Resection of acquired tracheal stenosis in childhood.

In a series of 74 tracheal resections, six were performed in boys aged 2 to 13 years. One patient was intubated shortly after operation because of bulbar paralysis with swallowing difficulties. He died 5 months later of traumatic hydrocephalus. Another patient was reintubated because of an accompanying nondiagnosed glottis stenosis which later was treated successfully. Four patients have been followed up from 4 to 10 years. In these patients tracheoscopy and x-ray examination have shown that the anastomosis has grown at the same rate as the rest of the trachea. It is concluded that operation c

Communication development in young children with long-term tracheostomies: preliminary report.

Communication development was studied in 77 subjects with long-term tracheostomies ranging in age from two months to 7 years. Children were categorized according to tracheostomy status (cannulated or decannulated), presence of speech practice prior to cannulation, and level of language development (prelinguistic or linguistic) at the time of decannulation. All were aphonic and consequently deprived of speech experience for extended periods while tracheostomized. Twenty-three children have been studied post-decannulation. Results for the children decannulated during the prelinguistic stage reve

Descending necrotizing mediastinitis.

From January 1975 through July 1981, ten patients with mediastinitis complicating an oropharyngeal infection, that is, a form of mediastinitis best termed as DNM, were encountered at our institution. Based upon rather relatively stringent diagnostic criteria, 21 other instances were found in the literature from 1960 to 1980, a time period well into the antibiotic era. The predominant underlying oropharyngeal infection was of odontogenic origin, specifically, infection involving the mandibular molars. Bacteriologically, DNM is most frequently a polymicrobial process, with anaerobes playing a ma

Short-term intubation in children with acute epiglottitis.

Management of the airway in acute epiglottitis is still controversial. One argument against intubation as opposed to tracheostomy is the increased complications from prolonged intubation. We describe a study of 26 children treated for acute epiglottitis from 1971 to 1979. Included in this series are 16 consecutive patients who were treated by nasotracheal intubation and simultaneous medical therapy. Fifteen of the 16 patients extubated safely in less than 48 hours, significantly decreasing the possibility for complications.

Management of acute epiglottitis.

Airway obstruction secondary to acute epiglottitis is a dramatic, life-threatening emergency. Early diagnosis and provision of an adequate artificial airway are critical in the management of these patients. Tracheostomy has been the traditional method of securing the airway in acute epiglottitis, but more recently, endotracheal intubation has been advocated. Whichever method of airway management is preferred, it is imperative that every hospital have a protocol for the management of acute epiglottitis so that immediate action can be taken.

Tracheal leiomyosarcoma: a unique cause of stridor.

A 26-year-old man presented with a 2-week history of hemoptysis and progressive upper airway obstruction. Roentgenograms and bronchoscopy confirmed a tracheal tumor, and a primary leiomyosarcoma was resected.

Acute epiglottitis - 25 years' experience in management, The Montreal Children's Hospital.

Acute epiglottitis is the most treacherous inflammatory process that can affect the respiratory tract. It can be a cause of sudden death in children. The clinical features of this entity are reviewed and our experience with its management at The Montreal Children's Hospital during the past 25 years is presented. The establishment of a "secure" airway by tracheostomy and in recent years, by nasotracheal intubation has decreased to practically nil the mortality of children who arrive alive at this hospital with acute epiglottitis.

[25 years experience with glottis widening surgery in our clinic (author's transl)].

Between 1949 and 1974, 137 patients with bilateral vocal cord paralyses were operated upon in our Department. In 127 cases King's method of extralaryngeal lateral fixation of the vocal cord or a modification thereof was executed on one or both sides. The modification consisted of securing the lateral-fixating suture around the superior thyroid cornu. This produces an additional displacement of the vocal cord in the vertical plane which provides good respiratory function in spite of only moderate lateral displacement of the cord. In seven cases endolaryngeal arytaenoidectomies and submucus hemi

Nasotracheal intubation in acute laryngotracheobronchitis.

The case histories of 150 children admitted to hospital with acute laryngotracheobronchitis (LTB) are reviewed. Relief of severe upper airway obstruction was achieved by nasotracheal intubation in 27 children (18%). There was no mortality in this series. Nasotracheal intubation is therefore a safe and effective means of treating severe LTB. In addition, the importance of meticulous nursing care is stressed.

Airway obstruction due to spontaneous retropharyngeal hemorrhage.

A patient is described with polycythemia vera who was taking anticoagulants and developed a spontaneous retropharyngeal hemorrhage after a violent sneezing episode. This progressed to tracheal compression with stridor after he had taken some aspirin for relief of the neck pain. An emergency tracheostomy was life saving.
