Obstructive sleep apnea in 2-6 year old children referred for adenotonsillectomy.

PURPOSE: Adenotonsillectomy is one of the most common surgical procedures performed in children. The indications for surgery are either frequent recurrent throat infections or hypertrophy of the tonsils/adenoid vegetation, which can cause obstructive sleep apnea (OSA). There is disagreement regarding the need for sleep studies before adenotonsillectomy to confirm a diagnosis of OSA. Several studies have evaluated questionnaires and physical examination as tools to identify OSA, with conflicting results. The aim of this study was to evaluate the prevalence of OSA among children referred for ade

Adenotonsillectomy for childhood obstructive sleep apnoea reduces thoraco-abdominal asynchrony but spontaneous apnoea-hypopnoea index normalisation does not.

The efficacy of adenotonsillectomy for treating obstructive sleep apnoea syndrome (OSAS) in children has been established, but its precise effects on inspiratory effort are not well documented. In 353 children enrolled in the Childhood Adenotonsillectomy Trial, randomised to undergo either early adenotonsillectomy (n=182) or a strategy of watchful waiting with supportive care (WWSC) (n=171), thoraco-abdominal asynchrony (TAA) was analysed during quiet, non-apnoeic and non-hypopnoeic breathing during sleep at baseline and at 7 months using overnight polysomnography.Children who underwent early a

Obstructive sleep apnea in obese children and adolescents, treatment methods and outcome of treatment - A systematic review.

OBJECTIVES: To systematically review and discuss the outcome of treating obstructive sleep apnea (OSA) in obese children and adolescents. METHODS: In February 2016 Pub Med was searched using a predetermined string to retrieve all relevant articles. The search identified 518 publications. In total 16 articles were included for review using the selected inclusion and exclusion criteria. The PRISMA guidelines was used. RESULTS: OSA was significantly more likely to persist in obese children after adenotonsillectomy. The prevalence of persistent OSA ranged from 33 to 76% in obese children and fro

Upper airway obstruction and resultant growth factors influencing malocclusions.

Upper airway obstruction is associated with many dental and skeletal malocclusions. Recognizing and removing the problems at an early age is encouraged. However, the malocclusions caused by upper airway obstruction can be addressed and corrected at different stages of dentitions using various types of appliances and orthodontic techniques. This article reports several cases, which were treated with the help of medical experts and otolaryngologists.

Role of paediatric intensive care following adenotonsillectomy for severe obstructive sleep apnoea: criteria for elective admission.

AIMS: This study aimed to critically review our criteria for elective admission to the paediatric intensive care unit following adenotonsillectomy for obstructive sleep apnoea. MATERIALS AND METHODS: We reviewed 122 children electively admitted between 1997 and 2011. During this time, our criteria for admission evolved. RESULTS: In these 122 children, the respiratory disturbance index during rapid eye movement sleep ranged from 6 to 159 (mean, 83). Forty-one per cent of the children had a recognised co-morbidity. Nine children required extra intervention, i.e. in addition to re-positioning a

Detection of Helicobacter pylori in adenotonsiller tissue specimens by rapid urease test and polymerase chain reaction.

In recent studies, there have been many arguments concerning Helicobacter pylori being reservoir in adenotonsillar tissue. In this study, our objective was to detect whether adenoid and/or tonsillar tissue of patients diagnosed with chronic adenotonsillitis was a reservoir for H. pylori. This study was performed with 47 patients with the diagnosis of chronic tonsillitits and adenoid hypertrophy. Helicobacter pylori was searched by rapid urease test (RUT) and polymerase chain reaction (PCR). Presence of H. pylori glmM gene (formerly named as ureC gene) was tested using ureC and ureC2 primers. Assessing the impact adenotonsilectomy has on the lives of children with hypertrophy of palatine and pharyngeal tonsils.

UNLABELLED: The hypertrophy of the palatine and pharyngeal tonsils is extremely common in children, being one of the most frequent causes of visits to otolaryngologists and such problem can impair the child's quality of life. AIM: to evaluate the impact of adenotonsillectomy on the lives of children with hypertrophied tonsils. MATERIALS AND METHODS: Contemporary longitudinal cohort study. A specific questionnaire used to evaluate the quality of life - OSD-6, was given to seventy five parents or guardians of children previously submitted to adenotonsillectomy, before the surgery and thirty da

Tonsil surgery in youths: good results with a less invasive method.

OBJECTIVE: Comparison of two types of tonsil surgery for 16- to 25-year-old patients, with respect to primary morbidity, snoring, and recurrent infections after 1 year. Teenagers and young adults are a significant proportion (26%) of the population that receive tonsil surgery each year and appear to suffer more pain than younger children. Recurrent tonsillitis, in combination with obstructive problems, is the main indication for surgery. METHOD: One hundred fourteen patients 16 to 25 years of age were randomized to tonsillotomy (TT) with radiosurgery (RF) (Ellman International) or to cold ton

Comparison of ondansetron and prochlorperazine for the prevention of nausea and vomiting after adenotonsillectomy.

