Temporal lobe abnormalities in semantic processing by criminal psychopaths as revealed by functional magnetic resonance imaging.

We tested the hypothesis that psychopathy is associated with abnormalities in semantic processing of linguistic information. Functional magnetic resonance imaging (fMRI) was used to elucidate and characterize the neural architecture underlying lexico-semantic processes in criminal psychopathic individuals and in a group of matched control participants. Participants performed a lexical decision task in which blocks of linguistic stimuli alternated with a resting baseline condition. In each lexical decision block, the stimuli were either concrete words and pseudowords or abstract words and pseud

Whole-brain voxel-based statistical analysis of gray matter and white matter in temporal lobe epilepsy.

Volumetric MRI studies based on manual labeling of selected anatomical structures have provided in vivo evidence that brain abnormalities associated with temporal lobe epilepsy (TLE) extend beyond the hippocampus. Voxel-based morphometry (VBM) is a fully automated image analysis technique allowing identification of regional differences in gray matter (GM) and white matter (WM) between groups of subjects without a prior region of interest. The purpose of this study was to determine whole-brain GM and WM changes in TLE and to investigate the relationship between these abnormalities and clinical …

Extradural temporal lobe retraction in the middle fossa approach to the internal auditory canal: biomechanical analysis.

HYPOTHESIS: The middle fossa (MF) approach is undergoing a marked resurgence in vestibular schwannoma surgery as a hearing conservation technique. It is widely recognized that the extradural temporal lobe retractors used in this procedure, despite their cleverness of design, could be improved. METHODS: To identify the characteristics of an ideal MF retractor, a systematic analysis of the safety and functionality of four commonly used retractors (House-Urban, Fisch, Garcia-Ibanez, and UCSF) in a human anatomical model was conducted. Intensity of temporal lobe compression, width of exposure, an…

[Relationship between imaging and pathological features and clinical factors in surgical cases of temporal lobe epilepsy].

The relationships between imaging, pathology and presumed causes in surgical cases of temporal lobe epilepsy (TLE) was studied. The subject was 62 patients (33 males and 29 females) who had had no attacks for more than one year after surgery. Average age at surgery was 28.2 +/- 9.9 years. Obvious neurological abnormalities were not found in these cases. MRI, PET and SPECT were performed. Hematoxylin and eosin was used for pathological judgement. Their medical charts were used to investigate their clinical factors. Although patients suspected of having encephalitis/meningitis had been hospitali…

Experimental models of temporal lobe epilepsy: new insights from the study of kindling and synaptic reorganization.

Temporal lobe epilepsy is a common localization-related epileptic syndrome characterized by complex partial seizures, ictal and interictal epileptic discharges arising from limbic structures of the temporal lobe, and association with hippocampal sclerosis. Temporal lobe epilepsy may follow perinatal injury and febrile convulsions, may be progressive, and frequently becomes refractory to standard antiepileptic therapy. The neurobiology that underlies these features of temporal lobe epilepsy is not known. Recent studies in experimental models have provided new insights that may help clarify the …

Neuronal, dendritic, and vascular profiles of human temporal lobe epilepsy correlated with cellular physiology in vivo.

Partial complex seizures are known to arise from abnormal firing of neurons in cortex that has histologic abnormalities associated with tumors, infarcts, or neuron loss. The latter pathology of sclerosis is most frequently found in the hippocampus, and partial seizures from this region are focalized by direct electrical recordings and treated by anterior temporal lobectomy. Although we can link this hippocampal sclerosis to nearby hyperexcitability, the synaptic mechanisms involved in hippocampal seizure genesis are not yet known. We have used in vivo microelectrode recordings from hippocampal…

Outcomes of surgically and non-surgically managed temporal encephalocoeles in the context of drug-resistant temporal lobe epilepsy: A retrospective single-centre case series.

INTRODUCTION: Temporal encephalocoeles are a recognised cause of drug-resistant temporal lobe epilepsy (TLE), with uncertain associations to epileptogenesis and an unclear optimal management approach. Operative management, particularly resective temporal lobe surgery, has been proposed, but outcomes and decision-making criteria remain debated. This study aims to evaluate the outcomes of surgically and non-surgically managed patients with temporal encephalocoeles in the context of drug-resistant TLE, focusing on seizure freedom rates, postoperative complications and factors influencing manageme…

Early Institution of Immunotherapy Alters the Course of Temporal Lobe Epilepsy With GAD65 Antibody: A Clinical Report.

INTRODUCTION: Glutamic acid decarboxylase 65 (GAD65) antibody-associated temporal lobe epilepsy (GAD65-TLE) is a rare form of autoimmune-associated epilepsy characterized by medically refractory seizures and cognitive decline. Diagnosis typically requires antibody testing and clinical correlation, and treatment options include immunotherapy and antiseizure medications (ASMs). However, many patients experience persistent or worsening symptoms despite ongoing treatment. CASE REPORT: A 20-year-old woman presented with mild cognitive impairment and focal aware seizures that gradually progressed i…

Emotion recognition in patients with temporal and frontal lobe epilepsy and its relationship with perceived social functioning.

