Non-pharmacologic management of atrial fibrillation.

Atrial fibrillation (AF) is the most common arrhythmia encountered in clinical practice today. Contemporary medical treatment options include atrioventricular nodal blocking agents to control heart rates during AF, antiarrhythmic drugs aimed at maintaining normal sinus rhythm, and anticoagulation therapies to reduce stroke risk. Invasive treatment of AF has emerged because of the toxicities and lack of long-term efficacy of available antiarrhythmic medications along with the lack of improvement in symptoms for rate-controlled patients. The investigators review the evolution of the current cath

Complete heart block in late presentation of inferior STEMI successfully treated with percutaneous coronary intervention.

A 55-year-old female presented with 4-day history of fatigue and exertional shortness of breath. A late presentation inferior ST elevation myocardial infarction (STEMI) was diagnosed based on ST elevation in the inferior leads of electrocardiography and elevated cardiac troponin T (TnT). She developed complete heart block 1 day after admission to the hospital and remained hemodynamically stable. She was taken to the catheterization laboratory for a temporary pacing wire insertion. Coronary angiogram at the same time showed an occluded right coronary artery at the mid-section. The lesion was su

Periostin regulates atrioventricular valve maturation.

Cardiac valve leaflets develop from rudimentary structures termed endocardial cushions. These pre-valve tissues arise from a complex interplay of signals between the myocardium and endocardium whereby secreted cues induce the endothelial cells to transform into migratory mesenchyme through an endothelial to mesenchymal transformation (EMT). Even though much is currently known regarding the initial EMT process, the mechanisms by which these undifferentiated cushion mesenchymal tissues are remodeled "post-EMT" into mature fibrous valve leaflets remains one of the major, unsolved questions in hea

Mechanism of cardiac output gain from cardiac resynchronization therapy in patients with coronary artery disease or idiopathic dilated cardiomyopathy.

The mechanisms underlying cardiac resynchronization therapy have consistently been studied at rest and remain ill defined. Peak stress total isovolumic time (t-IVT) is a major determinant of cardiac output (CO) in chronic heart failure. In this study, pharmacologic stress was used to assess the effects of atrioventricular (AV) delay shortening and ventricular resynchronization elements of cardiac resynchronization therapy. Thirty patients undergoing cardiac resynchronization therapy were studied <6 months after implantation. t-IVT and CO were measured during native activation (left bundle bran

Effect of acute cocaine administration on the QTc interval of habitual users.

The proarrhythmic effects of cocaine may be mediated in part by its effects on cardiac repolarization properties. This study evaluated the acute effects of smoking cocaine 25 mg on the electrocardiograms of 14 habitual cocaine users during a 12-minute observation period. After cocaine administration, heart rate increased by a mean of 22 beats/min (p <0.0001). One patient developed accelerated junctional rhythm, and 5 had nonspecific ST-T-wave abnormalities. The electrocardiograms revealed significant prolongation of the QTc interval (p <0.001) after cocaine administration. In addition, T-wave

Variability of AV nodal potentials recorded, in vivo: direct demonstration of dual AV nodal physiology.

OBJECTIVES OF STUDY: We developed a method to record extracellular A-V nodal potentials in the beating dog heart, in vivo. METHODS: In eleven Na-pentobarbital anesthetized, open-chest dogs, an octapolar electrode catheter (2 mm rings, 2 mm spacing) was inserted through a purse-string suture in the coronary sinus (CS) distal to the ostium and positioned electrographically so that the tip electrode recorded a His bundle (Hb) potential. RESULTS: Stable recordings of A-V nodal potentials (amplitude, 178 +/- 94 microV; duration 78 +/- 26 msec) were consistently made during sinus rhythm from the s

Acute comparative effect of right and left ventricular pacing in patients with permanent atrial fibrillation.

We tested the hypothesis that left ventricular (LV) pacing is superior to right ventricular (RV) apical pacing in patients undergoing atrioventricular (AV) junction ablation and pacing for permanent atrial fibrillation. The potential benefit of LV over RV pacing needs to be evaluated without the confounding effect of other variables that can influence cardiac performance. An acute intrapatient comparison of the QRS width and echocardiographic parameters between RV versus LV pacing was performed within 24 h after ablation in 44 patients. Both modes of pacing were also compared with pre-implanta

Catheter ablation of atriofascicular Mahaim fibers guided by the activation potential.

OBJECTIVE: To determine whether recording of the activation potential may be used as an isolated criterion to guide catheter ablation of atriofascicular Mahaim fibers. METHODS: We studied 6 patients (5 females, mean age of 26 +/- 7.3 years) with paroxysmal tachycardias with a wide QRS complex, whose electrophysiological study diagnosed atriofascicular Mahaim fibers. Mapping and catheter ablation were performed in sinus rhythm, guided only by the recording of the activation potential of the fiber. RESULTS: Efficacy in ablation was achieved in all patients. The fibers were located in the right

Progression of permanent atrial fibrillation after atrioventricular junction ablation and dual-chamber pacemaker implantation in patients with paroxysmal atrial tachyarrhythmias.

Among 63 patients affected by symptomatic drug refractory paroxysmal atrial fibrillation who had undergone atrioventricular junction ablation and dual-chamber pacemaker implantation, the actuarial estimate of progression of permanent atrial fibrillation was 22%, 40%, and 56% respectively, 1, 2, and 3 years after ablation. A stratification of the risk of development of permanent atrial fibrillation was obtained on the basis of several clinical variables.

Progressive anterior ablation in the coronary sinus region: evidence to support the presence of a 'slow pathway' input in normal patients?

