The extensor pollicis longus-loop-knot (ELK) procedure for dynamic balance of the paralyzed thumb interphalangeal joint.

Hyperflexion of the thumb interphalangeal (IP) joint interferes significantly with pinching between the thumb and the index finger in a paralyzed hand. The extensor pollicis longus-loop-knot (ELK) procedure was used successfully in 7 tetraplegic patients to balance the IP joint between strong restored flexor function and lacking or weak antagonism of the thumb extensors. A V-shaped incision was made over the extensor hood at the level of the IP joint, the extensor pollicis longus (EPL) tendon was elevated, a loop was formed, secured by sutures, and then turned proximally onto the EPL tendon it

Lateral column calcaneal lengthening, flexor digitorum longus transfer, and opening wedge medial cuneiform osteotomy for flexible flatfoot: a biomechanical study.

BACKGROUND: Lengthening of the lateral column is commonly used for reconstruction of the adult and pediatric flatfoot, but can result in supination of the foot and symptomatic lateral column overload. The addition of a medial cuneiform osteotomy has been used to redistribute forces to the medial column. The combined use of a lateral column lengthening and medial cuneiform osteotomy in a reproducible cadaver flatfoot model was evaluated. METHODS: Twelve cadaver specimens were physiologically loaded and each was evaluated radiographically and pedobarographically in the following conditions: 1)

[Restoration of flexion of the elbow joint in patients with old injuries of the brachial plexus].

Examination of 23 patients with complete disorder of the flexion function due to irreversible injury of plexus brachialis. The translocation of m. triceps brachii in 8 patients, m. latissimus dorsi--in 8, m. sternocleidomastoideus--in 5, m. pectoralis major transposition was conducted in 1-5 years after the injury for the m. biceps brachii function substitution. The most complete restoration of flexion in articulation ulnaris was achieved after m. latissimus dorsi transposition. Good result was achieved after m. triceps brachii transposition conduction.

[Surgical treatment for hallux valgus by metatarsal I osteotomy and soft tissue reconstruction in the area of the forefoot].

A series of 39 surgeries for hallux valgus has been analyzed. All cases had deformity exceeding 40 degrees (mean 61 degrees). Metatarsal I osteotomy has been supplemented with soft tissue reconstruction. McBride reconstruction and second toe extensors transfer were done in 32 cases; the best results were achieved in this group of patients.

[Transposition of flexor pollicis brevis muscle for reconstruction of opposition function of the thumb: anatomical study and clinical application].

Anatomical study was performed on 20 cadaveric hands and the results showed that M. flexor pollicis brevis overlap the M. abductor pollicis brevis for about half of its width at their origins and the overlapping reduced to about 1/3 of the width at the muscle belly level. The M. flexor pollicis brevis chiefly inserts on the palmar aspect of the base of proximal phalanges. The M. abductor pollicis brevis primarily inserts to the radial side of MP joints, the M. flexor pollicis brevis is entirely innervated by the deep branch of the ulnar nerve. In order to increase the angle bettween longitudin

Biomechanical basis of transfers for shoulder paralysis.

Elevation of the scapulohumeral joint can be executed by two different and distinct systems, both capable of separately performing this movement. The deltoid muscle is composed of several parts contracting independently, sometimes antagonistically between them and providing elevation in different directions. Parts two and three (Fick) of the deltoid are essential because they are lateral to the centers of rotation and work in the plane of the scapula. The supraspinatus and adjacent muscle system can produce compensatory movements with full elevation in the plane of the scapula. Lateral rotatio

Subtalar arthroereisis: a combined technique.

This study reports the evolution and results of an arthroereisis procedure utilizing a talonavicular desmoplasty, tibial tendon suspension (posterior and anterior) and implantation of a Silastic interpositional sphere in the sinus tarsi. Four cases were reviewed with a maximum 6-year follow-up. This study should be considered when presented with a severely hypermobile and symptomatic flatfoot in the child. The procedure is efficacious in combination with a strict postoperative regimen and appropriate biomechanical management.

Surgical rehabilitation of the upper limb in quadriplegia.

Fifteen quadriplegic (tetraplegic) patients from the Spinal Injuries Unit of the Austin Hospital have had surgery to improve function in 17 of their upper limbs. Changes in strength, function, subjective ratings and the influence on 198 activities of daily living are reviewed. The posterior third of the deltoid muscle was transferred into 8 triceps tendons to provide active elbow extension in seven patients. Six patients had transfers of forearm muscles to provide grasp and lateral pinch or active extension of wrist and fingers. Other operations included transfer of the latissimus dorsi to the

The posterior deltoid to triceps transfer: a clinical and biomechanical assessment.

Transfer of the posterior deltoid muscle to the triceps insertion for elbow extension provides improved function in patients with C5 and C6 level tetraplegia. We have modified the surgical technique using the tibialis anterior tendon as a graft. The length-tension characteristics and available amplitude of the posterior deltoid muscle were determined with intraoperative electrical stimulation. The excursion of the posterior deltoid muscle was 7.31 cm with a standard deviation of 1.23 cm. Postoperative mean torque measurements of elbow extension power were 36.4 kg cm with a standard deviation o

The treatment of relapsing clubfoot by tibialis anterior transfer underneath the extensor retinaculum.

Nineteen patients with 22 relapsing clubfeet were treated by tibialis anterior transfer underneath the extensor retinaculum. This operation is indicated in cases of relapse in which the clubfoot is still supple. It is not indicated in cases with subtalar rigidity. The results were excellent in 19 cases, fair in 2 and poor in one. The fair and poor results were attributable to faulty technique or errors of indication.

Feedback goniometers for hand rehabilitation.

Active range-of-motion exercises and related activities are an important means for applying progressive stress to induce remodeling of various tissues that restrict range of motion in the hand and wrist. Supplying joint angle feedback to patients as they exercise can help them more consistently to reach the flexion/extension goals set by the therapist. To accomplish this aim, feedback goniometers have been developed that are worn on the hand during exercise and therapeutic activities. These devices supply a threshold feedback signal when a predetermined joint angle is attained. Clinical trials

Free Flap Transfer with Arteriovenous Loop Establishment for Upper Limb Salvage in a Crush Injury.

Constructing arteriovenous (AV) loops saves a crushed limb when healthy recipient vessels are unavailable at the site of injury by supporting a transferred free flap. Literature about using AV loops in the upper limb defect is scarce. We present a case with a devastating upper limb crush injury that was successfully managed with fibula flap transfer preceded by AV loop construction. A 38-year-old woman was admitted to our hospital with severe crushing trauma to her right forearm and hand. There was significant skin loss with injuries to blood vessels and tendons. She underwent multiple staged