OBJECTIVE: The study examined emotion recognition (ER) across visual and auditory modalities in patients with temporal lobe epilepsy (TLE) and frontal lobe epilepsy (FLE), and explored associations with perceived social functioning (SF). METHOD: Fifty patients (30 TLE, 20 FLE) and 50 healthy controls (HC) completed tasks assessing recognition of facial emotion and emotional prosody across seven emotions: neutral, happiness, surprise, anger, disgust, fear, and sadness. Patients also completed the Social Functioning subscale of the Quality of Life in Epilepsy Inventory-31 (QOLIE-31) and self-re…

Unique deficits in place coding across subfields of the hippocampus in a mouse model of temporal lobe epilepsy.

OBJECTIVE: Memory problems are comorbid with temporal lobe epilepsy (TLE). Animal models of TLE reveal impairments in spatial firing fields of hippocampal place cells, providing a potential neural substrate for memory problems. Each subfield of the hippocampus carries out unique aspects of spatial memory, yet little is known about how individual subfields are perturbed. Here, we investigated the spatial coding properties of the three major subfields of the hippocampus. METHODS: Single unit recordings were made from CA1, CA3, and the dentate gyrus (DG) of mice (n = 10, 6 male [M]/4 female [F])…

The spatial learning task of Lhermitte and Signoret: Marker of bilateral hippocampal dysfunction in temporal lobe epilepsy.

OBJECTIVE: Bilateral hippocampal compromise in temporal lobe epilepsy (TLE) substantially increases the risk of significant adverse memory outcomes following unilateral anterior temporal lobectomy and may contraindicate surgery. The Spatial Learning Task (SLT) of Lhermitte and Signoret (1972) is a simple object-location arbitrary associative learning task proposed as a marker of bitemporal dysfunction. Here, we test whether the SLT might serve as a useful marker in TLE by examining, in individuals with TLE and hippocampal sclerosis (HS), whether performance on the SLT depends on the laterality…

A Comprehensive Review of Worldwide Case Series of Neurocysticercosis and Mesial Temporal Lobe Epilepsy with Hippocampal Sclerosis.

BACKGROUND: Neurocysticercosis (NCC) is a frequent cause of epilepsy in endemic regions. An association between NCC and hippocampal sclerosis (HS), a key pathological substrate of mesial temporal lobe epilepsy, has been increasingly reported but remains controversial. SUMMARY: A literature search was conducted in PubMed, Scopus, and ScienceDirect focusing on publications describing epilepsy due to NCC with HS. Twenty-three original articles, identified through 2023, were retrieved from Brazil, Ecuador, India, Mexico, and Korea. The proposed relationships between NCC and HS include incidental …

Identification and validation of diagnostic biomarkers for temporal lobe epilepsy related to ferroptosis and potential therapeutic targets.

Ferroptosis pathway activation is potentially correlated with temporal lobe epilepsy (TLE). However, the diagnostic significance and mechanism of ferroptosis-related genes (FRGs) in TLE require further investigation. A comprehensive analysis of the GSE134697 dataset from the Gene Expression Omnibus (GEO) database using Weighted gene co-expression network analysis (WGCNA) identified 3,212 differentially expressed genes (DEGs) between temporal lobe epilepsy (TLE) and control groups, with a critical focus on the turquoise module. Through intersection of DEGs and key module genes, correlation anal…

Developing an anatomically valid segmentation protocol for anterior regions of the medial temporal lobe for neurodegenerative diseases.

BACKGROUND: The anterior portion of the medial temporal lobe (MTL) is one of the first regions targeted by pathology in sporadic Alzheimer's disease (AD) and Limbic-predominant Age-related TDP-43 Encephalopathy (LATE) indicating a potential for metrics from this region to serve as imaging biomarkers. Leveraging a unique post-mortem dataset of histology and magnetic resonance imaging (MRI) scans we aimed to 1) develop an anatomically valid segmentation protocol for anterior entorhinal cortex (ERC), Brodmann Area (BA) 35, and BA36 for in vivo 3 tesla (T) MRI and 2) incorporate this protocol in a…

The antioxidant and anticonvulsant effects of ellagic acid in kainic acid-induced temporal lobe epilepsy in mice.

Oxidative stress (OS) resulting from high levels of free radicals contributes to the initiation and progression of epilepsy. Temporal lobe epilepsy (TLE) is associated with alteration in the structure and function of the hippocampus and is modeled in mice using kainic acid (KA). In this study, the neuroprotective effect of ellagic acid (EA) on KA-induced epilepsy in mice was evaluated. Sixty male Swiss albino mice were assigned to six groups: I received normal saline (NS; 10 ml/kg, intraperitoneally (i.p.)); II, received KA (15 mg/kg, i.p.); III, received diazepam (20 mg/kg, i.p.) and KA (15 m…

Positive correlation between cognition and inflammation in the cerebral cortex of patients with mesial temporal lobe epilepsy.

Cognitive impairments are commonly observed in individuals with mesial temporal lobe epilepsy (mTLE), yet the underlying mechanisms remain poorly elucidated. This study investigates the potential relationship between neuroinflammation as determined using radioligand binding to markers of neuroinflammation and cognitive dysfunction in this patient population. Quantitative autoradiography was performed using the radioligands [123I]CLINDE (for 18 kDa translocator protein (TSPO)) and [3H]JNJ-64413739 (for P2X purinoceptor 7 receptor (P2X7R)) on temporal neocortical specimens removed neurosurgicall…
