Chikungunya associated acute necrotizing encephalopathy in an adolescent: Expanding the neurological spectrum of the virus.

Acute necrotizing encephalopathy (ANE) is a rare and severe neurological disorder characterized by rapid onset encephalopathy following a viral febrile illness. A 12-year-old adolescent girl presented with fever, joint pain, and myalgia for 3 days, followed by the development of multiple convulsions and altered sensorium. Cerebrospinal fluid analysis was non-specific. Contrast-enhanced magnetic resonance imaging of the brain revealed two well-defined, bilaterally symmetrical, round-to-oval hyperintense lesions on T2W, and FLAIR sequences, representing acute necrotizing encephalopathy. Polymera

Supporting qualitative practitioner research in child and adolescent mental health.

This editorial piece addresses the relationship between clinical practice and qualitative research in child and adolescent mental health. We outline some guiding assumptions informing the development of a practice orientated research 'lab' which focusses on child and adolescent mental health and child welfare research with ethnographic and psychosocial methodologies. We consider cascading effects of practitioner-initiated research, where skills and ambitions for a 'bottom up' research culture can help professionals embed research-minded practice in services. We also address the role of researc

EXT2 transition in adolescent membranous nephritis.

Membranous nephropathy (MN) is the cause of 3% of pediatric nephrotic syndrome, with increasing incidence in adolescents. It was historically divided into primary and secondary forms but is increasingly described by antigen. The direct clinical value of knowing the MN antigen often depends on the strength of association between antigen and various underlying conditions, prognostic potential, and the presence of commercially available serum antibody testing. In this case, we describe an adolescent with PLA2R-, NELL1-, THSD7A-, and EXT2-negative MN who responded to B-cell depleting therapy. Thre

Adolescent alcohol exposure modifies adult anxiety-like behavior and amygdala sensitivity to alcohol in rats: Increased c-Fos activity and sex-dependent microRNA-182 expression.

Adolescent binge alcohol drinking is a serious health concern contributing to adult alcohol abuse often associated with anxiety disorders. We have used adolescent intermittent ethanol (AIE) administration as a model of binge drinking in rats in order to explore its long-term effect on the basolateral amygdala (BLA) responsiveness to alcohol and anxiety-like behavior. AIE increased the number of BLA c-Fos positive cells in adult Wistar rats and anxiety-like behavior assessed by the open field test (OFT). Additionally, in adult female rats receiving AIE BLA over expression of miR-182 was found.

Parental Knowledge is a Contextual Amplifier of Associations of Pubertal Maturation and Substance Use.

Earlier pubertal development and less parental knowledge have been linked to more substance use during adolescence. The present study examines interactions between pubertal timing and tempo and parental knowledge (children's disclosure, parental control, and parental solicitation) for adolescent substance initiation. Data are from a northeastern US-based cohort-sequential study examining 1023 youth (52 % female) semiannually for up to 6 assessments (ages 10.5-19 years). The findings supported the hypothesis that lower knowledge is a contextual amplifier of early timing-substance use associatio

Sibling Conversations about Dating and Sexuality: Sisters as Confidants, Sources of Support, and Mentors.

Using an observational methodology to examine sibling communication, sisters (N = 28 dyads) were videotaped discussing their ideas about dating and sexuality. Social provision theory was used as a framework for the examination of roles enacted by sisters during these conversations. Inductive thematic analytic procedures were conducted and three roles were identified: sisters as confidants, sources of support, and mentors. Older and younger sisters both served as confidants and sources of support for one another, whereas, older sisters were more likely to be mentors for their younger sisters th

Children's behavior and physiology and how it affects exposure to environmental contaminants.

Infant, child, and adolescent exposures to environmental toxicants are different from those of adults because of differences in behavior and physiology. Because of these differences, there is the potential for quantitatively different exposures at various stages of development. Pediatricians are well aware of these behavioral and physiologic differences from a clinical standpoint--namely, food and water intake, soil ingestion, mouthing behavior, inhalation physiology, and activity level--as they relate to the ratio of these parameters between the adult and the child when considering weight and

Dissociative symptomatology and traumatogenic factors in adolescent psychiatric patients.

This study describes the relation of different types of childhood trauma to the degree of dissociative experiences. Subjects were 198 consecutively admitted adolescent psychiatric patients, 11 to 19 years old (89 inpatients and 109 outpatients). All patients completed the Adolescent Dissociative Experiences Scale. A Checklist of Traumatic Childhood Events was filled out by clinicians. The results showed an increase in the degree of dissociative experiences in patients with a history of sexual abuse, physical abuse, neglect, and stressful life events. With the exception of life events, a modera

Feeling states during adolescence.

Adolescence is characterized by a wide variety of feeling states with certain stages in adolescence being associated with particularly strong affects, a result of the normative developmental process. Affective psychopathology during the adolescent years, particularly in the disorders of depression and mania/hypomania, is characterized by abnormalities in the quality and quantity of affective patterns and additionally by the appearance of feeling states, which may be historically uncharacteristic for the teen (i.e., out of keeping with his previous longitudinal pattern of personality developmen

Changes in growth hormone (GH) secretion and in growth during puberty.

During normal adolescent development in males testosterone induces the adolescent growth spurt, at least in part, by increasing GH production via an increase in the amplitude of the peaks of GH which are released, and not by increasing the frequency of GH pulses. Testosterone and estrogen administration at low or modest doses to individuals with the capacity to produce GH causes GH production and IGF-I levels to increase. Testosterone given to GHD patients does not increase either of these factors. In addition, there may be two actions for growth promotion by testosterone. One unequivocally re…

Effect of sub-elite competitive running on bone density, body composition and sexual maturity of adolescent females.