BACKGROUND: AV node modification is an emerging approach to rate control in patients with medically refractory atrial fibrillation. The mechanism of benefit of this procedure is not completely understood. METHODS AND RESULTS: Twenty-two patients (age, 65+/-11 years; 16 women) with medically refractory paroxysmal atrial fibrillation referred for complete AV node ablation underwent serial ablations beginning at the level of the coronary sinus os progressing in a superior and anterior direction toward the His bundle. Serial atrial extrastimulus testing was performed to determine the effect of th

Radiofrequency catheter ablation of septal accessory atrioventricular pathways.

OBJECTIVE: Septal accessory atrioventricular pathways are recognised as being more difficult to ablate than pathways in other locations. This paper describes an experience of 48 consecutive patients with septal accessory pathways who had catheter ablation with radiofrequency current. PATIENTS AND METHODS: There were 28 male and 20 female patients, mean (SD) age 35 (17). 43 patients had a single accessory pathway and 5 patients had multiple accessory pathways. Pre-excitation was present in 37 patients, and 11 patients had concealed accessory pathways. 21 patients had had a previous electrophys

Evidence that cocaine slows cardiac conduction by an action on both AV nodal and His-Purkinje tissue in the dog.

The effects of intravenous cocaine (2 mg/kg) were tested on several indices of cardiac electrical activity in sedated dogs. These included sinus rate, PR, AH, and HV intervals; AV nodal effective refractory period (AVNERP); ventricular effective refractory period; QRS duration; and the QT interval. Cocaine induced significant changes in six control animals with an intact-functioning autonomic nervous systems. After pharmacologic autonomic blockade with propranolol plus propantheline, cocaine increased the PR interval (+ 11 +/- 4.0 ms, p less than 0.05), primarily by slowing conduction at the A

Response of tertiary centres to pressure changes. Is there a mechano-electrical association?

STUDY OBJECTIVE: To investigate the mechanism of pressure related ventricular arrhythmias by examining them during atrioventricular (AV) block. DESIGN: Complete AV block, where all ventricular beats are ectopic, was induced by AV node ablation and/or by toxic digitalisation, and rhythm changes were studied while arterial blood pressure was repeatedly raised and lowered. SUBJECTS: 15 anaesthetised mongrel dogs, weight 15-28 kg, were used. AV block was induced in eight by chemical or mechanical ablation of the AV node. In five of these and in seven other dogs, 5.0-7.5 mg digoxin was also given

Findings on ambulatory electrocardiographic monitoring in subjects older than 80 years.

Twenty-four-hour electrocardiograms were recorded in 50 subjects (44 women, 6 men) older than 80 years without cardiovascular disease and with normal standard electrocardiographic responses. During waking and sleeping periods, the mean sinus rates were, respectively, 78 +/- 3 and 64 +/- 1 beats/min; heart rate ranged from 43 to 180 beats/min over 24 hours. Supraventricular tachycardia (SVT) was present in 28% of the subjects. Nocturnal sinus arrhythmia was only noted in 12% of the patients; it was accompanied by sinus pauses of 1.8 to 2 seconds, and 1 woman had a transient pattern compatible w

Initiation of atrioventricular nodal reentrant tachycardia in patients with discontinuous anterograde atrioventricular nodal conduction curves with and without documented supraventricular tachycardia: observations on the role of a discontinuous retrograde conduction curve.

To evaluate factors playing a role in initiation of atrioventricular (AV) nodal reentrant tachycardia utilizing anterogradely a slow and retrogradely a fast conducting AV nodal pathway, 38 patients having no accessory pathways and showing discontinuous anterograde AV nodal conduction curves during atrial stimulation were studied. Twenty-two patients (group A) underwent an electrophysiologic investigation because of recurrent paroxysmal supraventricular tachycardia (SVT) that had been electrocardiographically documented before the study. Sixteen patients (group B) underwent the study because of

The morphology of ventricular septal defects.

Ventricular septal defect (VSD) is the most common congenital cardiac lesion, occurring either in isolation or in hearts containing more complex lesions. Usually, the defect is between two ventricles, each of which is connected to a separate atrium and to a separate great artery, and surgical or spontaneous closure of the VSD basically corrects the circulation if the effects of any associated lesion are ignored. In other situations, the VSD is an integral part of the circulation, either because the atria connect to only one ventricle or because the ventricles give rise to only one patent great

Dual accessory nodoventricular pathways: role in paroxysmal wide QRS reciprocating tachycardia.

Clinical electrophysiological study in an otherwise healthy 21-year-old male with paroxysmal wide QRS tachycardia (cycle length 300 ms, heart rate 200/min) suggested the presence of two nodoventricular (NV) bypass connections. The first NV connection pre-excited the base of the interventricular septum (NVs), as evidenced by a short HV interval during sinus rhythm (15-20 ms), with local ventricular activation occurring earlier at the septal base than at either the right ventricular apex or the base of the left ventricle. The second NV connection appeared to connect the AV junction with the righ

Catheter ablation of the atrioventricular conduction system in patients with supraventricular tachycardia.

In rare circumstances, patients with disabling supraventricular tachycardia that proves refractory to treatment with medication or antitachycardia pacing devices may require interruption of normal atrioventricular conduction and implantation of a permanent pacemaker for effective control of their arrhythmia. In the past, a cardiac surgical procedure was required to interrupt normal atrioventricular conduction; however, a closed-chest catheter technique to interrupt normal atrioventricular conduction has recently been described. We describe three patients in whom normal atrioventricular conduct
