis of the l

The chlordiazepoxide/pentylenetetrazol discrimination: characterization of drug interactions and homeostatic responses to drug challenges.

Rats were trained to discriminate chlordiazepoxide (CDP) from pentylenetetrazol (PTZ) in a two-lever food motivated discrimination task. Training drug doses were adjusted until subjects emitted approximately 50% of their responses on each of the two drug-appropriate levers during saline injection tests. Tests that followed injection of CDP/PTZ combinations illustrated a reciprocal antagonism between the two drugs. Saline-injection tests that followed large dose injections of CDP revealed a period of predominantly PTZ-appropriate responding that persisted after the initial period of predominant

Effect of ethanol and of noise on reaction time in the monkey: variation with stimulus level.

To determine whether the latency-increasing effects of ethanol were differential with respect to the intensity of the stimulus that initiated the response, three rhesus monkeys were trained on a behavioral task in which the latency of a simple motor response was measured following the onset of a pure tone stimulus. Following training, the animals were tested at a number of different tone intensities and functions relating latency to tone intensity were constructed. When these were stable, the animals were given ethanol in doses of 1.0-2.5 g/kg and the effects on response latencies to different

Deconstructing Participant Behaviors in Virtual Reality Simulation: Ethnographic Analysis.

BACKGROUND: Virtual reality (VR)-based simulation is an increasingly popular tool for simulation-based medical education, immersing participants in a realistic, 3D world where health care professionals can observe nuanced examination findings, such as subtle indicators of respiratory distress and skin perfusion. However, it remains unknown how the VR environment affects participant behavior and attention. OBJECTIVE: This study aimed to describe clinician attention and decision-making behaviors during interprofessional pediatric resuscitation simulations performed in VR. We used video-based fo

Association Between REM Sleep Behavior Disorder and Cognitive Dysfunctions in Parkinson's Disease: A Systematic Review and Meta-Analysis of Observational Studies.

Background: Rapid eye movement sleep behavior disorder (RBD) is thought to be a prodromal symptom of Parkinson's disease (PD). RBD is also thought to be involved in cognitive decline and dementia in PD. In PD, although the relationship between RBD and cognitive dysfunctions was confirmed by considerable studies, whether RBD was associated with distinct types of cognitive defects is worth of study. Objectives: This systematic review summarizes the evidence relating to cognitive dysfunction in PD patients with RBD (PD-RBD) and those without and explores their specificity to cognitive domains. After delivery: How recipient cells shape the fate and function of transplanted mitochondria.

BACKGROUND: Mitochondrial transplantation is built on a simple idea: failing cells might be rescued by healthy mitochondria. The approach has entered early clinical testing, and encapsulation and cell-type targeting have improved uptake in selected systems. Better delivery, however, does not explain the therapy. MAIN BODY: Internalised donor mitochondria encounter an established network, quality-control machinery and sensors for mitochondrial material. Donor material may remain in the endolysosomal system or reach the cytosol. Cytosolic fates include network integration, clearance and a candi

The Interplay Between Neuromodulation and Stem Cell Therapy for Sensory-Motor Neuroplasticity After Spinal Cord Injury: A Perspective View.

Spinal Cord Injury (SCI) rehabilitation is undergoing a transformative shift with the emergence of new treatment strategies. Historically, treatment options were limited, and few offered meaningful recovery. Recent work in human models has shown that neuromodulation specifically with spinal cord epidural stimulation (SCES) paired with task-specific training (TsT) can partially restore motor function such as the ability to stand, step, and perform volitional movements. Despite these advances, the recovery has been shown to plateau even with the combination of therapies. The recovery process typ

Style-Aware Contrastive Test-Time Adaptation: A Dual-Cache Model for Robust Vision-Language Alignment.

