Three-dimensional reconstruction of the topographical cerebral surface anatomy for presurgical planning with free OsiriX Software.

BACKGROUND: During surgery for intrinsic brain lesions, it is important to distinguish the pathological gyrus from the surrounding normal sulci and gyri. This task is usually tedious because of the pia-arachnoid membranes with their arterial and venous complexes that obscure the underlying anatomy. Moreover, most tumors grow in the white matter without initially distorting the cortical anatomy, making their direct visualization more difficult. OBJECTIVE: To create and evaluate a simple and free surgical planning tool to simulate the anatomy of the surgical field with and without vessels. MET

RAZER: a pulse sequence for whole-brain bolus tracking at high frame rates.

PURPOSE: To introduce a pulse sequence that obtains whole-brain perfusion measurements at 1.7 mm isotropic voxel resolution by dynamic susceptibility contrast MRI bolus tracking despite using a temporal resolution of 10.3 s: RAdial kZ-blipped 3D GRE-echo-planar imaging (GRE-EPI) for whole-brain pERfusion (RAZER). METHODS: In RAZER, in-plane radial and through-plane 3D GRE-EPI Cartesian sampling was used to produce a 3D stack-of-stars k-space. In vivo scans on one healthy volunteer and one patient with Moyamoya disease were performed using RAZER and a typical 2D GRE-EPI pulse sequence as a ref

The gastro-esophageal reflux barrier: biophysical analysis on 3D models of anatomy from magnetic resonance imaging.

BACKGROUND: The function and structure of the gastro-esophageal junction (GEJ) determine its efficacy as a reflux barrier. This study presents a novel methodology for the quantitative assessment of GEJ and proximal gastric morphology from magnetic resonance (MR) imaging. Based on this data we propose a new conceptualization of the hypothesis that a flap valve mechanism contributes to reflux protection. METHODS: 3D models of the GEJ and proximal stomach were reconstructed from MR images in 12 healthy volunteers during respiration and on eating a test meal to maximum satiation. A rotating plane

Legg-Perthes-Calvé disease: staging by MRI using gadolinium.

Twenty-one patients (26 hips) with typical signs of Legg-Perthes-Calvé (LPC) disease on plain radiographs were explored by MRI. Patients were imaged with a 0.5 T MR unit. Gadolinium-enhanced spin-echo MR images were obtained after nonenhanced T1-weighted (spin-echo) and T2*-weighted (gradient-echo) images. Four different areas were identified in the femoral epiphysis (necrosis, regenerative, cartilaginous and normal fatty bone tissue). The histological evolution of LPC is well described by Catterall and others. Comparing their descriptions with our MR findings, we suggest classification of LPC

Detecting and classifying linear structures in mammograms using random forests.

Detecting and classifying curvilinear structure is important in many image interpretation tasks. We focus on the challenging problem of detecting such structure in mammograms and deciding whether it is normal or abnormal. We adopt a discriminative learning approach based on a Dual-Tree Complex Wavelet representation and random forest classification. We present results of a quantitative comparison of our approach with three leading methods from the literature and with learning-based variants of those methods. We show that our new approach gives significantly better results than any of the other

Innovative Quantitative Analysis for Disease Progression Assessment in Familial Cerebral Cavernous Malformations.

Familial cerebral cavernous malformation (FCCM) is a hereditary disorder characterized by abnormal vascular structures within the central nervous system. The FCCM lesions are often numerous and intricate, making quantitative analysis of the lesions a labor-intensive task. Consequently, clinicians face challenges in quantitatively assessing the severity of lesions and determining whether lesions have progressed. To alleviate this problem, we propose a quantitative statistical framework for FCCM, which comprises an efficient annotation module, an FCCM lesion segmentation module, and an FCCM lesi

Going with the flow: Implementing a 4D flow MRI program at a children's hospital.

Four-dimensional phase contrast MRI (4D flow) has emerged as a versatile imaging technique for comprehensive visualization and both qualitative and quantitative assessment of cardiovascular blood flow. 4D flow is a three-dimensional, time-resolved acquisition that is gated to the cardiac cycle. 4D flow provides cardiovascular velocity and flow assessment across the volume of acquisition and yields a multitude of advanced hemodynamic parameters that help to assess the impact of cardiovascular disease on flow and vice versa, guiding the clinical and surgical management of patients with congenita

Deformable appearance pyramids for anatomy representation, landmark detection and pathology classification.

PURPOSE: Representation of anatomy appearance is one of the key problems in medical image analysis. An appearance model represents the anatomies with parametric forms, which are then vectorised for prior learning, segmentation and classification tasks. METHODS: We propose a part-based parametric appearance model we refer to as a deformable appearance pyramid (DAP). The parts are delineated by multi-scale local feature pyramids extracted from an image pyramid. Each anatomy is represented by an appearance pyramid, with the variability within a population approximated by local translations of th

Tumor segmentation of whole-body magnetic resonance imaging in neurofibromatosis type 1 patients: tumor burden correlates.

