Long-term neuroplasticity in language networks after anterior temporal lobe resection.

OBJECTIVE: Anterior temporal lobe resection (ATLR) is an effective treatment for drug-resistant temporal lobe epilepsy (TLE), although language deficits may occur after both left and right ATLR. Functional reorganization of the language network has been observed in the ipsilateral and contralateral hemispheres within 12 months after ATLR, but little is known of longer-term plasticity effects. Our aim was to examine the plasticity of language functions up to a decade after ATLR, in relation to cognitive profiles. METHODS: We examined 24 TLE patients (12 left [LTLE]) and 10 controls across four…

Distilling the knowledge from large-language model for health event prediction.

Health event prediction is empowered by the rapid and wide application of electronic health records (EHR). In the Intensive Care Unit (ICU), … event prediction remains a challenge. Inspired by the strong capability in text processing of large language model (LLM), we propose the framework CKLE for health event prediction by distilling the knowledge from LLM and learning from multi-modal EHR. There are two challenges of applying LLM in the health event prediction, the first one is most LLM can onl …

Identifying Symptoms of Delirium from Clinical Narratives Using Natural Language Processing.

Delirium is an acute decline or fluctuation in attention, awareness, or other cognitive function that can lead to serious adverse outcomes. Despite the severe outcomes, delirium is frequently unrecognized and uncoded in patients' electronic health records (EHRs) due to its transient and diverse nature. Natural language processing (NLP), a key technology that extracts medical concepts from clinical narratives, has shown great potential in studies of delirium outcomes and symptoms. To assist in the diagnosis and phenotyping of delirium, we formed an expert panel to categorize diverse delirium sy…

Children with Auditory Brainstem Implants: Language Proficiency and Reading Comprehension Process.

INTRODUCTION: Auditory performance and language proficiency in young children who utilize auditory brainstem implants (ABIs) throughout the first 3 years of life are difficult to predict. ABI users have challenges as a result of delays in language proficiency and the acquisition of reading comprehension, even if ABI technology offers auditory experiences that enhance spoken language development. The aim of this study was to evaluate about the impact of language proficiency on reading comprehension skills in children with ABI. METHOD: In this study, 20 children with ABI were evaluated for thei…

Beyond tolerance toward inclusion: Response to homo- and trans-negative language.

Greater numbers of youth in Czech schools identify as sexual and gender minorities (SGM). This is occurring in a highly homogenous country that has a history of tolerance, yet limited guidelines exist on how to support these increasingly more visible communities. This provides a unique environment to examine how, when, and why educators advocate for SGM youth in the Czech Republic, and it sets the stage for more research. In this study, 114 teachers working in Czech schools completed a survey about when they intervened and facilitated discussions in response to homo-negativity and trans-negati

Person-first and identity-first disability language: Informing client centred care.

BACKGROUND: Addressing individuals with a disability can entail the use of person-first (person with a disability) or identity-first language (disabled person). However, there has been debate about use of these terms, yet there is a lack of evidence to inform preference across people with a broad range of health conditions, demographics, or health related factors. METHODS: A 42-item survey measuring demographic and health condition factors, implicit and explicit preference for person-first and identity-first language use was open for completion by individuals with a self-reported health condi…

Rectify representation bias in vision-language models for long-tailed recognition.

Natural data typically exhibits a long-tailed distribution, presenting great challenges for recognition tasks. Due to the extreme scarcity of training instances, tail classes often show inferior performance. In this paper, we investigate the problem within the trendy visual-language (VL) framework and find that the performance bottleneck mainly arises from the recognition confusion between tail classes and their highly correlated head classes. Building upon this observation, unlike previous research primarily emphasizing class frequency in addressing long-tailed issues, we take a novel perspec…

Language Skills Influence Transition Planning in Adolescents With Fragile X Syndrome.

BACKGROUND: Poor sleep is associated with longer recovery following adolescent concussion, making the longitudinal assessment of sleep important for monitoring recovery and identifying sleep disruptions. An important consideration for successful monitoring of sleep following concussion is the feasibility and adherence of a given sleep monitoring tool when used in an at-home environment. Understanding the usability of different sleep monitoring tools is essential for determining their applicability for longitudinal assessment in an ecologically valid environment. PURPOSE: The purpose of this s

Comparing classic-onset corticobasal syndrome to speech/language-onset corticobasal syndrome.

INTRODUCTION: Patients with classic-onset corticobasal syndrome (CBS) present with asymmetric limb apraxia and parkinsonism. We have, however, observed patients who initially present with speech and/or language (SL) problems and several years later develop CBS (i.e., SL-onset CBS). We aimed to compare clinical, neuroimaging and pathological characteristics of classic-onset CBS with SL-onset CBS. METHODS: We conducted a retrospective cohort study of 62 patients who met criteria for CBS (17 presented with classic-onset CBS and 45 had SL-onset CBS). We compared demographics, clinical characteris…

Large language models: a new frontier in paediatric cataract patient education.

BACKGROUND/AIMS: This was a cross-sectional comparative study. We evaluated the ability of three large language models (LLMs) (ChatGPT-3.5, ChatGPT-4, and Google Bard) to generate novel patient education materials (PEMs) and improve the readability of existing PEMs on paediatric cataract. METHODS: We compared LLMs' responses to three prompts. Prompt A requested they write a handout on paediatric cataract that was 'easily understandable by an average American.' Prompt B modified prompt A and requested the handout be written at a 'sixth-grade reading level, using the Simple Measure of Gobbledyg…

A social-semantic working-memory account for two canonical language areas.

Language and social cognition are traditionally studied as separate cognitive domains, yet accumulative studies reveal overlapping neural correlates at the left ventral temporoparietal junction (vTPJ) and the left lateral anterior temporal lobe (lATL), which have been attributed to sentence processing and social concept activation. We propose a common cognitive component underlying both effects: social-semantic working memory. We confirmed two key predictions of our hypothesis using functional MRI. First, the left vTPJ and lATL showed sensitivity to sentences only when the sentences conveyed s…

Undetected language deficits in left or right hemisphere post-stroke patients.

Previous studies have reported that widely used tests for aphasia identification are unable to detect the subtle language deficits of left hemisphere brain damaged (LHBD) individuals. Similarly, the language disorders of individuals with right hemisphere brain damage (RHBD) usually remain undetected, due to the lack of any specialized test for the evaluation of their language processing skills. The aim of the present study was to evaluate the language deficits of 80 individuals suffering from the effects of either a LHBD or RHBD stroke, who were diagnosed as having no aphasia or language defic…

Early access to language supports number mapping skills in deaf children.

The ability to associate different types of number representations referring to the same quantity (symbolic Arabic numerals, signed/spoken number words, and nonsymbolic quantities), is an important predictor of overall mathematical success. This foundational skill-mapping-has not been examined in deaf and hard-of-hearing (DHH) children. To address this gap, we studied 188 4 1/2 to 9-year-old DHH and hearing children and systematically examined the relationship between their language experiences and mapping skills. We asked whether the timing of children's language exposure (early vs. later), t…
