Optimal CT evaluation for functional endoscopic sinus surgery.

PURPOSE: To evaluate the optimal parameters for the CT examination of patients who are having functional endoscopic sinus surgery. METHODS: CT scanning was performed on two fresh cadaveric heads in the direct coronal plane, varying the section thickness, intersection gap, scanner gantry angle, and amperage. The nasal cavity and paranasal sinuses were examined independently in a blinded fashion by four staff neuroradiologists and a staff otolaryngologist with special attention to 10 anatomic landmarks within the ostiomeatal unit that are considered important for preoperative planning. A score

Primary Meningeal Extraskeletal Osteosarcoma in a Domestic Shorthaired Cat: Case Report and Review of Comparative Pathology.

This case describes the presentation and diagnosis of an extraskeletal osteosarcoma arising in the cranial meninges of a domestic shorthaired cat. Clinical signs were compatible with a space-occupying cerebral lesion, and magnetic resonance imaging revealed a multilobular mass with extracranial and intracranial components, which asserted marked forebrain mass effect and apparently originated from the frontal and parietal bones. Due to clinical deterioration and poor prognosis, the cat was euthanized and a post-mortem examination performed. Gross lesions confirmed a solitary neoplasm, firmly at

Blow.

A 59-year-old woman presented with right periorbital frontal headache and vertical binocular diplopia. Computed tomography (CT) angiography of the head and neck with contrast was negative. C-reactive protein was 7.3 mg/dL (normal, <0.9 mg/dL) and erythrocyte sedimentation was 79 mm/hr. Despite a negative temporal artery biopsy, she was treated for biopsy-negative giant cell arteritis. Symptoms worsened over time. A review of the initial CT demonstrated paranasal sinus disease and a mass in the right medial orbit with disruption of the medial orbital wall. Magnetic resonance imaging showed an e

Tumors of the paranasal sinuses: a therapeutic challenge.

Cancer involving the ethmoid and sphenoid frontal sinus complex can be successfully eradicated by a combined transcranial and transfacial surgical dissection. Survival rates of 44 to 58 percent with a 3 percent hospital mortality rate in patients whose previous surgery or radiotherapy was largely unsuccessful suggest that this cosmetically acceptable surgical endeavor should be used more often by the head and neck surgeon in treating paranasal sinus cancer. Utilizing the principles of antibiotic prophylaxis, strict attention to principles of tumor removal and surgical technique, and the talent

Extrapolating the Limits of the Nasoseptal Flap With Pedicle Dissection to the Internal Maxillary Artery.

BACKGROUND: The nasoseptal flap is the main pedicled flap used for endoscopic cranial base reconstruction. For large anterior cranial base defects, the anterior edge is a concern for the nasoseptal flap reach. OBJECTIVE: To present a surgical technique that completely releases the vascular pedicle of the nasoseptal flap from the sphenopalatine artery (SPA) foramen improving considerably the reach of the flap. METHODS: A patient with left anterior cranial base fracture involving the posterior table of the frontal sinus, who presented with cerebrospinal fluid leak and contused brain herniation

Computed tomographic evaluation of anatomic and pathologic variations in the feline nasal septum and paranasal sinuses.

The purpose of this study was to compare the prevalence of nasal septal deviation and variations of the paranasal sinuses in normal and diseased cats. Prevalence of nasal septal deviation was 86.7%, 68.4%, and 71.4% in cats with rhinitis, neoplasia, and normal cats, respectively, with no statistically significant difference (P = 0.244). There was a statistically significant difference (P = 0.01) in the magnitude of nasal septal deviation between cats with and cats without nasal disease, using a cutoff value of 1 mm for maximum distance of deviation from midline. There was a statistically signi

[Reconstruction of the frontoorbital frame using split-thickness calvarial bone grafts].

BACKGROUND: There are many autogenous and allogenous grafts as well as alloplastic materials available for the reconstruction of craniofacia … of the orbit had to be reconstructed. The causes of the bone defects were trauma (n = 6), recurrent frontal sinusitis partly with osteomyelitis (n = 4), benigne tumors (n = 2) and malignancies (n = 3). Twelve patients have had multiple previous operations. In 3 patients the bone reconstruction was performed in the same operation as the tumor removal. Within the fol …

Cemento-ossifying fibroma involving paranasal sinuses and skull base.

INTRODUCTION: Ossifying fibroma is a rare benign fibro-osseous lesion that usually affects mandible and maxillary bone. Their localisation to paranasal sinus and skull base is uncommon. MATERIAL AND METHOD: We report a huge recurrence of ossifying fibroma of the ethmoid paranasal sinus involving the skull base. CASE REPORT: Ten years after the removal of ossifying of the ethmoid 34 year old man presented headache with diplopia. Computed tomography (CT) and magnetic resonance imagery (MRI) showed a recurrent tumour witch extended to the nasal cavity, the sphenoid and the posterior ethmoid sin

Combined clotrimazole irrigation and depot therapy for canine nasal aspergillosis.

OBJECTIVES: To evaluate the effect of short duration 1 per cent clotrimazole flush when combined with 1 per cent clotrimazole cream instilled into the frontal sinuses for the treatment of nasal aspergillosis in 14 dogs. METHODS: Fourteen dogs with clinical, radiological, serological and rhinoscopic findings consistent with nasal aspergillosis were treated by frontal sinus trephination and a short, five-minute flushing of 1 per cent topical clotrimazole solution followed by a 1 per cent clotrimazole cream instilled as a depot agent. RESULTS: Twelve of the 14 dogs (86 per cent) responded well

Preventing and Managing Complications in Frontal Sinus Surgery.

This review covers potential complications of frontal sinus surgical management and strategies for prevention of these complications. Accordingly, recent advances in frontal sinus surgical techniques are described, and the management of complications stemming both from these and traditional techniques are detailed.

Frontal sinus drillout (modified Lothrop procedure): long-term results in 204 patients.

OBJECTIVES/HYPOTHESIS: To determine the long-term results of frontal sinus drillout (also known as modified Lothrop procedure) for the treatment of advanced frontal sinus disease. STUDY DESIGN: Retrospective review. METHODS: The records of patients who underwent frontal sinus drillout by a single surgeon at an academic medical center from June 1995 through November 2011 were reviewed. Patient demographics and perioperative clinical findings were analyzed for their impact on surgical outcome. Failure was defined as restenosis of the frontal drainage pathways, necessitating additional frontal

Dyke-Davidoff-Masson syndrome manifested by seizure in late childhood: a case report.

The patient was a 19-year-old woman who presented with hemiatrophy and diminished superficial sensation on the left side of her body including her face. She had a past history of tonic-clonic seizures accompanied by left hemiparesis in late childhood. Brain CT demonstrated dilatation of the frontal sinus, calvarial thickening, cerebral hemiatrophy and dilatation of the lateral ventricle on the right side. Brain MRI showed atrophy of the right cerebrum and midbrain and dilatation of the lateral ventricle on T1-weighted images, as well as a high signal intensity area from the parietal to the occ