Test-time adaptation (TTA) has emerged as a key strategy to enhance vision-language models (VLMs) under real-world distribution changes. However, existing methods always face two problems: 1) The fundamental trade-off dilemma: parameter-free TTA retains inference efficiency but fails to correct modality misalignment, while prompt tuning adapts to shifts, incurs high computational costs, and lacks knowledge retention. 2) Discriminative collapse also exists in TTA when faced with fine-grained downstream tasks. To alleviate these two bottlenecks, we introduce Style-aware Contrastive Test-Time Ada

CXR-LT 2026 Challenge: Multi-Center Long-Tailed and Zero Shot Chest X-ray Classification.

Chest X-ray (CXR) interpretation is hindered by the long-tailed distribution of pathologies and the open-world nature of clinical environments. Existing benchmarks often rely on closed-set classes from a single institution, failing to capture the prevalence of rare diseases or the appearance of novel findings. To address this, we present the CXR-LT challenge. The first event, CXR-LT 2023, established a large-scale benchmark for long-tailed multi-label CXR classification and identified key challenges in rare disease recognition. CXR-LT 2024 further expanded the label space and introduced a zero

Pitch Strength & Percent Vocal Creak to Assess Treatment Response in Adductor Laryngeal Dystonia.

OBJECTIVE: Quantifying treatment-related voice change in adductor laryngeal dystonia (AdLD) is essential for optimizing botulinum neurotoxin (BoNT) therapy, yet clinical practice lacks objective, automated metrics to track response. Counting voice disruptions remains the standard but is labor-intensive and prone to error. This study evaluated whether pitch strength (PS) and percent vocal creak (%VC) (automated indices of pitch salience and irregular phonation) are sensitive to BoNT-related changes. We examined whether these metrics capture treatment effects at the group level, and through indi

Embracing Large Language Models for Medical Applications, Part II: Building a Framework for Clinical Stewardship.

Large language models (LLMs) have moved rapidly from experimental demonstrations to early clinical, educational, and administrative use. The first phase of medical LLM discourse emphasized opportunities in transfer learning, domain adaptation, clinical decision support, education, privacy, fairness, and regulation. The next phase requires a more specific framework for clinical stewardship. Since 2023, medical LLMs have demonstrated substantial progress in medical question answering, diagnostic reasoning in interactive clinical dialogue, patient communication, documentation support, and retriev

Perspectives on the Limits and Clinical Alignment of Medical AI from Population Statistics to Individual Care.

The clinical integration of artificial intelligence has outpaced the development of robust evaluative frameworks, raising critical safety concerns. This perspective establishes a clear taxonomy distinguishing probabilistic language models from deterministic classifiers and applies a multi-dimensional combinatorial model to calculate the requirements for complete diagnostic coverage. Our analysis demonstrates that comprehensive diagnostic coverage requires between 50,000 and 150,000 distinct, task-specific classifiers under subspecialty-level clinical granularity; conservative aggregated estima

Experience-dependent rapid structural changes in the human brain: A systematic review.

Structural brain plasticity has traditionally been viewed as a gradual process developing over days to weeks of training or experience. Recent neuroimaging evidence, however, demonstrates that measurable structural changes can occur within hours of behavioral or physiological manipulation, a phenomenon referred to as rapid structural plasticity. This systematic review synthesizes evidence for such short-interval (<2 h) structural alterations in gray matter across magnetic resonance imaging (MRI) modalities in humans. Thirty studies met inclusion criteria, assessing diffusion-weighted magnetic

Age-dependent synergistic control of sit-to-stand motion.

PURPOSE: Sit-to-Stand (STS) is a demanding postural task whose performance becomes challenging with aging. While muscle synergy analyses have helped describe its modular control, most evidence is based on unilateral EMG, which can characterize within-limb structure but cannot test whether the two sides share stable motor modules or reveal subtle inter-limb compensations in an apparently symmetric task. This study investigated the effects of aging on the neuromuscular organization of STS by examining bilateral muscle synergies and their relation to the whole-body center-of-mass (CoM) kinematics
