Surgical Management of Pott's Paraplegia in a 27-Week Pregnant Woman with Continuation of Pregnancy: A Case Report.

INTRODUCTION: Spinal tuberculosis is primarily managed with antitubercular therapy, while surgical intervention is reserved for cases with neurological compromise or spinal instability. The occurrence of Pott's paraplegia during pregnancy is rare and presents unique diagnostic and therapeutic challenges due to concerns regarding fetal safety, imaging limitations, and surgical risks. Management during the second trimester is particularly complex, as options, such as medical termination or delivery are not feasible. Only a limited number of case reports describing Pott's paraplegia during pregna

Infectious Spondylodiscitis of Bacterial Causes in Adults: Epidemiology, Pathophysiology, Diagnostic and Treatment Challenges.

Spinal infections in general, and infectious spondylodiscitis in particular, are increasingly diagnosed in the Western world, in recent decades. This rise in incidence is associated with an ageing population and with an increased availability of accurate diagnostic modalities. Even so, due to the non-specific nature of clinical manifestations, and of the implicated blood and serum markers, there is a risk of underdiagnosis or misdiagnosis of the disease in its initial stages. Ionizing radiation methods, such as plain radiography (X-ray) and computed tomography (CT), are also not reliable in th

Francesco Parona (1842-1908) and his contributions to our understanding of surgery through anatomy.

Much of the life of Francesco Parona and many of his contributions to medicine are unknown outside of Europe. Parona made novel contributions to many surgical techniques and medical treatments and was an active member of society and the Italian political regime. Parona's name lives on eponymously by his "space" in the forearm. This paper will discuss the personal life and medical contributions of Francesco Parona.

Comparison of anterior instrumentation systems and the results of minimum 5 years follow-up in the treatment of tuberculosis spondylitis.

Anterior debridement, strut grafting and instrumentation have an increasing popularity in the treatment of tuberculosis of spine. Anterior fixation can be done either by a plate or a rod system. This study reports on the surgical results of 59 patients with Pott's disease that had anterior radical debridement and anterior fusion and anterior instrumentation with 5 years follow-up. Average age at the time of operation was 46.3+/-13.5 years. Average follow-up was 84.6+/-11.3 months. Local kyphosis was measured as the angle between the upper and lower end plates of the collapsed vertebrae preoper

Tuberculosis of the cervical spine.

Tuberculosis of the cervical spine is rare, comprising 3-5% of cases of tuberculosis of the spine. Eight patients with tuberculosis of the cervical spine seen during 1989-1992 were reviewed. They all presented with neck pain. The 4 children presented with a kyphotic deformity. In all the children the disease was extensive, with a large prevertebral abscess formation, while in the adults it was localised to one or two motion segments. Cord compression was present in 4 of the 8 patients. All the patients were treated with antituberculosis drugs and 6 underwent surgery. There was full neurologica

[Inflammatory and tumorous metastases of the thoracic spine: operative management and results (author's transl)].

133 patients with inflammatory or tumorous metastases of the thoracic spine were operated upon within the last 11 years. Radical removal of inflammatory lesions with consecutive corticalis-spongiosa-plasty, taken from the christa pelvis, and chemotherapy are a therapeutical unit. Excision of the tumor with supplement bone grafting or "Pallacosplombe" and stabilization according to the principals of osteosynthesis are rewarding in individual cases. Four out of nine tumor patients survived two years after surgery.

Disseminated Tuberculosis Revealed by Conus Medullaris Tuberculoma.

A 29-year-old man from Comoros presented with rapidly progressive paraplegia and sexual dysfunction. Magnetic resonance imaging (MRI) showed a contrast-enhanced conus medullaris lesion. Differential diagnoses included tumors, abscesses, and inflammatory diseases. Neurosurgery was delayed to complete examinations. Cerebral MRI showed three abscesses. Body computed tomography scan showed supracentimetric polyadenopathies, pulmonary nodules, prostatic lesion, and enhanced seminal vesicle, with hypermetabolism on positron emission tomography with 2-deoxy-2-[fluorine-18]fluoro-D-glucose scan. Surgical management of complex post-tuberculous kyphosis among African patients: clinical and radiographic outcomes for a consecutive series treated at a single institution in West Africa.

STUDY DESIGN: Retrospective review of consecutive series. OBJECTIVE: To assess the clinical and radiographic outcomes after surgical management of post-tuberculous kyphosis. Post-tuberculous (TB) kyphosis can lead to progressive pulmonary and neurological deterioration. Surgery is indicated to decompress neural elements and correct the spine deformity. Although posterior vertebral column resection (PVCR) has been established as the treatment of choice for severe TB kyphosis, there is paucity of studies on the clinical outcomes among patients treated in West Africa. METHODS: Clinical and radi

TUBERCULOSIS CAUSED BY MYCOBACTERIUM ORYGIS IN A GREATER ONE-HORNED RHINOCEROS (RHINOCEROS UNICORNIS): FIRST REPORT IN THE WESTERN HEMISPHERE.

Mycobacterium orygis, a newly identified member of the Mycobacterium tuberculosis complex, has been isolated predominantly from hoofstock in eastern Africa and the Arabian Peninsula, and sporadically in cattle (Bos taurus indicus), rhesus monkeys (Macaca mulatta), humans, and a greater one-horned rhinoceros (Rhinoceros unicornis) in South Asia. In rhinoceros, tuberculosis typically presents as a chronic progressive respiratory disease. The report describes the postmortem diagnosis of tuberculosis caused by Mycobacterium orygis in a greater one-horned rhinoceros with hind limb paresis due to ne

Severe Pott's Kyphosis in a 19-Month-Old Child: Case Report and Review of Literature.

BACKGROUND: Spinal osseous tuberculosis, or Pott's spine, although very common in endemic countries, has a lower incidence in very young children. However, the infection has the propensity to cause greater vertebral destruction in this age group, leading to severe structural kyphotic deformity and associated neurologic deficits. We report the case of a 19-month-old child with severe tubercular kyphotic deformity of the upper thoracic spine managed with posterior vertebral column resection (VCR) and nonfusion posterior pedicle screw instrumentation. CASE DESCRIPTION: This 19-month-old boy pres

[Histoplasmosis mimicking tuberculosis spondylodiscitis in a patient with rheumatoid arthritis].

Osteoarticular infection caused by lt i gt Histoplasma capsulatum lt i gt is rare in Rheumatoid Arthritis (RA) making its diagnosis difficult. In the immunocompetent individuals this infection is autolimited or localized, while in immunodepressed patients the infection may be disseminated, and represents the reactivation of latent focuses or exogenous acquisition. Fungemia occurs in 20% of the cases; bones and joints are involved in 15%, being the spine the most common site of infection. We describe a clinical case of a woman with RA and spondylodiscitis caused by Histoplasma capsulatum with a

[MR imaging in the diagnosis of intradural extramedullary tuberculoma. Report of a case and review of the literature].

Intradural extramedullary tuberculoma without any bony involvement is exceptional. Nineteen cases were found in the literature. We report the case of 5-year-old boy who presented acute paraplegia 8 months after a tuberculous meningitis under antituberculous treatment. Dorsal gadolinium enhanced MRI revealed an intradural extramedullary bunch-shaped enhancing mass at T5-T6 level. The tuberculous nature was confirmed by histology after surgical removal of the lesion followed by a good recovery. To our knowledge, this is the first intradural extramedullary tuberculoma detected by MRI. We underlin
