In adults, tissue Doppler myocardial velocity imaging (TDI) is a recommended component of routine echocardiography and particularly useful to assess diastolic function of the left ventricle. In contrast, color and pulsed-wave TDI velocities are less accepted in pediatrics, perhaps due to their strong age dependence in children. This review discusses the strengths and limitations of TDI velocity imaging in the pediatric age group. In this report we describe the surgical treatment of a 27-year-old patient with complete Cantrell's syndrome, i.e. multiple ventricular septal defect, left ventricular diverticulum, dextrorotation of the heart, an anterior diaphragmatic defect, and a midline supraumbilical abdominal wall defect with tetralogy of Fallot. A case history of treatment with verapamil for verapamil-sensitive ventricular tachycardia with a left bundle branch block pattern is reported. Verapamil suppresses and prevents supraventricular tachycardias. Idiopathic ventricular tachycardias of the left ventricular outflow tract origin are verapamil sensitive.
