Control of nausea and autonomic dysfunction with terfenadine, a peripherally acting antihistamine.

Terfenadine (Seldane) was administered to 14 male subjects in a randomized, double-blinded, and crossed-over design to assess the efficacy of this peripherally active antihistamine as an anti-motion sickness drug. Terfenadine possesses practically no central side effects. A Staircase Profile Test was administered 4 h following placebo or a single oral dose of terfenadine (300 mg). The study revealed a statistically significant therapeutic effect from terfenadine (p less than 0.05). This led us to conclude that because the drug does not or only poorly crosses the blood-brain barrier, a selectiv

Scopolamine blood levels following buccal versus ingested tablets.

Speed of absorption and elimination of an antimotion sickness drug sets limits on the protection afforded. The aim of this experiment was to determine whether a well proven antimotion sickness drug--scopolamine (hyoscine)--could be absorbed more rapidly from buccal tablets than from the standard issue ingested tablets. Plasma scopolamine levels were measured using a radioreceptor assay of repeated blood samples from 10 volunteers, each of whom took buccal and standard ingested tablets (both 0.6 mg scopolamine hydrobromide) on two different occasions, and from a further 8 volunteers following i

Visually-induced sickness in normal and bilaterally labyrinthine-defective subjects.

A group of nine normal subjects (with no overt vestibular dysfunction) and a group of 6 bilaterally labyrinthine-defective subjects were exposed to a visual field rotating about an Earth-horizontal axis (orthogonal to the gravity axis). The visual stimulus was provided by a 3-m diameter sphere with random dots rotating at 30, 45, and 60 degrees per second (degree/s) about the stationary subject's roll, pitch and yaw axes. The subject's head was positioned at the center of the sphere such that it experienced apparent motion in all three axes. Results indicated that in the normal group, symptoms

Altered sensorimotor control of the body as an etiological factor in space motion sickness.

Exposure to nonterrestrial force levels affects the activity of gravitoinertial force sensitive receptors of the body, both of labyrinthine and nonlabyrinthine origin. It also disrupts the normal patterning of motor control of body orientation and movement. The patterns and levels of muscle innervation necessary to achieve particular body configurations and to bring about particular body movements are greatly affected by background force level and body orientation relative to the force vector. The present studies demonstrate that such altered sensorimotor control of head and body posture along

Tumbling and spaceflight: the Gemini VIII experience.

A malfunctioning orbital flight attitude thruster during the flight of Gemini VIII led to acceleration forces on astronauts Neil Armstrong (commander) and David Scott (pilot) that created the potential for derogation of oculo-vestibular and eye-hand coordination effects. The spacecraft attained an axial tumbling rotation of 50 rpm and would have exceeded this had not the commander accurately diagnosed the problem and taken immediate corrective action. By the time counter-measure controls were applied, both astronauts were experiencing vertigo and the physiological effects of the tumbling accel

Threshold altitude resulting in decompression sickness.

A review of case reports, hypobaric chamber training data, and experimental evidence indicated that the threshold for incidence of Altitude Decompression Sickness (DCS) was influenced by various factors such as prior denitrogenation, exercise or rest and period of exposure, in addition to individual susceptibility. Fitting these data with appropriate statistical models has the potential for estimating the frequency of occurrence of DCS at various altitudes under different experimental conditions and allows us to examine the influence of various factors on the threshold for DCS. This approach w

Sustained peripheral vasoconstriction while working in continuous intense noise.

The study examined the effects of exposure to continuous noise on pulse volume (vasoconstriction) and rate, as well as four-choice serial reaction time. There were 2 separate groups of 12 subjects that performed the 20-min task during simultaneous monitoring of their physiological response to continuous 93 dBA white noise or 70 dBA quiet control conditions. Pulse volume showed marked reduction, implying increased arousal, in the first 3-min exposure to noise. Although some habituation of the vasoconstriction response then occurred, pulse volume continued to remain significantly higher than in

Acupressure and motion sickness.

The effectiveness of the "Sea Band" acupressure band compared with placebo and hyoscine (0.6 mg), also known as scopolamine, to increase tolerance to a laboratory nauseogenic cross-coupled motion challenge was assessed using 18 subjects. The results showed that the subjects had a significant increase in tolerance with hyoscine but had no increase in tolerance with the "Sea Band" or placebo. Possible reasons for the failure to show any significant protection from the use of these acupressure bands are insufficient movement of the wrist to provide continuous stimulation, and/or the likelihood th

The stability of individual patterns of autonomic responses to motion sickness stimulation.

As part of a program to develop a treatment for motion sickness based on self-regulation of autonomic nervous system (ANS) activity, this study examined the stability of an individual's pattern of ANS responses to motion sickness stimulation on repeated occasions. Motion sickness symptoms were induced in 58 people during 2 rotating chair tests. Physiological responses measured were heart rate, finger pulse volume, respiration rate, and skin conductance. Using standard scores, we examined stability of responses of specific magnitudes across both tests. Correlational analyses, analysis of varian

The electrocardiographic response to high +Gz centrifuge training.

