[Correction of vestibular disorders in patients with otitis media purulenta chronica complicated by labyrinthitis].

A vestibulogic examination was made in 66 patients with otitis media purulenta chronica complicated with labyrinthitis. The patients were divided into two groups: group 1 received surgical treatment in an acute period of labyrinthitis, group 2 was treated conservatively with delayed operation. The patients were examined at admission, 2 weeks, 1, 3, 6 and 12 months after the treatment. It was found that compensation of the vestibular function in group 1 took place 6 months after temporal bone surgery, in group 2-9 months after delayed operation, i.e. 1 years after hospitalization.

Functional reach: does it really measure dynamic balance?

BACKGROUND: Functional reach (FR) is a new clinical measurement intended to assess dynamic balance. The purposes of this study were (1) to measure the mean FR distance in healthy elders compared with individuals with known balance impairments, (2) to analyze the extent to which FR measures dynamic balance, and (3) to describe movement strategies used during FR. METHODS: Thirteen healthy elders and 15 individuals with vestibular hypofunction (VH) were tested during FR and free gait. Whole body kinematic and kinetic data including the center of gravity (CG) and center of pressure (CP) using 11

Localization and prevalence of hydrops formation in Ménière's disease using a test battery.

This study investigated the localization and prevalence of hydrops formation in 20 patients with unilateral Ménière's disease using a battery of tests, including audiometry, caloric, ocular vestibular evoked myogenic potential (oVEMP) and cervical VEMP (cVEMP) tests. The latter two tests used air-conducted sound (ACS) and bone-conducted vibration (BCV) as stimuli. Thirteen patients (65%) had abnormal hearing with a 4-tone average >26 dBHL. In the caloric test, 4 patients (20%) had abnormal responses, including canal paresis in 3 and caloric areflexia in 1 patient. The percentages of patients w

Effects of endolymphatic sac decompression surgery on endolymphatic hydrops.

CONCLUSIONS: The present findings suggest that complete control of vertigo after endolymphatic sac decompression surgery (ESDS) does not always depend on improved vestibular function or reduced endolymphatic hydrops. Vertigo control is, however, associated with hearing stability. OBJECTIVE: Among surgical treatments for intractable Meniere's disease, ESDS is performed to preserve and improve inner ear function. We examined the correlation between changes in vertigo frequency and neuro-otologic function to understand the condition of the inner ear in patients whose vertigo was completely contr

Delayed facial nerve palsy after vestibular schwannoma resection: risk factors, extent and prognosis.

INTRODUCTION: Facial nerve palsies may develop during the postoperative period of microsurgical removal of vestibular schwannomas (VSs), even after normal facial function for days or weeks after surgery. The aim of this study was to identify the pathomechanism and predictive factors of delayed palsy. MATERIAL AND METHOD: The clinical data of 193 patients who underwent vestibular schwannoma surgery between 2012 and 2021 were retrospectively analyzed. A total of 134 patients were included. The patients showed intact facial nerve function up to 24 h after surgery. All patients (n = 20) with pals

Vestibular dysfunction in migraine: effects of associated vertigo and motion sickness.

The mechanisms of vestibular migraine and motion sickness remain unknown. The aims of this study were to determine interictal vestibular dysfunction in migraineurs according to associated dizziness/vertigo and motion sickness, and to find out whether impaired uvulonodular inhibition over the vestibular system underlies the vestibular symptoms and signs by measuring tilt suppression of the vestibulo-ocular reflex (VOR). One hundred and thirty-one patients with migraine [65 with vestibular migraine (MV), 41 with migrainous dizziness (MD), and 25 with migraine only (MO)] and 50 normal controls un

Recovery of the vestibular function after vestibular neuronitis.

We performed steroid therapy on 34 cases with vestibular neuronitis and compared them with 77 patients not subjected to this therapy to examine the role of recovery of their vestibular function. Since no relation was noted between use of steroid and changes in subjective symptom of dizziness, the use of steroid is likely to facilitate the disappearance of spontaneous nystagmus in the early recovery stage. Canal paralysis recovered significantly by steroid and in cases of slight and moderate paralysis at the onset, recovery was more significant. Steroid therapy is argued to be effective for the

Effects of adaptation of vestibulo-ocular reflex function on manual target localization.

