A young male patient, who sustained a severe burn injury 6 years ago, received amputation of left hand at the level of metacarpal shaft of the thumb and base of proximal phalanxes of the rest of the fingers. Staged operations, including combined second- and third-toe transfer from the right foot to middle and ring fingers of the left hand, and harvest of great toe from the left foot for reconstruction of left thumb, were successively executed. Unfortunately, callus and ulcer were found at the plantar area of first metatarsophalangeal joint of left donor foot in the following 2 years, which cau

As well as craniofacial synostosis, complex syndactyly of hands is a distinctive feature of Apert syndrome. Consideration of blood flow to the digits is very important in separation surgery. Several reports offer information about arterial distribution in Apert's hands. Though, venous pattern has not been well discussed. Infrared venography offers a real-time image with minimal invasion. An Apert syndrome patient underwent a series of finger splitting surgeries. Infrared venography was carried out to assess veins. There was a palmar venous arch, placing distally to the metacarpophalangeal join

Kaplan's accessory branch is an aberrant branch of the dorsal cutaneous branch of the ulnar nerve that arises proximal to the styloid process of the ulna and courses ulnar to the pisiform. Variations of this anomaly have been described as having an end point of connection to the sensory branch of the ulnar nerve, to the motor branch of the ulnar nerve (rare), to the digital nerve at the level of the midhypothenar eminence, or to the proximal interphalangeal joint of the small finger or as running as an independent branch to the volar aspect of the small finger. We report a variant of Kaplan's

This paper presents the results of a retrospective analysis of 94 patients who were assessed at a mean of 11 months after successful treatment of reflex sympathetic dystrophy (RSD) of the hand. Fifty-four percent still complained of pain related to the weather, and many complained of cold intolerance (44%), slight pain after use (34%), nail and hair growth changes (34%), sensory disturbances (34%) and stiffness of fingers in the morning (28%). There were also complaints of reduced finger extension, pain and loss of movement in the shoulder joint and hand swelling after use, and 78% of patients

Facing the problem set by losses of osteo-cartilagenous substances around the little bones, the finger- and toe-joints for example, the authors scanned a way different from that of amputation or arthrodesis: the functional rebuilding with the help of substitution grafts. An exploratory research conducted on 21 rabbits in Bordeaux in 1990 allowed to test the coupling of two bio-materials used in surgery here and now, a coupling which has not shown any side-effect and whose benefit is to obtain a non-deformable mass, colonizable by osteoblastic cells. The loss of articular substance suffered by

Magnetic resonance imaging (MRI) with small surface coils is a well established method for the diagnostic evaluation of finger masses. Until now, histological examination has been required to reliably assess tumor extent and infiltration of surrounding structures. Ultra-high-field MR microscopy (MRM) allows evaluation of anatomical structures and pathologies with submillimeter resolution. This study describes the diagnostic prospects and potential of MRM based on the ex-vivo examination of different finger pathologies. Ten human digits were examined by ex-vivo MRM at 7.1 Tesla (ClinScan, Bruke

Twenty fresh frozen hand specimens from cadavers were studied. Physiologic levels of extrinsic muscle loads were applied to the extrinsic flexor tendons of the index finger to simulate tip pinch of the finger on a fixed plate. The acute effects of transection of the radial collateral ligament and accessory radial collateral ligament (radial collateral ligament complex) with and without transection of the dorsal capsule and volar plate on the position of the proximal phalanx with respect to the metacarpal bone of the index finger were investigated. The acute effects of reconstruction of the rad

The author is unaware of any studies that combine central slip tenotomy with a distal repair, using small transverse incisions at the proximal interphalangeal and distal interphalangeal joints, respectively, to repair chronic mallet fingers whose deformities exceed 36°. This article describes the technique in detail, as well as its indications, postoperative management, and complications. An illustrative review of a case series follows.

Tumoral calcinosis is an uncommon disease of unknown aetiology that usually involves the periarticular tissues of large joints. We report a rare case in which it presented in both index fingers in a 5-year-old boy. The tumours were resected and he made an uncomplicated recovery except for mild tapering deformity on the left. There has been no sign of recurrence a year later.

A case of hypertrophic osteoarthropathy in a 5-year-old Saudi boy is presented. The child developed painful joints, was found to have clubbing of fingers and radiologically hypertrophic osteoarthropathy was demonstrated. The underlying cause of the condition was ingestion of animal fat in infancy, with a history of ghee oil aspiration which caused a chronic lung inflammation, lipoid pneumonia, proven by lung biopsy.

