In vivo imaging of synaptic function in the central nervous system: II. Mental and affective disorders.

This review gives an overview of those in vivo imaging studies on synaptic neurotransmission, which so far have been performed on patients with mental and affective disorders. Thereby, the focus is on disease-related deficiencies within the functional entities of the dopaminergic, serotonergic, cholinergic, histaminergic, glutamatergic, or GABAergic synapse. So far, in vivo investigations have yielded rather inconsistent results on the dysfunctions of specific synaptic constituents in the pathophysiology of the diseases covered by this overview. Among the more congruent results are the finding

Using latent patient-centered fear extinction profiles to guide treatment in pediatric OCD.

BACKGROUND: While exposure with response prevention (ERP) is the frontline treatment for pediatric obsessive-compulsive disorder (OCD), not all patients benefit. Impairments in extinction learning are implicated in pediatric OCD's etiology and treatment, with impaired pre-treatment extinction learning associated with worse ERP outcomes. However, available methods for assessing patients' difficulties with fear extinction are costly and underdeveloped. In response, we used person-centered profiles to characterize latent fear extinction patterns among youth with OCD, and investigated the degree t

Individualized, connectome-based, non-invasive stimulation of OCD deep-brain targets: A proof-of-concept.

Treatment-resistant obsessive-compulsive disorder (OCD) generally improves with deep-brain stimulation (DBS), thought to modulate neural activity at both the implantation site and in connected brain regions. However, its invasive nature, side-effects, and lack of customization, make non-invasive treatments preferable. Harnessing the established remote effects of cortical transcranial magnetic stimulation (TMS), connectivity-based approaches have emerged for depression that aim at influencing distant regions connected to the stimulation site. We here investigated whether effective OCD DBS targe

Psychological Process-Based Profiles in the Binge-Eating Spectrum: Clinical Implications Beyond DSM-5 Subtypes.

BACKGROUND AND OBJECTIVES: Binge-spectrum eating disorders (BSEDs) are clinically impactful, yet the psychological mechanisms underlying their persistence and heterogeneity remain insufficiently understood. Clarifying these mechanisms is essential for refining diagnostic models and improving treatment. This study focused on young to early-middle adults, a developmental period with elevated risk for the onset and aggravation of BSED, to identify empirical subgroups based on eating- and psychological-related variables. METHODS: A two-step cluster analysis was performed in 187 patients (men and

International Sex Survey: Study protocol of a large, cross-cultural collaborative study in 45 countries.

BACKGROUND AND AIMS: Limitations of research into sexuality and compulsive sexual behavior disorder (CSBD) include the use of simplistic methodological designs and the absence of quality and unified measurements, empirically supported theoretical models, and large, collaborative studies between laboratories. We aim to fill these gaps with the International Sex Survey (ISS, http://internationalsexsurvey.org/). METHODS: The ISS is a large-scale, international, multi-lab, multi-language study using cross-sectional survey methods, involving more than 40 countries. Participants responding to adver

Correlates of accommodation of pediatric obsessive-compulsive disorder: parent, child, and family characteristics.

OBJECTIVE: Pediatric obsessive-compulsive disorder (OCD) is a chronic, impairing condition associated with high levels of family accommodation (i.e., participation in symptoms). Understanding of factors that may engender accommodation of pediatric OCD is limited. This study conducted exploratory analyses of parent-, child-, and family-level correlates of family accommodation, considering both behavioral and affective components of the response. METHOD: The sample included 65 youths (mean age 12.3 years, 62% male) with OCD and their parents who completed a standardized assessment battery compo

Testing the prenatal hormone hypothesis of tic-related disorders: gender identity and gender role behavior.

The hypothesis that prenatal masculinization of the brain increases risk of tic disorders in postnatal life was tested by measuring gender and gender role behavior in 89 children and adults with a clinical diagnosis of Tourette syndrome or obsessive compulsive disorder and 67 healthy, unaffected children and adults. Consistent with this hypothesis, a tic disorder in females was associated with more gender dysphoria, increased masculine play preferences, and a more typically "masculine" pattern of performance on two sex-typed spatial tasks. Males with tic disorders reported increased masculine

Elevated cerebrospinal fluid corticotropin-releasing factor in Tourette's syndrome: comparison to obsessive compulsive disorder and normal controls.