The goal of the present study was to determine if adaptive modulation of vestibulo-ocular reflex (VOR) function is associated with commensurate alterations in manual target localization. To measure the effects of adapted VOR on manual responses we developed the Vestibular-Contingent Pointing Test (VCP). In the VCP test, subjects pointed to a remembered target following passive whole body rotation in the dark. In the first experiment, subjects performed VCP before and after wearing 0.5X minifying lenses that adaptively attenuate horizontal VOR gain. Results showed that adaptive reduction in hor

Sacculo-collic response in otosclerosis and following successful stapes surgery.

INTRODUCTION: The sacculo-collic reflex is believed to be a short latency, otolith-mediated myogenic response to sound. With the application of air-conducted sound, one would expect an absent response in stapes fixation, as a fixed stapes footplate will not transmit a pressure wave to the saccule. METHODS: Fifty patients (70 stapes surgery ears, 26 otosclerotic ears and four normal ears) and 40 controls underwent repeated sacculo-collic tests. RESULTS: The results support the proposed mechanism for the sacculo-collic response. The study also suggests that, whilst stapedotomy piston prosthese

Subjective visual vertical in pitch and roll in right hemispheric stroke.

BACKGROUND AND PURPOSE: Patients with right hemispheric stroke usually present an anticlockwise deviation of the subjective visual vertical (SVV) in the frontal (roll) plane. However, the occurrence of a similar disorder in the sagittal (pitch) plane has never been assessed. We investigated the subjective visual vertical in both planes in those patients. METHODS: Eight patients, 4 with spatial neglect (N+) and 4 without neglect (N-), were compared with 4 healthy participants (C). They sat facing a luminous bar adjustable in rotation, either in the roll or in the pitch plane, and had to orient

Gamma knife radiosurgery for vestibular schwannomas.

The purpose of this study was to analyze tumor control and possible complications of gamma knife radiosurgery (GKRS) in patients with vestibular schwannomas using low marginal doses and conformal multiple shots to fit irregular tumor shapes. The authors evaluated 152 patients with more than 5 years of follow-up. Marginal doses were 9-15 Gy (median 12 Gy), with corresponding treatment volumes ranging from 0.1 to 18.7 cm3 (median 2.0 cm3). The number of isocenters varied from 2 to 24 shots (median 9 shots). The actuarial tumor control rates were 94% at 5 years and 92.4% at 8 years. Larger tumors

Frequency Amplitude Ratio of Ocular Vestibular Evoked Myogenic Potential in Different Age Groups.

BACKGROUND: High levels of sound intensity can be detected by the vestibular system. It is a clinical method used to assess the vestibular system's response to sound stimulation. In healthy individuals, research has indicated that the use of low-frequency stimuli results in decreased vestibular evoked myogenic potential (VEMP) thresholds, especially a 500 Hz tone-burst. As people age, their frequency tuning changes, moving towards higher frequencies. METHODS: Eighty healthy participants participated in this cross-sectional investigation. Both air conduction (AC) and bone conduction (BC) were

Cerebellar ataxia, neuropathy, vestibular areflexia syndrome (CANVAS): a review of the clinical features and video-oculographic diagnosis.

The association of bilateral vestibulopathy with cerebellar ataxia was first reported in 1991 and delineated as a distinct syndrome with a characteristic and measurable clinical sign--an absent visually enhanced vestibulo-ocular reflex--in 2004. We reviewed 27 patients with this syndrome and show that a non-length-dependent sensory deficit with absent sensory nerve action potentials is an integral component of this syndrome, which we now call "cerebellar ataxia with neuropathy and bilateral vestibular areflexia syndrome" (CANVAS). All patients had brain MRI and 22/27 had evidence of cerebellar

The results of vestibular evoked myogenic potentials, with consideration of age-related changes, in vestibular neuritis, benign paroxysmal positional vertigo, and Meniere's disease.

CONCLUSION: We interpreted VEMP findings in patients with the three major peripheral vertigo diseases, taking age-related changes into consideration. We found different abnormal VEMP rates among the three diseases, as well as differences in the proportion of parameters that were abnormal, according to the type of disease. OBJECTIVES: Vestibular neuritis, benign paroxysmal positional vertigo (BPPV), and Meniere's disease, common diseases that cause peripheral vertigo, often affect the saccule or inferior vestibular nerve, which are pathways of vestibular evoked myogenic potential (VEMP). Also,