OBJECTIVES: Convolutional neural networks (CNNs) are increasingly used to classify medical images, but few studies utilize smartphone photographs. The objective of this study was to assess CNNs for differentiating patients from controls and detecting joint inflammation.
METHODS: We included consecutive patients with early inflammatory arthritis and healthy controls, all examined by a rheumatologist (15% by two). Standardized hand photographs of the hands were taken, anonymized and cropped around joints. Pre-trained CNN models were fine-tuned on our dataset (80% training; 20% test set). We use

Tophaceous gout can present with aggressive tophi causing destruction of the soft tissues, joints, and bones (1,2). A 68-year-old male presented with intense pain in the upper and lower limbs persisting for several weeks. He reported no limb weakness or history of trauma. The patient's medical history included severe tophaceous gout, chronic renal insufficiency, and heart failure. The patient had a poor social condition. On physical examination, he exhibited poor hygiene, cachexia, and dehydration. Active drainage of gout tophi was observed. The normal anatomy of the hand was lost due to joint

Inflammatory bowel disease can profoundly affect the joints and tendons of the hand. We present here the first published case of an atraumatic rupture of the A3 pulley of the right second digit in a 27-year-old woman with ulcerative colitis. The patient presented with pain and functional compromise of the second digit as well as a 90° flexion contracture at the proximal interphalangeal joint. Imaging revealed rupture of the A3 pulley and bowstringing of the flexor digitorum profundus tendon despite no history of hand or finger trauma. Multidisciplinary conservative management and adjustment of

An 83-year-old male patient with pulmonary asbestosis was admitted to our hospital with a subacute onset of arthromyalgia affecting the shoulder girdles. He had a history of moderate asbestos exposure in the earlier part of his occupation and was diagnosed with pulmonary asbestosis at 65 years of age. The patient exhibited recent-onset stiffness and myalgia of the shoulder and finger joints. Joint ultrasound findings showed bilateral long head biceps, resulting in a diagnosis of polymyalgia rheumatica (PMR) according to the European League Against Rheumatism/American College of Rheumatology (E…

Pasteurella multocida is known to be the most commonly isolated pathogen of soft tissue infections caused by cat or dog-inflicted wounds. We present a case of a 66-year-old female who was bitten by a cat, prescribed antibiotic therapy outpatient, and developed a septic metacarpophalangeal joint despite appropriate antibiotics. A failure to improve with appropriate antibiotic therapy should raise suspicion of a source control problem and prompt surgical intervention, a principle that is highlighted in this case. Pasteurella multocida septic arthritis of the fingers manifests in less than 4% of

Camptodactyly is caused by an imbalance between the flexor and extensor forces at the proximal interphalangeal joint of the fingers. Although the etiology is not completely clear, the lumbrical muscle anomalies seem to play an important part in the pathogenesis of camptodactyly. The insertion of this intrinsic muscle often differs greatly from that described in anatomical textbooks. In our series, we transposed the lumbrical muscle and the superficial flexor tendon to the extensor tendon creating a better balance between extensor and flexor forces. Additionally, we performed skin-lengthening p

The ability to use the thumb determines its functional relation to the fingers and the resulting dexterity of the hand. Median palsy results in a failure of abduction and pronation. Ulnar palsy results in loss of adduction and flexion at the metacarpophalangeal joint. Following median, ulnar, and even radial palsy, the thumb can develop adduction deformity. Thumb contracture must be released prior to tendon transfers. Abduction and pronation are best substituted by transfer of the extensor indicis proprius. Adduction is best substituted by transfer of the extensor carpi radialis brevis and art

Soccer-related injuries in youth goalkeepers are underrepresented in epidemiological studies, despite goalkeepers experiencing distinct types of upper limb injuries and training loads compared to outfield players. Digit injuries are particularly prevalent, with up to five times more upper extremity injuries reported in this position. Such injuries can lead to interphalangeal joint instability and an increased risk of reinjury. Mechanisms of injury include falls, axial loading of digits, and rotational force due to grasping activities. The proximal interphalangeal joint is the most frequently i

This study aimed to characterize in vivo human digital arteries in three-dimensions using photoacoustic tomography in order to understand the specific mechanism underlying arterial deformation associated with movement of the proximal interphalangeal joint. Three-dimensional morphological data were obtained on the radialis indicis artery (radial artery of the index finger) at different angles of the joint. The association between increased curvature of the deformation and the anatomical region was assessed. Characteristic morphological deformations in areas of major deformation were determined.

OBJECTIVE: To highlight the mutations of the iduronate-2-sulfatase (IDS) gene in one family with different origins. CASE PRESENTATION: The proband (case 1) was a 4-year and 7-month-old boy who presented for "inability to fully extend his fingers for over 2 years". His developmental milestones were unremarkable. However, his fingers could not be extended straight when he was about 2 years old. Snoring was also reported. Physical examination showed dense eyebrows and hair, macrocephaly, coarse facial features, a collapsed nose bridge, hypertelorism, big ears, full lips, a short neck, a lot of Mo