The attainment of optimal peak bone mass during adolescence is important in the primary prevention of osteoporosis. Exercise may contribute to skeletal development and bone density during growth, although competitive exercise is suggested to have an adverse effect. This study assesses the effect of moderate exercise on the bone density of adolescent females. Additionally, other factors which significantly influence attainment of peak bone mass were identified. This was a cross-sectional study of 42 adolescent females, classified as runners (n=15) or non-runners (n=27). Nutrient intake, energy

Risk and protective factors associated with substance use disorders in adolescents with first-episode mania.

OBJECTIVE: Adolescents with bipolar disorder (BD) are more likely to develop substance use disorders (SUDs) than adolescents without psychiatric disorders; however, to our knowledge, specific risk factors underlying this relationship have not been prospectively examined. The purpose of this study was to identify predictors of developing SUDs after a first manic episode. METHOD: Participants aged 12 to 20 years and hospitalized with their first manic episode associated with bipolar I disorder (BP-I) were recruited as part of the University of Cincinnati First-Episode Mania Study and prospectiv

Morphine exposure during adolescence induces enduring social changes dependent on adolescent stage of exposure, sex, and social test.

Drug exposure during adolescence, when the "reward" circuitry of the brain is developing, can permanently impact reward-related behavior into adulthood. Epidemiological studies show that opioid treatment during adolescence, such as pain management for a dental procedure or surgery, increases the incidence of psychiatric illness including substance use disorders. Moreover, the opioid epidemic currently in the United States is affecting younger individuals raising the impetus to understand the pathogenesis of the negative effects of opioids. One reward-related behavior that develops during adole

Body, image, and digital technology in adolescence and contemporary youth culture.

The physical, psychological and social changes that occur during adolescence constitute a physiological crisis that is necessary for development and growth. The establishment of a suitable "self-image" is important for facilitating harmonious psychophysical development during this time. In the current era, digital technology (DT) serves as an extraordinary means of communication for young people, who make significant use of images as a mode of expression. Accordingly, there is growing interest in the relationship between physical development, self-image and use of DT. A review of the published

The production of scoliosis after pinealectomy in young chickens, rats, and hamsters.

STUDY DESIGN: This study involved weekly radiographic examination of pinealectomized rats, hamsters, and chickens to observe the development of scoliosis. OBJECTIVES: To determine whether pinealectomy produces scoliosis in animals more closely related phylogenetically to humans than to chickens, namely rats and hamsters, which are representative of mammals. SUMMARY OF BACKGROUND DATA: Pinealectomy in 3-day-old chickens has consistently resulted in the development of scoliosis with many characteristics similar to those seen in patients with adolescent idiopathic scoliosis. It has not been det

Disorders of Sex Development in Office Practice.

Disorders of sex development (DSD) is a broad term for congenital conditions with a discrepancy in chromosomal, gonadal, or anatomic sex. Pediatricians are often faced with the challenge of managing a newborn/infant with atypical genitalia or an older child with disordered puberty, which come under the purview of DSD. This article provides an update for pediatricians on comprehensive approach to DSD with a focus on atypical genitalia.

Ovotesticular Disorder of Sex Development Presenting as a Scrotal Emergency.

Ovotesticular (OT) disorder of sex development (DSD) is a rare condition that affects the development of reproductive organs and manifests in a wide range of phenotypic presentations. The clinical diagnosis of this condition is challenging because of its atypical nature, and the variability of presentation in 46,XX OT-DSD cases makes it a complex issue in medical practice. We report a case of a 13-year-old boy who presented with left scrotal pain. Further exploration revealed a tunica rupture without testicular torsion of the left testis, whereas the histopathological analysis of a nodule exci

Ossicula Bertini in human adults.

Remnants of ossicula Bertini in their embryological position, i.e., joined to the ethmoid, were found in three out of 20 adult individuals aged 17-23 years. The ossicles closed the inferior and/or a part of the anterior wall of the sphenoidal sinus, being joined to the wall of the sinus by sutures. The existence of these formations might be of importance in inflammation of the sphenoid sinus, or disease in or around the nasopharynx.

Pubertal development is unaffected in female Mecp2 -heterozygous mice.

Rett syndrome (RTT) affects predominantly females and is characterized by regression after an initial period of typical development. Previously, we showed that while adolescent female Mecp2 -heterozygous mice (Het, mouse model of RTT) perform pup retrieval comparably to wild-type littermate controls (WT), adult Het regress and become inefficient. Here, we analyzed the pubertal trajectory of naïve nulliparous WT and Het to test whether abnormal pubertal development contributes to this regression phenotype. We found no genotypic differences in pubertal onset nor estrous cyclicity during adolesce

Chlamydia trachomatis screening in the adolescent population.

Advances in the development of highly sensitive tests for Chlamydia trachomatis offer new options for screening and testing in adolescents. The availability of nucleic acid amplification technologies may make noninvasive urine testing available for young men and for young women when a gynecologic examination is not otherwise required. Accurate detection of asymptomatic chlamydial disease in a timely, cost-effective, and noninvasive manner as well as development of effective partner treatment strategies remain important challenges. This review provides a clinical update on office-based testing
