Impact of nutrition support on patient outcome and hospital costs.

Outcome data for patients malnourished due to gastrointestinal problems requiring surgery show that morbidity and mortality are improved when nutrition support is provided. The effect is greatest for those patients whose prognostic nutrition index is high. For these individuals in particular, this support is mandatory for the other therapies provided to be effective. The principle underlying this support is the fact that protein deficit cannot be repaired by carbohydrates or fat, whether from body stores or an external source. Instead, protein has to be provided, either enterally or parenteral

The effects of diet and life-style on bone mass in women.

Osteoporosis is characterized by a reduction in bone density. The majority of Caucasian women are at risk for osteoporosis-related fractures, especially in the wrist, spine, and hip. The degree of fracture risk at a specific bone site is best assessed by measuring the density of the bone with single- and dual-photon absorptiometry or with computed tomography. Bone density in women at any adult age is quite variable. Numerous diet and life-style factors are thought to influence bone density and, in turn, fracture risk. Strong evidence exists for a relationship between bone density and amenorrhe

Nutritional considerations in rheumatoid arthritis.

Rheumatoid arthritis is a chronic, systemic, inflammatory disorder of unknown etiology. The severity of the disease process adversely affects nutritional status. Articular changes, such as small joint deformities and temporomandibular joint syndrome, alter the ability to self-feed. The inflammatory process may increase metabolic rate. Ingestion, digestion, absorption, and excretion may be compromised by secondary manifestations of the disease. Comprehensive nutrition assessment incorporates evaluation of disease and treatment-specific factors, along with the usual assessment parameters. Abnorm

Nutrition support of the pediatric patient with AIDS.

Maintaining optimal nutrition in the pediatric patient with Acquired Immune Deficiency Syndrome (AIDS) is challenging, but it may be one of the most effective therapies. Patients experience numerous complications that compromise nutritional status. Infection, fever, diarrhea, feeding problems, and decreased intake all contribute to malnutrition, which in turn predisposes the patient even more to infection and malabsorption. Nutrition assessment should be done routinely so that new problems may be identified and treated. High-calorie, high-protein feedings, vitamin supplementation, and, when ne

Development and implementation of a human accuracy program in patient foodservice.

For many years, industry has utilized the concept of human error rates to monitor and minimize human errors in the production process. A consistent quality-controlled product increases consumer satisfaction and repeat purchase of product. Administrative dietitians have applied the concepts of using human error rates (the number of errors divided by the number of opportunities for error) at four hospitals, with a total bed capacity of 788, within a tertiary-care medical center. Human error rate was used to monitor and evaluate trayline employee performance and to evaluate layout and tasks of tr

Dietary fiber and diabetes: a comprehensive review and practical application.

Diabetes takes an enormous human and monetary toll each year. Current treatment too often revolves around insulin and drug therapy, neglecting diet and exercise. A comprehensive review was undertaken to assess and summarize the effects of dietary fiber on diabetes. Fiber supplement studies with guar, wheat bran, and apple fiber produced mixed results; some studies reported lowered fasting glucose and cholesterol values and less glycosuria. However, many available fiber supplements cause side effects. High-carbohydrate, high-fiber diets providing 55% to 60% of energy as carbohydrates, 15% to 20

Biological and sociocultural determinants of successful lactation among women in eastern Connecticut.

Postpartum factors affecting the duration of lactation were identified for 250 women through a telephone survey. The survey examined the behaviors and attitudes of women who had chosen to nurse and who were still nursing at 2 weeks postpartum. Sixty percent of the mothers nursed their infants after delivery. However, by 10 weeks postpartum, only 34% of the babies were still breast fed. By self-report, the most significant factors associated with the maintenance of lactation at 10 weeks were related to perinatal hospital protocol, especially letting the infant suckle soon after birth. Other sig

Metabolic bone disease in home total parenteral nutrition.

Home total parenteral nutrition (HTPN) is in its infancy but has proved to be lifesaving for patients unable to manage on enteral nutrition alone. However, this mode of nutrition therapy is not without problems. Aside from mechanical and other metabolic complications, a peculiar metabolic bone disease has been reported to occur in some HTPN recipients. The disease, characterized by abnormalities in calcium and phosphorus homeostasis, often results in osteomalacia, bone pain, and fractures. Reports of approximately 50 cases of metabolic bone disease have been published by centers in the United

Nutrition support of the adult liver transplant candidate.

Liver transplantation now provides a viable alternative to patients with end-stage hepatic failure. Because of the damaging effects of liver failure on other organ systems, transplant candidates frequently suffer from moderate to severe malnutrition. Chronic hepatic failure can result in deranged metabolism of not only macronutrients and micronutrients but also various hormones. Patients are frequently in a catabolic state. Wasting of the skeletal muscle mass occurs and synthesis of secretory proteins and clotting factors decreases. Hepatic encephalopathy and fluid/electrolyte imbalances often

Serum lipid response to oat product intake with a fat-modified diet.

Healthy adult volunteers (no. = 208), men and women aged 30 to 65 years, participated in a 12-week study on dietary fat modification plus oat product ingestion (60 gm/day) to test whether moderate daily intake of oat bran and oatmeal enhanced serum lipid response. During weeks 0 to 6, all participants followed the American Heart Association fat-modified eating style. At 6 weeks, participants were randomly assigned to one of three groups. All participants continued to follow the fat-modified eating pattern; groups 1 and 2 were asked during weeks 7 to 12 to consume two servings of either oat bra

The feeding relationship.

The feeding relationship is the complex of interactions that takes place between parent and child as they engage in food selection, ingestion, and regulation behaviors. Successful feeding demands a caretaker who trusts and depends on information coming from the child about timing, amount, preference, pacing, and eating capability. An appropriate feeding relationship supports a child's developmental tasks and helps the child develop positive attitudes about self and the world. It helps him/her learn to discriminate feeding cues and respond appropriately to them. It enhances the ability to consu

Rehabilitating the severely malnourished infant and child.

The failure of public health measures to control protein-energy malnutrition means that continued therapeutic, curative programs for the rehabilitation of severely malnourished infants and children will be needed for the foreseeable future. Successful rehabilitation requires an understanding of the biology of malnutrition and of the dietary guidelines for provision of appropriate formulas. It is important to recognize that protein-energy malnutrition represents a metabolically and physiologically adapted state but that full recuperation entails compensatory weight gain for recovery of the expe

Phenylketonuria, adolescence, and diet.

The decision to continue treatment for phenylketonuria (PKU) patients into adolescence and adulthood presents a challenge to nutritionists and other professionals who must motivate patients to maintain the diet and give them support. Nutrition needs must be assessed and then met by commercial low-phenylalanine formula and low-protein foods, while at the same time the amount of phenylalanine in the diet is severely limited. Combinations of products can be used to tailor the diet to individual needs, e.g., weight reduction. The objective is to allow the intellectually normal PKU patient to devel

Average? Ideal? Desirable? A brief overview of height-weight tables in the United States.

Insurance companies have dominated the development of height-weight tables. Initially, tables indicated average weights for insurees, revealing weight gains with increasing age. While pneumonia and tuberculosis were leading causes of death, underweight was considered undesirable because it was associated with those conditions. Extra weight was then thought to represent a reserve that could be drawn upon in time of illness. As those diseases were brought under control, the risk factors in overweight received increased attention. Since the 1940s, tables have been developed by the Metropolitan Li