The electrocardiographic (ECG) responses of 59 asymptomatic, healthy flight surgeons to the acceleration profiles included in current U.S. Air Force and U.S. Navy high-G centrifuge training programs are documented. ECG dysrhythmias were frequently observed during exposure to both gradual and rapid onset training profiles. Short self-limited episodes of ventricular tachycardia occurred in 5 subjects. Advanced Lown grade ventricular ectopy occurred in 13 subjects. The type of cardiac ectopy and the frequency of occurrence for each of the training profiles is described. The results suggest that s

Instability of ocular torsion in zero gravity: possible implications for space motion sickness.

Inherent asymmetries of the gravity-sensitive otolith organs of the inner ear may be well-compensated in ordinary 1 G, but rendered unstable in novel gravitational states. Several aspects of ocular counterrolling and spontaneous eye torsion, reflexes governed by the otoliths, were examined during the hypo- and hypergravity in parabolic flight on the NASA KC-135 aircraft. Among the subjects were two astronauts, one who had suffered space motion sickness during his mission and one who had not. Using an observed separation of scores of torsional instability at 0 G as the criterion, we divided our

Catecholaminergic responses to rotational stress in rat brain stem: implications for amphetamine therapy of motion sickness.

The fact that amphetamine, a noradrenaline releaser, prevents motion sickness leads the hypothesis of Wood and Graybiel that the noradrenergic neuron system in the brain stem acts against the development of motion sickness. To evaluate the hypothesis, the effects of rotational stress on the turnovers of noradrenaline and dopamine in the rat brain stem were examined. Rats were rotated about two axes simultaneously (double rotational) or about one axis (single rotation) for 60 min. Measurement of kaolin intake (pica) induced by rotation, as an index of motion sickness, showed that double rotatio

Use of phenytoin in the prevention of motion sickness.

In a placebo-controlled double-blind crossover pilot study of acute Coriolis-induced motion sickness treatment/prevention in humans employing an anticonvulsant dose of phenytoin, a mean increase in tolerance to motion stress from 4.87 min (S.D. = 5.55) to 46.87 min (S.D. = 32.6) was obtained. This represents a greater than fourfold improvement in efficacy over any currently available single agent and is more than twice as effective as the scopolamine/dexadrine combination. There were none of the usual side effects of blurred vision, dizziness, dry mouth, or sedation.

Atrial natriuretic factor increases vascular permeability.

An increase in central blood volume in microgravity may result in increased plasma levels of atrial natriuretic factor (ANF). Since elevations in plasma ANF are found in clinical syndromes associated with edema, and since space motion sickness induced by microgravity is associated with an increase in central blood volume and facial edema, we determined whether ANF increases capillary permeability to plasma protein. Conscious, bilaterally nephrectomized male rats were infused with either saline, ANF + saline, or hexamethonium + saline over 2 h following bolus injections of 125I-albumin and 14C-

Adverse effects of dietary and furosemide-induced sodium depletion on thermoregulation.

In this study, the diuretic furosemide was used in combination with dietary sodium (Na) restriction to quantify the effects of moderate to severe Na depletion on heat tolerance in a validated model of heat stress in rats. Rats were subjected to an Na depletion regimen as follows: a control group (I, n = 17) had free access to a normal diet and tap water; group II (n = 20) consumed the same normal diet and tap water, but was treated with the diuretic furosemide at a dose of 10 mg.kg-1.d-1, ip; group III (n = 18) had free access to an Na-free diet and deionized drinking water; group IV (n = 21)

Insidious hypothermia during raft use.

It is commonly thought that increasing protection against cold water results in greater physiological efficiency in maintaining core temperature. To examine the relationship of physiological changes resulting from cold exposures as a function of increased insulation, five subjects (4 males, 1 female) wearing anti-exposure ensembles covering the entire body and head were exposed in a pool to water temperatures (Twater) = 4.4 degrees C and air temperatures (Tair) = 5.6 degrees C. Trials consisted of subjects undergoing either head-out immersions with mean exposure time (t = 150 +/- 9 S.E.M. min)

The use of tympanometry in predicting otitic barotrauma.

Static acoustic impedance tympanometry was used to examine 80 subjects prior to and after exposure to decreased ambient pressure in a hypobaric chamber. The predictive value of tympanometry in detecting those individuals likely to suffer from otitic barotrauma (aerotitis media) was evaluated. The results suggest that testing prior to altitude exposure is of no value in identifying those individuals who will suffer from otitic barotrauma during flight. Tympanometry however proved a useful tool in confirming the presence of barotrauma following flight, but it was no more useful than taking a his

Navy helicopter pilot with a juxtasellar mass: case report with aeromedical considerations.

A Navy helicopter pilot was found to have a suprasellar mass during evaluation for primary infertility and mildly elevated prolactin level. Extensive evaluation revealed no other abnormalities. After 1 year of followup without radiographic tumor enlargement, he was returned to flying duties with continuing medical monitoring. Aeromedical considerations for tumors in the juxtasellar region are reviewed, including neuroendocrine disorders, neuro-ophthalmologic defects, and neurological impairment. The impact of improved diagnostic capabilities on aeromedical disposition is discussed.

Orthostatic responses following 30-day bed rest deconditioning with isotonic and isokinetic exercise training.

To determine if intensive isotonic or isokinetic exercise training during 30 d of -6 degrees head-down bed rest (BR) would accentuate orthostatic intolerance, 19 men (32-42 years) were divided into a no-exercise control group (N = 5), and isotonic (Quinton ergometer, N = 7) and isokinetic (Lido ergometer, N = 7) exercise groups. Training was two 30-min periods per day for 5 days per week.