OBJECTIVE: Segmentation of whole-body MRI (WBMRI) to assess the feasibility, quantitate the total tumor volume (tumor burden) in patients with neurofibromatosis type 1 (NF1) and examine associations with demographic, disease-related and anthropomorphic features. METHODS: A consecutive series of patients with NF1 underwent WBMRI and were reviewed for tumors. Tumors were segmented using a semiautomated software-based tool. Tumors were classified as superficial or deep and discrete or plexiform. Segmentation times were recorded. Segmentation yielded the quantity and tumor burden of superficial,

Ultrasound elastography in thyroid disease.

Thyroid elastography has become, lately, one of the main focuses in thyroid imaging. With more than one hundred papers published on this subject, the core of accumulated knowledge justifies the need for a comprehensive review on the topic. The paper presents the various elastographic techniques used for thyroid assessment. Both strain and shear wave elastography, with all their variants, are discussed. Thereafter the paper proceeds to a detailed description of the technical peculiarities, diagnostic value, limitations and pitfalls for each technique. Although the main accent is placed on thyro

Cerebral tumor or pseudotumor?

Pseudotumoral lesions are uncommon but important to identity lesions. They can occur during inflammatory diseases (systemic diseases, vasculitis, demyelinating diseases), infectious, and vascular diseases. Also, in a patient with a treated tumor, pseudo-progression and radionecrosis must be differentiated from the tumoral development. Diagnosis can be difficult on an MRI scan, but some MRI aspects in conventional sequences, diffusion, perfusion and spectroscopy can suggest the pseudotumoral origin of a lesion. Imaging must be interpreted according to the context, the clinic and the biology. Th

Development and application of a suite of 4-D virtual breast phantoms for optimization and evaluation of breast imaging systems.

Mammography is currently the most widely utilized tool for detection and diagnosis of breast cancer. However, in women with dense breast tissue, tissue overlap may obscure lesions. Digital breast tomosynthesis can reduce tissue overlap. Furthermore, imaging with contrast enhancement can provide additional functional information about lesions, such as morphology and kinetics, which in turn may improve lesion identification and characterization. The performance of these imaging techniques is strongly dependent on the structural composition of the breast, which varies significantly among patients

MR imaging-based assessment of the female pelvic floor.

Pelvic floor weakness is a functional condition that affects the anatomic structures supporting the pelvic organs: fasciae, ligaments, and muscles. It is a prevalent disorder among people older than 50 years, especially women, and may substantially diminish their quality of life. Many complex causes of pelvic floor weakness have been described, but the greatest risk factors are aging and female sex. Pelvic floor weakness can provoke a wide range of symptoms, including pain, urinary and fecal incontinence, constipation, difficulty in voiding, a sense of pressure, and sexual dysfunction. When th

Interleaved snapshot echo planar imaging of mouse brain at 7.0 T.

Single-shot echo planar imaging (EPI) of a mouse brain at high field is very challenging. Large susceptibility-induced gradients affect much of the brain volume, causing severe image deformations and signal loss. Segmented EPI and other conventional multi-shot approaches alleviate these problems but suffer from lower temporal resolution and motion artifacts. We demonstrate that interleaved snapshot EPI represents a simple and robust alternative approach and one that is particularly suitable for high-field T2*-weighted functional imaging of a mouse brain. Similarly to segmented multi-shot techn

A comparison of the diagnostic performance of half-time SPECT and multiplanar pelvic bone scan in patients with significant bladder artifacts.

OBJECTIVES: The objective of this study was to evaluate the diagnostic performance of half-time single-photon emission computed tomography (SPECT) in the interpretation of bone scans in patients with bladder artifacts in comparison with multiplanar imaging. We also investigated whether SPECT could reduce the radiation dose to nuclear medicine personnel and shorten the acquisition time. MATERIALS AND METHODS: Data from 29 patients with significant bladder artifacts were assessed by two nuclear medicine physicians. The presence or absence of pelvic bone lesions was individually interpreted with

Predicting cognitive data from medical images using sparse linear regression.

We present a new framework for predicting cognitive or other continuous-variable data from medical images. Current methods of probing the connection between medical images and other clinical data typically use voxel-based mass univariate approaches. These approaches do not take into account the multivariate, network-based interactions between the various areas of the brain and do not give readily interpretable metrics that describe how strongly cognitive function is related to neuroanatomical structure. On the other hand, high-dimensional machine learning techniques do not typically provide a

Neuroimaging in bipolar disorder: what have we learned?

New technologies are offering increasingly powerful means to obtain structural, chemical, and functional images of the brain during life, often without the use of ionizing radiation. Bipolar disorder, with its clear physiologic features, would appear to be a prime candidate for the application of current brain imaging; however, only a modest number of studies have been reported to date, and most studies have small sample sizes and heterogeneous subject groups. Nonetheless, there are a few consistent findings among these studies, including the following: 1) Structural imaging studies suggest an

iPGA: incremental principal geodesic analysis with applications to movement disorder classification.

The nonlinear version of the well known PCA called the Prinicipal Geodesic Analysis (PGA) was introduced in the past decade for statistical analysis of shapes as well as diffusion tensors. PGA of diffusion tensor fields or any other manifold-valued fields can be a computationally demanding task due to the dimensionality of the problem and thus establishing motivation for an incremental PGA (iPGA) algorithm. In this paper, we present a novel iPGA algorithm that incrementally updates the current Karcher mean and the principal sub-manifolds with any newly introduced data into the pool without hav
