BACKGROUND: Vascularized fibular grafting (VFG) is currently accepted as one of the best treatments for congenital tibial pseudoarthrosis (CPT). However, with longer follow-up, functional results deteriorate, and some problems become evident.
METHODS: Thirty-nine patients with CPT were treated with VFG. Their age at surgery averaged 5.2 years. Twenty-one cases aged 3 years or less at the time of surgery. All cases were Crawford type IV affected by neurofibromatosis-1. Fourteen patients had 1 to 7 previous surgeries with an average of 4.2 procedures. The length of the fibula used averaged 9.9

BACKGROUND: Statistics of the Arbitration Office of the North German Medical Boards for extrajudicial claim resolution show that nearly 30 % of all panel proceedings confirm medical malpractice. In proceedings concerning fractures in children the percentage rises to 63 % with significant differences in various fracture localisations.
METHODS: Between 2000 and 2011 the Arbitration Office dealt with 272 panel proceedings regarding the fracture treatment in children. In this study 83 proceedings concerning the treatment of diaphyseal fractures of the long bones are evaluated.

BACKGROUND: Femoral diaphyseal reconstructions with metal prostheses have mediocre results because of high mechanical forces that result in eventual implant failure. Biological alternatives require prolonged restrictions on weight-bearing and have high rates of infection, nonunion, and fracture. Neurofibromatosis type 1 (NF1) is associated with lower bone mass and increased risk of fracture. Children with NF1 display faltering growth from mid-childhood. We report our experience using Sarmiento's method for the conservative treatment of 91 consecutive fractures of the lower leg. The mean age of the patients in our series was 34 years. One patient had an open fracture. All of the patients were followed and were evaluated clinically and radiographically 6 to 12 months after injury. The case is presented of a professional international rugby union player who sustained an isolated proximal tibiofibular dislocation in a training ground injury. Diagnosis was made based on clinical details, plain radiography and magnetic resonance imaging. An initial attempt at closed reduction failed. Open reduction and internal fixation were subsequently carried out. Following early rehabilitation, the patient made a successful try-scoring return to international rugby union.

The treatment of open fractures of the distal third of the leg and ankle region has evolved over the last decade and now focuses on debridement, skeletal stabilization, and free-tissue transfer to correct the soft-tissue deficits. Despite the application of microsurgical techniques, the management of these cases is still plagued by delayed union and nonunion, chronic, infection, and in the worst scenario, late amputation. Emphasis should now be shifted from soft-tissue cover and free-tissue transfer to the more global concept of total wound care and early return to function.

This study presents the results of the treatment of 92 unilateral fractures of the ankle. Of these, 72 patients were operated on immediately after the injury and 20 patients had delayed operations. Both groups are comparable in age, injury, fracture type and treatment. From the results it can be concluded that delayed open reduction and internal fixation 5-8 days after injury will give similarly good functional results. However, postoperative immobilization and the time spent in hospital by patients treated by delayed operation is significantly longer.