This study has compared the incidences of nausea, vomiting and headache after ondansetron 0.06 mg kg-1 i.v., prochlorperazine 0.2 mg kg-1 i.m. and prochlorperazine 0.1 mg kg-1 i.v. given during induction of general anaesthesia to 282 patients undergoing adenotonsillectomy. The cardiovascular effects of the drugs were similar. After operation, nausea per se and vomiting per se occurred with similar frequency, in between 6% and 11% and 11% and 19%, respectively, in each test group. Nausea and vomiting in the same patient was reduced from 29% to 2% by i.v. ondansetron (P < 0.0005) and to 3% by i.

Surgery for glue ear--a modern epidemic.

Analysis of routine hospital statistics confirms anecdotal reports of an epidemic of surgery for glue ear. The surgical rate rose by 74% in Oxford Region between 1975 and 1981. The peak ages for surgery are 5-7 years, and the rates are higher in boys than in girls (1.4:1.00). The relative risk is high in social class I (1.16) and low in social class V (0.61). Although the prevalence of glue ear is hard to ascertain, the current surgical epidemic probably reflects an increase in apparent prevalence rather than a real increase in morbidity.

BLOOD LOSS DURING PEDIATRIC OPERATIONS.

The gravimetric method of measuring blood loss was used during all types of pediatric operations at The Montreal Children's Hospital. Results of 1787 such measurements indicated that the method is of value in the management of transfusion. Grading blood loss as minor, moderate or severe, in terms of its relation to total blood volume or body weight is a procedure of practical value. Operations were grouped into those that usually led to a loss of less than 10% of the blood volume (7.5 ml./kg.), those with a loss usually between 10% and 14% and those with blood loss usually over 14% (10.5 ml./k

The interest of ketamine as an adjuvant to fentanyl in post-tonsillectomy analgesia in children: a randomized controlled trial.

INTRODUCTION: tonsillectomy is the most commonly performed surgery in children. It is a painful surgery, which is often an ordeal for both children and their parents. The study aimed to evaluate the effects of ketamine used instead of or as an adjuvant to fentanyl on early postoperative pain scores in children undergoing tonsillectomy. METHODS: we conducted a double-blind prospective randomized study including 60 children, aged between 2 and 7 years, scheduled to undergo adenotonsillectomy. Patients were randomly assigned to one of three groups: group G1 received 2 μg/kg of fentanyl, group G2

Rationale for and design of the "POSTA" study: Evaluation of neurocognitive outcomes after immediate adenotonsillectomy compared to watchful waiting in preschool children.

BACKGROUND: IQ deficits are linked to even mild obstructive sleep apnoea (OSA) in children. Although OSA is commonly first diagnosed in the pre-school age group, a randomised trial is still needed to assess IQ outcomes after adenotonsillectomy in the pre-school age-group. This randomised control trial (RCT) will primarily determine whether adenotonsillectomy improves IQ compared to no adenotonsillectomy after 12 months, in preschool (3-5 year-old) children with mild to moderate OSA. METHODS: This protocol is for an ongoing multi-centred RCT with a recruitment target of 210 subjects (105 in ea

Palatal eversion for the treatment of combined nasopharyngeal stenosis and tonsillar pillars adhesion.

OBJECTIVE: Rarely the tonsillar pillars and the soft palate became adherent to the posterior nasopharyngeal wall by strong fibrous tissue due to excessive dissection and cauterization during surgery leading to nasopharyngeal stenosis. Therefore, many treatment modalities are being tried to cure this problem. The aim of this study is to explore our results of modifying the basic technique to accommodate those patients with combined nasopharyngeal stenosis and tonsillar pillars adhesions in one stage. STUDY DESIGN: Case series. METHODS: This study was conducted on 10 patients with combined nas

Do respiratory cycle-related EEG changes or arousals from sleep predict neurobehavioral deficits and response to adenotonsillectomy in children?

STUDY OBJECTIVES: Pediatric obstructive sleep apnea (OSA) is associated with hyperactive behavior, cognitive deficits, psychiatric morbidity, and sleepiness, but objective polysomnographic measures of OSA presence or severity among children scheduled for adenotonsillectomy have not explained why. To assess whether sleep fragmentation might explain neurobehavioral outcomes, we prospectively assessed the predictive value of standard arousals and also respiratory cycle-related EEG changes (RCREC), thought to reflect inspiratory microarousals. METHODS: Washtenaw County Adenotonsillectomy Cohort I