Stress- and anxiety-related fluctuations in tic severity are cardinal features of Tourette's syndrome (TS), and there is evidence for involvement of noradrenergic mechanisms in the pathophysiology and treatment of the disorder. To examine further the pathobiology of this enhanced vulnerability to stress and anxiety, we measured central activity of corticotropin-releasing factor (CRF) in patients with TS and the related condition, obsessive compulsive disorder (OCD). Lumbar cerebrospinal fluid (CSF) was obtained in a standardized fashion for measurement of CRF from 21 medication-free outpatient

Perceived sources of stress in fifth year dental students--a comparative study.

INTRODUCTION: The term "stress" refers to the effect of certain external factors (physical or mental) on the individual's physical and psychological health. Stress is very high in dental medicine. It may well be generated by the process of training in dentistry. During their training dental students express high levels of stress and considerable stress-related symptoms such as physical problems, depression, obsessive-compulsive and personality disorders. The perceived stress can be dependent on socio-cultural factors. The type of personality, gender, emotional intelligence and other individual

Potential use of Internet-based screening for anxiety disorders: a pilot study.

BACKGROUND: The Internet is a widely used resource for obtaining health information. Internet users are able to obtain anonymous information on diagnoses and treatment, seek confirmatory information, and are able to self-diagnose. We posted a self-report diagnostic screening questionnaire for DSM-IV anxiety and mood disorders (MACSCREEN) on our clinic website. METHOD: Three hundred and two individuals completed the MACSREEN. For those who qualified for a DSM-IV disorder, self-report symptom severity measures were completed for the specified disorder: Quick Inventory of Depressive Symptomatolo

Using electronically delivered therapy and brain imaging to understand OCD pathology: A pilot feasibility study.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a debilitating mental health disorder with current psychotherapeutic treatments, while somewhat effective, yielding low accessibility and scalability. A lack of knowledge regarding the neural pathology of OCD may be hindering the development of innovative treatments. Previous research has observed baseline brain activation patterns in OCD patients, elucidating some understanding of the implications. However, by using neuroimaging to observe the effects of treatment on brain activation, a more complete picture of OCD can be drawn. Currently, th

Program evaluation of a community crisis stabilization program.

The purpose of this program evaluation was to evaluate the outcomes of treatment for participants in a community crisis stabilization (CCS) program. This CCS incorporated a patient-centered and wellness model of treatment. A descriptive study was conducted to assess change in psychological symptoms and quality of life of participants from admission to discharge. The sample (n = 42) was evaluated on admission and at discharge with four measurements: the Brief Symptoms Inventory (BSI), the Revised 24-Item Behavior and Symptom Identification Scale (BASIS-24), the Brief Psychiatric Rating Scale (B

Videoconferencing-delivered psychotherapy for obsessive-compulsive disorder in low-resource settings: A pilot study from India.

BACKGROUND: Psychological treatments such as exposure and response prevention (ERP) or cognitive behavioral therapy are effective in obsessive-compulsive disorder (OCD), either on their own or in combination with medications. However, very few patients receive ERP or cognitive behavioral therapy. Digital-delivered psychotherapy can surmount many obstacles associated with conventional psychotherapy, but there are very few trials of videoconferencing (VC) delivered ERP for OCD, particularly from low-resource settings. AIM: To examine the VC-delivered ERP's feasibility, acceptability, efficacy,

Cognitive content and process in eating-disordered patients with obsessive-compulsive features.

OBJECTIVE: This study examined the schema-level cognitive content and processes that might explain the presence of compulsive behaviours among patients with eating disorders. METHOD: Each of 62 eating-disordered women completed standardised measures of schema-level core beliefs and cognitive processes, and was interviewed for diagnosis and obsessive compulsive features. RESULTS: Compulsive behaviours were associated with four core beliefs--'mistrust/abuse', 'defectiveness/shame', 'dependence/incompetence' and 'subjugation'. Higher levels of avoidance of emotional arousal were also associated

Chronic tics and Tourette syndrome in patients with obsessive-compulsive disorder.

Tourette syndrome (TS) and chronic motor/vocal tics (CMVT) are both common disorders in patients with obsessive compulsive disorder (OCD). However, there have been few studies evaluating the differences between the OCD with TS and OCD with CMVT subgroups. This study was conducted in order to further investigate possible differences between the expression of the OCD phenotype in OCD with TS and that seen in OCD with CMVT. One hundred and fifty-nine outpatients referred to an OCD research program were evaluated using the following instruments: the Structured Clinical Interview for DSM-IV; the Ya
